DT: CONSUMER WAS IN A CAR ACCIDENT ON AUGUST 17, 2005 AND SIDE/CURTAIN AIR BAGS DID NOT DEPLOY. IT WAS SIDE IMPACT AT 45 MPH. HAD NOT CONTACTED TOYOTA AT THIS POINT. *AK
The contact's client owned a 2006 Toyota Highlander. The contact stated while his client was driving at an unknown speed when the front passenger tire blowout and he crashed into a tree. The contact stated the front driver side air bag did deploy but the front passenger-side air bag failed to deploy causing the client's wife to have a fractured clavicle and spine, and internal injuries. The contact stated the wife was having a third surgery on 10/6/2021. The driver sustained 6 fractured ribs and did seek medical attention at a hospital. The contact also stated the client was traveling with a dog who was injured in the accident and decided to put down the dog due to the extended injuries. A police report was filed. The vehicle was not drivable. The vehicle was towed to towing lot. The contact called the local dealer and spoke to them about a possible air bag recall inquiry. The manufacturer had been informed of the failure. The failure mileage was approximately 120,000.

I NOTICED HESITATION IN GEAR SHIFTING AND SOME LUNGING WHEN MY FOOT WAS OFF THE BRAKE AFTER I PURCHASED A NEW 2006 HIGHLANDER IN JULY 2006 AND MADE A MENTAL NOTE TO HAVE IT CHECKED AT THE FIRST FOLLOW-UP SERVICE. IN OCTOBER 2006, I WAS PULLING OUT OF A POST OFFICE PARKING LOT WHILE AWARE OF 2 CARS AT A SAFE DISTANCE AWAY. WHEN IN THE INTERSECTION, 1 OF THE 2 CARS WAS TRAVELING MUCH FASTER THAN ANTICIPATED. WHEN I ACCELERATED QUICKLY, THE CAR JUST SEEMED TO CHOKE, AND THE CAR COLLIDED INTO MY REAR SIDE (DRIVER'S SIDE). I AND THE OTHER DRIVER HAD ONLY MINOR INJURIES, BUT THE COLLISION WAS VERY FRIGHTENING AND IT COULD HAVE BEEN MUCH WORSE. I ALSO FELT THAT SOMETHING MORE SERIOUS WAS WRONG WITH MY CAR THAN I HAD THOUGHT. AFTER I GOT MY CAR BACK FROM THE BODY REPAIR SHOP (7 WEEKS LATER), I TOOK IT INTO TO THE DEALER AND ASKED THEM TO CHECK THE PROBLEM. I WAS TOLD ALL OF THE HIGHLANDERS HAVE THAT HESITATION IF THE GAS PEDAL IS PRESSED HARD. I WAS TOLD TO PRESS DOWN ONLY GRADUALLY. SINCE THEN, I CONTINUE TO NOTICE THE PROBLEM WITH MY CAR. IT MAKES ME FEEL VERY UNSAFE AND AM NOT SURE HOW TO RESOLVE THE ISSUE OTHER THAN TO SELL THE CAR. I AM WORRIED HOWEVER THAT OTHERS MAY BE AT RISK BECAUSE OF THIS FEATURE WITH THE TRANSMISSION. *NM
FROM A PARKED POSITION, I STARTED ENGINE ON 2006 TOYOTA HIGHLANDER. THE CAR MOVED FORWARD FOR A FEW SECONDS AT SLOW SPEED, THEN VERY RAPIDLY ACCELERATED SO THAT I COULD NOT STOP THE CAR. I WAS DRIVING MY CAR INTO AN UNDERGROUND PARKING LOT. I PUT ON THE BRAKES AND THEY WOULD NOT ENGAGE. I TRIED AGAIN. I EVENTUALLY SLAMMED INTO THE WALL AT THE END OF THE PARKING LOT. MY CAR CAUGHT ON FIRE AND I WAS HURT, TAKEN TO THE HOSPITAL. THE CAR WAS INSPECTED AND FOUND TO HAVE NO BRAKE PROBLEMS, SO I WAS DEEMED AT FAULT FOR THE ACCIDENT. THIS WAS BEFORE WE FOUND OUT ABOUT THE FLOOR MAT AND ACCELERATOR PROBLEMS WITH TOYOTAS. MY INCIDENT WAS OBVIOUSLY ONE OF A RUNAWAY CAR. IT WAS A 2007 TOYOTA HIGHLANDER. *TR

AS A RESULT, THE VEHICLE STRUCK A PEDESTRIAN. A POLICE REPORT WAS FILED. THE PEDESTRIAN SUFFERED INJURIES AND MEDICAL ATTENTION WAS REQUIRED. SUDDEN ACCELERATION. VEHICLE WAS STOPPED, WITH FOOT ON BRAKE, WHEN SUDDEN ACCELERATION OCCURRED VERY RAPIDLY. BRAKE PRESSURE COULD NOT KEEP VEHICLE FROM LURCHING FOWARD AND HITTING THE REAR A OF STOPPED VEHICLE. I NEVER USE MY LEFT FOOT ON THE BRAKE, SO I CAN NEVER HIT THE BRAKE AND ACCELERATOR SIMULTANEOUSLY.
I SWERVED TO AVOID A DEER AND HIT A CONCRETE ABUTMENT ON THE SIDE OF THE ROAD. THE TOYOTA HIGHLANDER WAS TOTALED .THE HIGHLANDER AIRBAG DID NOT WORK PROPERLY; I HIT THE STEERING WHEEL COLUMN VERY HARD AND RECEIVED A CONCUSSION. ALSO A DAMAGED DISK IN MY NECK, WHIPLASH, CHEST CONTUSIONS, AND FACIAL ABRASIONS. *TR

THE CONTACT STATED THAT WHILE DRIVING APPROXIMATELY 50 MPH, THE BRAKE PEDAL WAS DEPRESSED CONTINUOUSLY HOWEVER, THE VEHICLE CONTINUED TO ACCELERATE RAPIDLY. THE CONTACT CRASHED INTO THE REAR OF ANOTHER VEHICLE, CAUSING A CHAIN REACTION CRASH. THE AIR BAGS DEPLOYED AND BRUISED THE CONTACT'S LEFT ARM, CHEST AND NECK. THE CONTACT ALSO SUSTAINED MULTIPLE BRUISES FROM THE SEAT BELT. A POLICE REPORT WAS FILED OF THE INCIDENT. THE VEHICLE WAS TOWED TO A COLLISION CENTER FOR INSPECTION OF THE MASSIVE FRONT END DAMAGES. THE MANUFACTURER WAS NOT NOTIFIED OF THE INCIDENT. THE APPROXIMATE FAILURE MILEAGE WAS 99,000.

Southeast Toyota Distributors, LLC (SET) is recalling certain 2005-2011 4Runner, Highlander, Sequoia, and Sienna, 2005-2010 Avalon and Tundra, 2006-2008 Camry Solara, 2006-2010 Highlander hybrid, 2006-2011 Rav4 and Tacoma, 2007-2010 FJ Cruiser, 2007-2011 Camry hybrid, 2009-2010 Venza, 2010-2012 Prius, 1988-1990 and 2005-2010 Camry, and 1989 and 2009-2010 Corolla vehicles equipped with aftermarket accessory seat heaters with a copper strand heating element. The electrical wiring in the seat heaters may be damaged when the seat cushion is compressed.
Remedy: SET will notify owners, and dealers will disconnect the seat heaters, free of charge, and refund the purchase price of the seat heater accessory. The recall began on July 14, 2016. Owners may contact SET customer service at 1-866-405-4226. SET number for this recall is SET16B.

The affected vehicles may experience compression of the seat cushion which may damage the seat heater wiring.
Remedy: SET will notify owners, and dealers will disconnect the heaters with copper strand heating elements and refund the purchase price of the seat heater accessory, free of charge. The recall is expected to began in January 2015. Owners may contact SET customer service at 1-866-405-4226.

IF THE FORWARD RETENTION CLIP USED TO SECURE THE FLOOR CARPET COVER, WHICH IS LOCATED IN FRONT OF THE CENTER CONSOLE, IS NOT INSTALLED PROPERLY THE COVER MAY LEAN TOWARD THE ACCELERATOR PEDAL AND INTERFERE WITH THE ACCELERATOR PEDAL ARM.

BRAKE FLUID CONTAINING POLYMERS IS USED THAT ACTS AS LUBRICANTS FOR CERTAIN BRAKE SYSTEM COMPONENTS. IF REPLACEMENT BRAKE FLUID IS USED THAT DOES NOT CONTAIN SUCH POLYMERS, OR THAT CONTAINS ONLY SMALL AMOUNTS, A PART OF THE RUBBER SEAL LOCATED AT THE REAR OF THE BRAKE MASTER CYLINDER MAY BECOME DRY, AND THE SEAL MAY CURL DURING MOVEMENT OF THE PISTON. IF THIS OCCURS, A SMALL AMOUNT OF BRAKE FLUID COULD SLOWLY LEAK FROM THE SEAL INTO THE BRAKE BOOSTER.
