PRIUS V TRAVELING N ON OUTSIDE LANE CROSSED INSIDE LANE AND 11' GRASS MEDIAN BORDERED BY CONCRETE INTO S BOUND INSIDE LANE AND COLLIDED MY MOTHER DRIVING ONCOMING VEHICLE ON INSIDE S BOUND LANE. WOMAN DRIVING PRIUS V WAS KILLED ON IMPACT, AND MY MOTHER DIED LATER THAT DAY AT UF HOSPITAL JACKSONVILLE. TWO WITNESSES DRIVING BEHIND MY MOTHER SAW CRASH AND SAID THE PRIUS V APPEARED TO BE ACCELERATING ACROSS MEDIAN. CRASH OCCURRED APPROX. 2 MONTHS BEFORE TOYOTA'S RECALL ON JULY 15, 2015 OF 109,000 PRIUS V HYBRIDS. THE PRIUS V THAT KILLED MY MOTHER WAS PART OF THIS RECALL. IN FEB 2014, THERE WAS A RECALL ON THE PRIUS ECU. THE PRUIS V HAD THE SAME ECU, BUT TOYOTA CHOSE NOT TO RECALL THE PRIUS V UNTIL ALMOST A YEAR AND A HALF LATER, 2 AND A HALF MONTHS AFTER MY MOTHER'S FATAL CRASH. IF THEY HAD RECALLED THE PRIUS V IN 2014, WOULD MY MOTHER STILL BE ALIVE?
I WAS COMING UP THE FREEWAY OFF RAMP AND MY CAR JUST TURNED OFF I HAD NO BRAKING I HIT SIX OTHER CARS ROLLED OVER AND WAS SEVERLY IMJURED IN THE HOSPITAL FORR THREE DAYS
CAR WAS INVOLVED IN A 18+ CAR ACCIDENT. TRAVELING AT APPROX 55MPH ALL TRAFFIC CAME TO A SUDDEN STOP. THE CAR DID STOP BEFORE IMPACT. DRIVER PRESSED HIMSELF TO THE SEAT WHILE SLAMMING ON BREAKS. A CAR SLAMMED INTO THE BACK OF THE PRIUS WHICH SHOT THE PRIUS FORWARD, WEDGING IT BETWEEN THE 2 VEHICLES IN FRONT OF IT. UPON IMPACT WITH THE CARS IT WAS SLAMMED INTO THE DRIVER'S SEATBELT DID NOT LOCK. DRIVER FLEW FORWARD BREAKING BONES IN HIS FACE AND OBTAINED INJURIES TO A KNEE FROM IMPACTING THE CONSOLE, AND WHIPLASH. A CHILD PASSENGER IN THE BACK SEAT OBTAINED A KNOT ON HEAD AND WHIPLASH. AIR BAGS DID NOT DEPLOY. THE BELT ELR DID NOT ENGAGE. THE CAR WAS DEEMED TOTALED.
The contact owned a 2012 Toyota Prius. The contact stated that while making a left turn at 10 MPH, another vehicle crashed into the vehicle and t-boned the front passenger's side door of the vehicle. The air bags did not deploy. There was no warning light illuminated. After the impact, the front windshield slowly cracked. A Police report was filed. The contact and her husband were transported by ambulance to the hospital. The contact suffered a fractured sternum, five fractured ribs, a crushed left wrist, and two fractured bones in the left ankle. The contact's husband had a fractured neck and five fractured ribs. The vehicle was towed to a tow yard and was declared a total loss by the insurance company. The manufacturer was not made aware of the failure. The failure mileage was approximately 140,000.
I WAS STOPPED AT A GREEN LIGHT WAITING FOR THE CAR IN FRONT ME; WHEN THEY TURNED LEFT AN ONCOMING CAR SWERVED TO MISS THEM, THEN OVERCORRECTED AND HIT ME. HIT ME HEAD ON; ON THE FRONT RIGHT SIDE. THEY HIT ME WITH SO MUCH FORCE THEY MOVED MY CAR ALMOST 6 FEET SIDEWAYS. NONE OF THE MY AIRBAGS WENT OFF AND MY SEAT BELT DID NOT LOCK. I HIT THE STEERING WHEEL WITH SUCH FORCE I HAD THE TOYOTA SYMBOL IMPRINTED ON MY CHEST FOR A DAY. (I WAS WEARING 3 LAYERS A JACKET AND 2 SHIRTS).WHY DID NONE OF THE SAFETY FEATURES WORK?
TL* THE CONTACT OWNS A 2012 TOYOTA PRIUS. THE CONTACT STATED THAT THE VEHICLE INDEPENDENTLY ACCELERATED WHILE PULLING INTO A PARKING SPACE WITH THE BRAKE PEDAL APPLIED. THE CONTACT APPLIED EXTREME FORCE AND THE VEHICLE CONTINUED TO UNINTENDEDLY ACCELERATE AT A HIGH RATE OF SPEED. THE VEHICLE TURNED RIGHT AND CRASHED INTO A TRASH DUMPSTER. THE DRIVER AND FRONT PASSENGER SUSTAINED WHIPLASH. MEDICAL ATTENTION WAS NOT REQUIRED. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO THE DEALER'S COLLISION CENTER WHO REPAIRED THE BODY DAMAGES AND WAS TO SEND A THIRD PARTY TO INVESTIGATE THE CAUSE OF THE FAILURE. THE CONTACT WAS WAITING ON THE DEALER AND THE MANUFACTURER TO DETERMINE THE REMEDY TO REPAIR THE VEHICLE. THE APPROXIMATE FAILURE MILEAGE WAS 20,000.
MY WIFE WAS INVOLVED IN A TWO CAR CRASH WHEN A CAR COMING IN THE OPPOSITE DIRECTION MADE AN ILLEGAL LEFT TURN DIRECTLY IN FRONT OF HER. IT HAPPENED SO QUICK THAT SHE NEVER HAD TIME TO HIT HER BRAKES. SHE WAS TRAVELING BETWEEN 30 AND 35 MPH IN A 35 MPH ZONE ON A TWO LANE ROAD. I WOULD THINK THAT AT THAT SPEED IF ALL HER SAFETY EQUIPMENT WORKED SHE WOULD NOT SUFFER ANY INJURIES. HER SEAT BELT FAILED CAUSING THE SEAT LATCH TO FAIL AND SHE ENDED UP JAMMED INTO THE BOTTOM AND UNDER SIDE OF THE DASHBOARD. SHE SUFFERED A BLOW TO HER CHEST AND INJURIES TO HER KNEES, LOWER LEGS AND FOOT. SHE HAD SURGERY ON HER RIGHT KNEE SO WE COULD GET HER ON HER FEET AGAIN AND IS AWAITING SURGERY ON HER RIGHT SHOULDER. SHE HAD A PRE EXISTING HEART CONDITION THAT WAS RAISED TO EMERGENCY STATUS AFTER THE BLOW TO HER CHEST AND SHE HAD SURGERY FOR THAT AND THEY FOUND A SHORT CIRCUIT IN THE MIDDLE OF HER HEART AND ZAPPED IT. THERE IS ONLY A 2% CHANCE IT WILL EVER RETURN. I HAVE PICTURES OF THE FAILED SEAT BELT AND SEAT. THE SEAT BELT IS IN THE FULLY EXTENDED POSITION AND LOCKED AND CAN NOT BE RELEASED EVEN BY YANKING ON IT. I AM WRITING THIS REPORT SO THIS SITUATION CAN BE CHECKED AND IF FOUND FAULTY CAN BE CORRECTED BEFORE ANY ONE ELSE GOES THROUGH WHAT MY WIFE IS GOING THROUGH.
THE BRAKES WERE APPLIED GENTLY AS THE CAR WAS APPROACHING A STOPPED CAR. THE PRIUS C THEN ACCELERATED, AT WHICH TIME THE BRAKES WERE FIRMLY APPLIED, THE ABS SYSTEM ENGAGED, BUT NOT IN ENOUGH TIME FOR THE VEHICLE TO STOP. THE PRIUS REAR ENDED THE VEHICLE IN FRONT, ALL AIRBAGS WERE DEPLOYED CAUSING INJURY TO THE DRIVER, AND THE FRONT END OF THE PRIUS WAS DESTROYED. IT CANNOT BE FIXED AND WILL NEED TO BE SCRAPPED. *TR
ON MY WAY HOME I SUDDENLY SAW A SIGN ON THE SCREEN "CHECK THE HYBRID SYSTEM. STOP IN A SAFE PLACE". I PULLED OVER, SMELLED SMOKE AND SAW SMOKE IN THE REAR. I OPENED THE TRUNK ANS FRIGHTENED BY FLAME. I PULLED THINGS OUT AND COVERED THE FLAME WITH TWO BIG TOWELS AND SUCCEEDED IN PUTTING IT OUT, ALTHOUGH I GOT BURNED ON MY FINGERS. I CALLED GEICO. THEY SENT A TOWING CAR AND IT TOOK MY CAR TO MY APARTMENT. WHEN GEICO TOOK MY CAR TO A COLLISION CENTER, THEY SAID IT IS TOTAL LOSS AFTER INSPECTING IT. I CLAIMED IT TO TOYOTA. THEY INSPECTED IT, BUT SAID THAT THERE WAS NO EVIDENCE THAT THE CAR HAD ELECTRICAL OR MECHANICAL DEFECT. I DID NOTHING BUT DRIVING THE CAR, BUT IT CAUGHT FIRE. WHOSE RESPONSIBILITY IS THAT? I BELIEVE THE PRODUCER SHOULD TAKE THE RESPONSIBILITY BUT THEY DO NOT. WHERE CAN I BE COMPENSATED FOR THE LOSS?
On February 27 2024 I had spend 2300 dollars on car repairs see enclosed then on April 11 2024 while backing out of my driveway, I experienced a sudden acceleration of my vehicle which led to body damage to my car yet Toyota after inspection says that now the car system works perfectly. Prior to this incident I experienced warning light and windshield wiper malfunction .
Broken bones in face. Nose split in two parts Eye socket broken along with cornea messed up. Broken ribs. Bleeding spleen. Broken neck.
The front and driver side air bags did not deploy (although the passenger side air bag did deploy). Initial impact came from behind, toward the passenger side, causing the vehicle to turn fully sideways, where the vehicle was then t-boned on the driver's side. The lack of driver side air bag deployment resulted in a lot of glass making it into the left side of the driver's head.
I SWITCHED INTO NEUTRAL AND PUSHED THE PARK BUTTON TO PARK. I LEANED FORWARD TO GRAB MY WALLET AND FELT THE CAR ACCELERATING OVER THE PARKING CURB. I IMMEDIATELY SLAMMED THE BRAKES AND THE CAR ACCELERATED INTO A BUILDING. I AM FORTUNATE TO BE ALIVE BECAUSE IF THE AIRBAG WOULD HAVE ACTIVATED, I WOULD HAVE BROKE MY NECK.

DRIVER SUSPECTED CARBON MONOXIDE POISONING. SUBSEQUENTLY, DRIVER PLACES RESIDENTIAL CARBON MONOXIDE DETECTOR INSIDE PRIUS WHICH ALARMED WHILE PARKED AND TURNED OFF AT RESIDENCE DRIVEWAY. MY FRONT AND DRIVER'S SIDE AIR BAGS DIDN'T OPEN WHEN MY CAR WAS HIT ON THAT SIDE BY ANOTHER CAR DRIVEN BY A LADY WHO RAN A STOP SIGN; AND STRUCK MY MOVING CAR; TOTALING MY CAR. I SUFFERED SERTOUS CHEST, NECK AND BACK SPRAINS THAT STILL L BOTHER ME. I WAS STRUCK PRETTY HARD, I SUFFERED A TERRIBLE JOLT TO MY CHEST AREA WHEN MY CAR CAME TO A HARD STOP, AND I STILL HIT THE STEERING WHEEL. THIS ACCIDENT COSTS ME MY JOB AND I AM NO LONGER ABLE TO STAND OR WALK FOR LONG PERIODS OF TIME.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2010-2014 Toyota Prius and 2012-2014 Toyota Prius V vehicles. Excessive voltage in the Intelligent Power Module (IPM) within the inverter may cause the hybrid system to shut down, causing the vehicle to stall while being driven.
Remedy: Toyota will notify owners, and dealers will update the software for the motor/generator control electronic control unit (ECU) and the hybrid control ECU as necessary, free of charge. The recall began January 22, 2019. Owners may contact Toyota customer service at 1-888-270-9371. Toyota's number for this recall is J0V. Note: The software updates in this recall are needed even if the vehicle had the software previously updated under recalls 14V-053 or 15V-449.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2010-2012 Toyota Prius, 2010 and 2012 Toyota Prius Plug-in Hybrid, and 2011-2012 Lexus CT200h vehicles. A weld in the curtain shield air bag inflator may crack causing one or both portions of the inflator to eject into the passenger cabin.
Remedy: Toyota will notify owners, and dealers will install retention brackets to the curtain shield air bags, free of charge. The recall began on October 17, 2016. Owners may contact Toyota Customer Service Department at 1-800-331-4331. Toyota's number for this recall is Toyota G0U, and Lexus GLJ.
Southeast Toyota Distributors, LLC (SET) is recalling certain 2005-2011 4Runner, Highlander, Sequoia, and Sienna, 2005-2010 Avalon and Tundra, 2006-2008 Camry Solara, 2006-2010 Highlander hybrid, 2006-2011 Rav4 and Tacoma, 2007-2010 FJ Cruiser, 2007-2011 Camry hybrid, 2009-2010 Venza, 2010-2012 Prius, 1988-1990 and 2005-2010 Camry, and 1989 and 2009-2010 Corolla vehicles equipped with aftermarket accessory seat heaters with a copper strand heating element. The electrical wiring in the seat heaters may be damaged when the seat cushion is compressed.
Remedy: SET will notify owners, and dealers will disconnect the seat heaters, free of charge, and refund the purchase price of the seat heater accessory. The recall began on July 14, 2016. Owners may contact SET customer service at 1-866-405-4226. SET number for this recall is SET16B.
Toyota is recalling certain model year 2010 through 2014 Prius vehicles. In the affected vehicles, the Intelligent Power Module (IPM) inside the inverter module (a component of the hybrid system) contains transistors that may become damaged from high operating temperatures. If this occurs, various warning lamps will be illuminated on the instrument panel and the vehicle will have reduced power allowing it to only drive a short distance.
Remedy: Toyota will notify owners and dealers will update the software for both the motor/generator control electronic control unit (ECU) and the hybrid control ECU, free of charge. If an owner experiences a failure of the inverter before the vehicle receives updated software, the dealer will repair or replace the inverter assembly with a new one at no charge. The recall began on March 21, 2014. Owners may contact Toyota at 1-800-331-4331.
Southeast Toyota is recalling certain model year 2008 and 2010-2013 Toyota Tundra, 2010-2012 Rav4, 2012 Toyota Sequoia, 2010-2011 Toyota Corolla, 2010-2011 Toyota Camry and Camry Hybrid, 2010-2013 Toyota Highlander and Highlander Hybrid, 2010-2013 Toyota FJ Cruiser, 2011 Toyota Land Cruiser, 2010-2013 Toyota Venza, 2010-2011 Toyota 4Runner, 2010-2013 Toyota Tacoma, 2011-2012 Toyota Sienna, 2012 Toyota Prius, 2013 Scion FR-S, 2011 Scion XD, 2011 Scion XB, and 2012 Scion TC vehicles. These vehicles were sold with labels that were outside the allowable one percent of accuracy of actual weight added. Thus, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) Number 110, "Tire Selection and Rims."
Remedy: Southeast Toyota will notify owners and provide a corrected label with instructions concerning its installation. Southeast Toyota Distributors, LLC (SET) is recalling certain models interspersed through model years 2009 through 2013 as follows: model year 2009-2012 Tacoma, 4Runner, Camry, Camry Hybrid, Prius, and RAV4; model year 2009-2010 Avalon, FJ Cruiser, and Highlander Hybrid; model year 2010-2013 model year Corolla, Sienna and Tundra; model year 2009-2013 Highlander and Venza; model year 2012 Prius V; and model year 2010-2012 Sequoia. During modification by SET to include accessories such as leather seat covers, seat heaters or headrest DVD systems, these vehicles may not have had the passenger seat occupant sensing system calibration tested. Without passing the calibration test, the occupant sensing system may not operate as designed.
Remedy: Southeast Toyota will notify owners, and dealers will test the sensitivity of the occupant detection sensors, and recalibrate them as necessary. The recall began on March 21, 2013. Owners may contact Southeast Toyota at 1-800-301-6859.

Due to a software issue, the Parking Assist control unit that works with the Panoramic View Monitor (PVM) to show the rearview camera image may briefly freeze when backing up if reverse gear is selected shortly after starting the vehicle. In addition, the rearview camera may not appear the next time the vehicle is started if the vehicle is turned on and off again within a short time window. This may increase the risk of a crash with a person or another vehicle during a backing event.
