While travelling home from Champaign, IL on [XXX] towards Springfield, IL, we had been on the road for approximately an hour, when there was a loud metallic bang & I lost all steering control. The CR-V violently jerked to the right and exited the interstate at approximately 70mph (I was on cruise control), travelled down an embankment hitting several trees and came to a stop in trees on the edge of a farm pond. All airbags deployed, and per the Tow company, the engine and transaxles departed the vehicle and were found a short distance in front of the vehicle. The car stayed upright and continued straight after departing the roadway. what component failed & is it available for inspection - steering, exact component unknown. unknown if available for inspection, vehicle released to state farm 1/29 how was safety put at risk - caused severe crash with serious injuries. total loss of control could have been fatal if traffic was heavier has the problem reproduced - no has the vehicle been inspected by manufacturer, police, insurance etc - unknown, vehicle was released to state farm 1/29 were there any warning lamps or symptoms? - no INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The contact owns a 2020 Honda CR-V. The contact stated that while his wife was attempting to park the vehicle at 1 MPH, the vehicle independently accelerated without warning. The vehicle jumped a curb and crashed into a tree which stopped the acceleration; the contact, who was in the passenger seat, was able to place the vehicle into neutral and shut off the engine. The contact stated that the airbags did not deploy upon impact. The police were called the scene and a police report was filed. The contact was able to drive the vehicle back home despite the severe front-end damage. The contact was also able to drive the vehicle to an independent body shop as several warning lights had appeared on the instrument panel which he linked to the accident. They remained in the possession of the body shop for repair. The contact and his wife sought medical treatment from a doctor where his wife was treated for a large lump on the back of her head and the contact was diagnosed for chest contusion after having a chest X-ray. The dealer was notified of the contact of the failure and was informed that they do not perform body repairs. The manufacturer had yet to be notified of the failure. The vehicle had yet to be repaired. The failure mileage was approximately 11,500. Consumer stated the total repair cost of the car was $10,446.10.

The contact owned a 2020 Honda CR-V. The contact stated while driving approximately 15 MPH and crossing an intersection to enter a school parking lot, the contact did not see that a vehicle had merged into the opposite lane heading straight across the intersection into the path of the contact. The contact stated that she crashed into the driver's side door. The contact's vehicle spun 180 degrees and remained in the lane she was driving in. The contact stated that the air bags did not deploy. The contact stated that the front end of her hood, and grill were crushed by the impact up to the front right and left tire wheel wells. The contact struck her head on the steering wheel but had no physical injuries. The contact was not sure of the disposition of the other vehicle. The police arrived on the scene and issued the contact a traffic violation for failing to yield at the intersection. The contact was attended by an EMS team and was offered to be taken to the ER. The contact declined and was advised by EMS team that if she suffered nausea or headaches later to go to an ER immediately. The contact stated that later that day she was driven to the ER by her husband. The ER physician determined that she had suffered a concussion. The contact's vehicle was towed to an impound lot and later declared a total loss by her insurance provider. The manufacturer was informed of the failure. The failure mileage was approximately 40,000.

My car, a 2020 Honda CR-V EX 1.5T AWD, was totaled in a fatal accident on a rainy night on an interstate highway. It spun on the highway a couple of turns, hit the highway barrier at all of its corners and sides, caused the airbags to all came out, and eventually deadly stopped in the middle of the highway on a rainy night. My wife and I were in our undrivable vehicle, watching countless coming vehicles approaching us rapidly. We were just waiting in the dead car unmovable, just as in a coffin for our fate to come but can do nothing. Thank GOD who made someone call 911. Thanks to the emergency rescue team. The ambulance sent us to a nearby local hospital. We bought the vehicle, the 2020 Honda CR-V EX 1.5T AWD, as new, and we realized there could be a problem at a very earlier time. It has a serious problem with its AWD control system. The rear wheels are always engaged in all road conditions, as it is displayed on the instrument panel. This causes the two symptoms of the car, (1) the MPG (miles-per-gallon) is extremely lower than the standard 34 MPG on the highway. It is only 23 to 25 MPG because it always consumes about 32 to 36 gallons of gasoline on a trip of 810 miles distance (85% on interstate highways). (2) the rear tires are worn out extremely fast. It wore 86% of the usage range at 22,000 miles usage, which is about 2 to 3 times quicker than tires on other cars on average. We visited the Honda dealer in Troy, Michigan for the tire problems on May 9, 2022, which is about five months before the severe accident. The Honda dealer refused to give any compensation for changing new tires. The car is just over 25,000 mileage (see [XXX] ). On November 9, 2022, I was making a left turn while I had a green light. A car coming from the left did not stop at the red light he had and t-boned my car on the driver side. My airbags deployed as they should. However, when I came to, I smelled smoke which was coming from the airbags. I touched my forehead and realized that something had cut my face. However, no glass from my car (windows, mirrors) was broken. I have no idea where the piece of glass came from but suspect it came from one of the airbags that deployed. While this was a small piece of glass (simply noted as an abrasion by the ER), I was not severely injured by it (however, I do have a scar from the incision). But I think this is worth investigating in case there is glass in other vehicle airbags that could more seriously injure someone. The vehicle was totaled by the insurance company and was sold at auction. I am sure it could be tracked down if necessary. Because the car was totaled, it is not able to be tested. The vehicle was inspected by Honda for repair estimates only. There were no warning notices or other symptoms of the problem prior to the accident air bag deployment which shot a small piece of glass and other debris into my face and the vehicle. The debris shot into the vehicle consists or small pieces of paper or fabric.

The contact stated while at a complete stop, another vehicle going at high speeds crashed into the front end of the passenger's side of the vehicle, and the driver's side air bag deployed abnormally. The air bag exploded and propelled a chemical powder that burned the contact's right hand, throat, chest, and caused the contact ears to ring. Additionally, there was smoke inside the vehicle. A police report was filed. The contact was able to exit the vehicle. The contact was transported to the hospital with the assistance of the emergency units. The contact was still receiving medical assistance. The vehicle was towed to a an auto repair body shop. The dealer and the manufacturer were not notified of the failure. The vehicle was not diagnosed nor repaired. The failure mileage was approximately 18,000.

I was in an accident this morning on the freeway where they are doing road construction. The lanes are VERY narrow due to cement wall placed for construction. The vehicle in front of me slammed on their brakes suddenly so when i did the Automatic brakes kicked in on my car surpassing the level of my own braking and took over. The car came to a compelete stop. IT was terrifying because i didnt have control to keep moving to try and get out of the way from the semi driving behind me. I moved over as much as i could even with a cement wall so close. I ended up getting rear ended by the Semi. This automatic braking system is DANGEROUS. With the car taking over my brakes lost me time to try and get out of the way even more which ccould have possibly prevented the accident from happening.

When I was driving my vehicle on 7-21-22 at approximately 4:20pm my emergency breaking system was flashing and breaking on it's own. There was nothing in front of my vehicle at the time. Later at approximately 4:25pm a woman hit my passenger side after running a red light. My passenger side airbag did not deploy. However, about 5-10 seconds after impact my steering wheel airbag deployed and hit me in the left side of my face. The delayed reaction has caused lacerations on my face, burns on my hand, bruising and loss of part of my tooth. I am also suffering from head issues and pain in my chest from this delayed airbag deployment. Vehicle had to be towed away to a garage and is not drivable.

Vehicle was parked on a slight incline with transmission in Park and electronic parking brake engaged. Engine was running. While unloading groceries, driver leaned into vehicle and pressed the engine start/stop button to turn the vehicle off. Immediately after engine shutoff, the vehicle began rolling backward. Driver attempted to re-engage Park and electronic parking brake multiple times without effect. Vehicle continued rolling for approximately 20–30 seconds before stopping. Driver’s foot was run over by the vehicle, resulting in injury. No warning messages were observed prior to the incident. Cause unknown. Vehicle remains available for inspection.
