The contact owns a 2017 Toyota Corolla. The contact received notification of NHTSA Campaign Number: 20V024000 (Air Bags). The contact stated that while driving at 15-20 MPH, the forward collision avoidance feature failed to alert her of vehicles ahead. The contact swerved the vehicle, crashing into the bridge wall. The air bags did not deploy. Several unknown warning lights were illuminated. The contact suffered bruising and neck pain, but no medical attention was received. The contact's mother, who was a passenger, suffered body aches and pain. The contact stated that her mother passed away from an unrelated illness a month after the crash. There was no police report filed. The vehicle was able to be driven away from the crash site. Furthermore, the contact stated that the vehicle was equipped with unknown tires, and that the front driver's side tire and spare tire had a blowout four times within a year. The front driver side tire was replaced by a tire retailer, Walmart, and an independent mechanic, but they were unable to determine the cause of the tire failure. The contact stated that after the crash, the front driver's side tire was flat, the rearview camera was inoperable, and the front passenger door handle had fractured and detached. The message "No Rear Visibility" was displayed. The vehicle was towed to the dealer, and the front bumper detached while being towed. The dealer informed the contact that the recall was not related to the failures. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure and had an engineer inspect the vehicle. The engineer determined that the rearview camera and forward collision avoidance feature had failed. The failure mileage was approximately 60,000.

The vehicle was towed to a tow lot and deemed a total loss by the insurance company. The dealer or the manufacturer was not notified of the failure. The approximate failure mileage was 65,000.

A POLICE REPORT WAS FILED AND THE VEHICLE WAS TOTALED BY STATE FARM. THE WRECKAGE WAS TOWED TO A SALVAGE YARD. THE MANUFACTURER AND DEALER WHERE NOT CALLED BY THE CONTACT. THE FAILURE MILEAGE WAS 27,000.

While I was driving in the middle lane through an intersection, a car from the opposite side failed to yield while turning and hit me on my driver's side. My car was traveling at approximately 35 MPH, and the other vehicle was exceeding the speed limit. The impact caused my car to veer in the opposite direction and I lost consciousness immediately. The fire department had to extract me from the passenger side door as my driver's side door was jammed shut. Both the driver's door and the rear door on that side cannot be opened, rendering the car undriveable and necessitating towing from the crash scene. The collision was severe, causing my head to snap back and forth against the steering wheel, along with impact to the left side of my body and whiplash to my neck, back, and chest. I was taken to the nearest ER by ambulance, experiencing bruises as well as severe pain in my head, neck, back, and chest, making breathing difficult. Despite the intensity of the crash, the airbags did not deploy, leading to further jolting of my body against the car interior. I had bought the car directly from a Toyota dealer, but I have not been contacted by Toyota regarding any potential manufacturing defects. The failure of the airbags to deploy after such a significant impact and the speed of the other vehicle has raised concerns about the safety of my car, as I was under the impression that it was free of defects.

There was no reported fire. A police report was filed. The vehicle was towed to an unknown location. The frame and fuel tank were bent. The vehicle was towed to the local dealer, who confirmed no recalls. Six months after the failure, the vehicle was taken to the same dealer who repaired the vehicle under NHTSA Campaign Number: 20V024000 (Air Bags). The contact believed that the vehicle was sold with an open recall. The vehicle was repaired. The manufacturer was contacted, who opened a case, but no assistance was provided. The failure mileage was approximately 9,800.

I was t-boned by a driver on 5/16/2023. Since this occurred, my vehicle has been repaired. However I have a shake and hoovering noise coming from the front of the vehicle. It has been aligned, tires have been rotated, and suspension has been checked. My mechanic can find no fault. However he has stated that he can feel the shake and can hear the noise. He also says it pulls to the left really hard when accelerating. These issues were not present before my accident. Also my seatbelts on my driver and passenger side were replaced shortly after my accident. I felt like this wasn't checked by the body shop.

Twice within one month my Corolla experienced Sudden Unintended Acceleration. The first time I had just entered the parking lot of my local Star Market. I stopped for a man in the crosswalk and was just about to ease off the break when I saw a woman approaching the crosswalk so I stayed on my break. Suddenty the car revved up and went forward. I think it was so loud the woman heard it and stopped. My car returned to normal operation within maybe five seconds and as I drove by the woman, I yelled there was something wrong. Went home and scheduled an appointment with Toyota. It took a few days for them to check my car. I think this type of issue brings in the next level up of repair. I was told they went over everything and could find nothing wrong. My car was safe to drive. I drove it for exactly one month and it did the same thing. This time I was inching my way into a parking space that was up against a wall. Nowhere near the gas. I was maybe a couple of feet from the wall when the car accelerated again. The engine really revved up again and I went into the wall. The car was drivable, and I got it right back to Toyota. It is still sitting there pending investigation. I am waiting to hear from someone from their liability department. I'm not sure what will happen, but I know I will not drive that car again. My biggest concern is that Toyota will claim I'm elderly and hit the gas, not the brake. That is not what happened.

I was driving southbound on a main road when a car pulled out right Infront of me. This caused me to hit the left side of his car between the front and back door (Nissan Sedan) with the front right of my car. I hit his car at roughly 45mph. My car (Toyota Corolla) did not deploy any airbags. His car almost all the airbags went off. My car was totaled. The front right wheel was completely ripped apart, the fluids from around the engine all spilled out, and the front right door does not open. There is a police report filed and the accident is filed with my insurance (Progressive). The car was inspected by insurance just to determine if it is totaled and the value it's worth. There were never any warnings from my car and the car was regularly maintained. I believe my safety was at risk by the airbags not going off. I hit my face on the steering wheel causing injury to my jaw.

AIR BAGS: AIR BAG/RESTRAINT CONTROL MODULE
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2011-2019 Corolla, 2011-2013 Matrix, 2012-2018 Avalon, and 2013-2018 Avalon Hybrid vehicles. During certain crashes, the air bag electronic control unit (ECU) may malfunction, possibly disabling the deployment of the air bags and/or seat belt pretensioners.
Abhilfe: Toyota will notify owners, and dealers will inspect the ECU and install a noise filter between the air bag control module and its wire harness, as necessary, free of charge. Owners were notified of the safety risk beginning March 2, 2020. A second letter notifying owners of the remedy repair will be mailed between March 16, 2020 and June 27, 2020. Owners may contact Toyota customer service at 1-888-270-9371. Toyota's numbers for this recall are 20TB03, 20TA03 and 20TA05.

The load carrying capacity modification label may be incorrect. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 110, "Tire Selection and Rims."

The spare tire air pressure was not adjusted to the proper pressure as stated on the Tire Pressure Label.
Abhilfe: Gulf States will notify owners, and dealers will inspect the spare tires, adjusting their pressure as necessary, free of charge. The recall began June 15, 2017. Owners may contact Gulf States customer service at 1-800-444-1074. Toyota's number for this recall is 17R2.
