Incident: [XXX] Conditions: Clear daylight, dry pavement, normal highway conditions Crash/Injuries: Allstate Claim #[XXX]; several people injured At approximately [XXX], I was driving my 2020 Toyota Highlander Hybrid at about 55 mph in the slow/right lane. The vehicle ahead suddenly made a hard stop. I immediately and forcefully applied the brakes for an emergency stop. My front-seat passenger, [XXX], also saw the danger and yelled for me to stop at the same time. Based on our recollection, approximately five seconds elapsed between his warning and the collision. Despite the rapidly closing distance, the Toyota Pre-Collision System (PCS) did not appear to activate. There was no audible warning/beeping, no flashing red warning, and no “BRAKE!” indicator on the dashboard. This was also observed by [XXX] and [XXX], who was seated in the second row and is particularly attentive to warning signals. The vehicle also did not appear to provide any automatic emergency braking or brake-assist intervention. Despite my immediate and forceful braking, the vehicle did not stop in time and collided with the vehicle ahead, causing significant damage and injuries. The forward-facing sensors and windshield were clean and unobstructed. There were no dashboard warning lights or known system malfunction indicators before the crash. I purchased the Toyota Plus service package after completing the required maintenance during the initial coverage period. All scheduled maintenance and inspections were performed on time by certified Toyota technicians. I was repeatedly assured that the vehicle, including the braking system, was operating normally. I request that NHTSA investigate the possible failure of the PCS, forward collision warning, automatic emergency braking/brake assist, and their integration with the braking system. Police didn’t inspect the vehicle. Insurance company is going to look at the vehicle on February 13, 2023. No indication of any service lights for air bags on vehicle’s dash.

While making a legal left turn on a green arrow, we were struck on the passenger side of our vehicle by a vehicle that did not stop at the red light.
In my 2020 Highlander I approached a stop sign at a "T" intersection behind 4 other cars. After stopping and looking to see road was clear of oncoming traffic, I pulled out and started my left turn. As I was turning into my lane the car engine turned OFF and the steering wheel was LOCKED in the left position. There was no power for the brakes, which left me jamming on the brake pedal as hard as I could only slowing the car slightly. The steering wheel was LOCKED LEFT and I thought I was going to break it trying to force it back. With no ability to move the steering wheel or brake, I braced myself as the Highlander continued turning in the left direction into the other lane. The result was a head on collision with a Mini Cooper, both cars totaled, and injuring both myself and the other driver. Highlander was investigated by forensic company in the employ of the insurance company, but they could not find any error in the cars CDR (Crash Data Retrieval) system.

I was told by oxmoor dealership I had to talk to toyota corporate before I could get my car touched and that I did not need to talk to a lawyer. I called. Prior to the accident, the vehicle had been exhibiting multiple symptoms: an intermittent airbag warning light had illuminated briefly on the dashboard on a few occasions, and the vehicle had begun requiring noticeably longer physical distances to come to a complete stop at traffic signs and lights when not using cruise control. The vehicle was experiencing regular phantom seatbelt warnings—dinging as if a passenger was unbuckled when the seat was completely empty. The phantom seatbelt warnings i didnt think we're a big deal were occurring across multiple seating rows simultaneously in front and back. Directly preceding the crash, while making a turn, the vehicle swerved slightly. When I applied the physical brake pedal, the vehicle did not decelerate normally; instead, it experienced a sudden, abrupt forward acceleration surge into the building. I still haven't gotten my vehicle back or any information on what has been done or when I am getting my car back. I was also charged for an extra day for my rental when I was told to return it on the 16th. I haven't had a rental since then. I keep contacting oxmoor toyota and they said they were waiting on toyota corporate. I haven't had my car for a month. I tried to email. It was returned. My mailbox on my phone has been full of drs and other calls non stop. I texted online on the toyota page for the claim as well. Ive repeatedly asked the toyota dealership what is going on and I get we are waiting on corporate. I called toyota corporate again today and was told they would get back to me. I told the dealership. I am under medical care and in severe pain. Yes my safety was put at risk. Police and fire department were here the night it happened. They said they don't do a police report on private property. There is an insurance claim through state farm.
I pushed the brake and swerved and the car kept going and a greater speed and surged into a house.

The contact stated that she sustained pain in her right chest, head, and left arm, and medical attention was sought. A police report was filed. The vehicle was towed to a local tow lot, where it remained for further investigation. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 89,083.

I WAS AT A STOP SIGN IN MY 2020 HIGHLANDER WAITING TO TURN LEFT ONTO A BUSY ROAD. UPON TURNING LEFT ONTO PA STATE HIGHWAY 999, THE HIGHLANDER ENGINE TURNED OFF AND THE STEERING WHEEL WAS "LOCKED" INTO A LEFT POSITION. THIS CAUSED THE CAR TO CONTINUE TURNING INTO THE OPPOSING LEFT LANE RESULTING IN A HEAD-ON COLLISION WITH AN ON-COMING CAR. WITH THE ENGINE TURNED OFF THERE WAS NO POWER STEERING, NO BRAKES, AND NO WAY TO UNLOCK THE STEERING WHEEL. THE COLLISION CAUSED MY CAR TO BOUNCE BACK AND SLOWLY FREE ROLL BACK TO THE STOP SIGN AS THOUGH I NEVER ENTERED THE ROADWAY.

THEY WERE ABLE TO CATCH UP TO THE VEHICLE AND RETURN THE VEHICLE TO THEIR HOME THE CONTACT STATED THAT THEIR MOTHER RECEIVED INJURIES TO HER FACE, RIGHT ARM, RIGHT HAND, LEFT RIB AREA, LEFT LEG, AND FOOT AREAS. MEDICAL ATTENTION WAS NEEDED. THE CONTACT HAD THE VEHICLE TOWED TO THE DEALER (HIGHLAND TOYOTA LOCATED AT 5500 45TH AVENUE DR, MOLINE, IL 61265; (309) 764-2481 ) WHO DIAGNOSED THAT NO DEFECT OR MALFUNCTION COULD BE DETECTED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND SENT AN INVESTIGATOR. THE APPROXIMATE FAILURE MILEAGE WAS 500.

The minor occupant sustained light abrasions from the seatbelt rubbing but no medical treatment was sought. The vehicle was taken to the dealer; however, the vehicle was not diagnosed of repaired. The manufacturer was not notified of the failure. The approximate failure mileage was 59,000.

While driving around 60 MPH all the sudden I heard a loud explosion like a shot gun going off and then my sunroof exploding and shattering all over me and the interior of my car. There was no impact from another vehicle, rock, tree, anything. It just exploded into thousands of little pieces out of nowhere. It was terrifying. I was able to safely pull over. There were cuts experienced.

The Pre-Collision System with Pedestrian Detection is a good thing, but there's a problem. It stops the car too abruptly on the highway, leading to rear-end collisions. When you release the brake pedal, the car remains stationary. To get it moving, you need to press the accelerator pedal. There have been four instances of the Pre-Collision System with Pedestrian Detection being activated in the last 12 months. Two of them ended in rear-end collisions. In the other two cases, there were no cars behind.

Was following a truck that was pulling a utility trailer. We had just stopped and started moving again when when he suddenly stopped. I had just sped up maybe going 10 to 15 mph when I applied my brake, the brakes did not engage until after I hit the trailer in front of me and jammed it into truck it was being pulled by.I had front end damage and radiator damage and the car was not drivable. I had noticed previous to this that at times at low speeds the brakes would not engage and would need to be double tapped to get them to engage. Could go weeks without it happening or could happen multiple times in a day. No consistency to the issue.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2020-2021 Avalon, Avalon Hybrid, Corolla, Highlander, Highlander Hybrid, RAV4, RAV4 Hybrid, Lexus ES350, Lexus RX350, Lexus RX450H, 2021 Sienna Hybrid, Lexus ES250, 2020-2022 Camry, Camry Hybrid, and ES300H vehicles. A short circuit may develop in the Occupant Classification System (OCS) sensor, preventing the front passenger air bag from deploying.
Rimedio: Dealers will inspect and replace the sensor as necessary, free of charge. Interim letters notifying owners of the safety risk were mailed in February 2024. Some final remedy owner letters were mailed October 31, 2024. Letters will continue to be mailed in phases through January 2026. Owners may contact Toyota customer service at 1-800-331-4331. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2020 Highlander vehicles equipped with a 3.5 L, V6 (2GR-FKS) gasoline engine. Due to an Engine Control Unit (ECU) programming error, fuel may not be correctly supplied to the engine while using the stop and restart feature.
Rimedio: Toyota will notify owners, and dealers will reprogram the ECU, free of charge. The recall began May 15, 2020. Owners may contact Toyota customer service at 1-888-270-9371. Toyota's number for this recall is 20TA06.
