We were at a complete stop waiting for traffic to go, I had my foot on the brake pedal and the car lunge/revved up and hit the car in front of me. This is the 3rd time this has happened. The first time we were advised it was probably the front-end sensor that were probably covered with ice. We were advised they are very sensitive. The 2nd time it was winter still, so we took the car through the car wash to make sure no ice was built up. We did not get in an accident because traffic started to move. We took the vehicle to the dealership, and they could not find anything wrong with it. Chrysler Corp got involved and they sent a 3rd party to check the van and black box. They advised nothing was found on the black box (not even an accident) and their investigators found nothing wrong with the vehicle. No warning messages were reported after the crash. Police were called to the scene and insurance was notified. Our insurance company advised that we should file this on here. There were 3 people in the van besides myself. We had a loaner vehicle for almost 3 months and we cannot get any information or paper work from Chrysler Corp regarding the investigation. We were advised the investigation was confidential and proprietary.

The vehicle was driven to the residence. The contact stated that several days later, the failure reoccurred. The contact stated that while the husband was driving at an undisclosed speed, the brake pedal was depressed, however the vehicle failed to stop as intended. The contact's husband depressed the brake pedal to the floorboard while attempting to stop the vehicle. The rented vehicle crashed into the rear of the vehicle that was in front of the rented vehicle before coming to a complete stop. No warning lights were illuminated. The vehicle was driven to the residence. No injuries were sustained. A Police report was filed. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The manufacturer was not made aware of the failure. The failure mileage was approximately 26,000.

My son was driving the vehicle down the interstate and attempted to exit. He hit the brakes and the car would not slow down. He had the pedal all the way to the floor and the vehicle still would not slow down. The vehicle went off the exit ramp, through a grassy hill area, across the entrance ramp, and finally stopped when the tire broke off. Even with the tire broke off and the vehicle not moving the engine was showing it was going 30-40 mph. No airbags were deployed. I received a Luckily the only injury was my daughter's sprained wrist. The entire front end of the vehicle has been replaced, including the frame, and yet no airbags were deployed. I did receive and ABS failure alert on my app but was told that was because of the tire breaking off and not what caused the accident. I have requested the vehicle be inspected since the accident occurred. Insurance stated it would need to be repaired before an inspection could occur. I have filed a case with Stellantis who now states they will not have anyone look at it because it has to be done before repairs are made and since insurance has paid for the repairs, they have to request an inspection. We had multiple issues with the TPM system not working properly and had it to a dealership for repairs, that was the only warnings we received. The vehicle is still sitting at the dealership/collision repair shop as they feel it warrants further investigation as well.

On Saturday July 25th, while driving at 70 mph on the highway, my van lost the ability to accelerate or maintain velocity. There was no warning or indication that this was going to happen. There was a beep and the screen flashed to a screen that said to pull over immediately and safely. Surprised and scared, I had to cut across 3 lanes of traffic while losing velocity cutting off a tractor trailer because my velocity dropped quickly. After restarting, the vehicle ran normally. I drove in the slow lane for a mile an it happened again. I was able to get the vehicle off the highway and drive slowly and safely on backroads to get the van to a Ourisman Chrysler in Clarksville, MD. The check engine light was on when I arrived at the dealer. Immediately upon arrival I was told by a service representative that it will most likely need a new transmission. On Tuesday he told me a tech verified it needs a new transmission. He told me the vehicle was classified as VOR (Vehicle Off Road) meaning it was unsafe or unable to be driven on the road and that parts were ordered and expedited. I did not hear back from the dealership until Friday at which point they surprisingly told me my vehicle was ready to pick up. In an email I was told: "what was found was a stored fault code P0A43-00 Drive motor "A" position sensor circuit intermittent. There is a TSB 21-037-26 for that fault code and vehicle going into limp mode and the repair is a rebuild kit for the transmission. " Despite this they also said in an email: "cleared codes and perform test drive. Code never came back and it states in the procedure you must verify the problem meaning it happened or is happening via active fault code which it is not." They refuse to fix the problem, despite acknowledging it happened. I was told to take the vehicle back and drive it as normal until it happens again, which I refused due to safety concerns. The vehicle currently has a bumper-to-bumper warranty.

But I am not aware of a federal, state, nor county regulation establishing such restriction. The restriction thus seems to be self-imposed and therefore under the control of the manufacturer.

My Chrysler Pacifica was parked in my driveway, and I went out and heard a large boom and my panoramic sunroof shattered. This is a known problem with panoramic sunroofs especially Chrysler. The weather was warm and sunny. This is a no impact incident and a safety related glass failure. I have several photos and am waiting for the Dealer and Chrysler customer care to open today.

We purchased a 2025 Chrysler Pacifica in September 2025. In October, 2025 we begin to receive Urgent Safety Recall notifications regaring Driver Side Curtain Air Bag. My husband went to the dealer where we purchased van and was told we would have to be on a waiting list and we were number 20 on her list. The dealer has never contacted us about scheduling a repair on the Driver Side Curtain Air Bag. We have made 3 trips to the dealer and have been told the same story...we are 20th on her list. The Recall clearly states that because of this defect, "occupant could be ejected during a crash." To us, that seems critical to have repaired. Chrysler evidently doesn't think so putting us at great risk in case we were in a accident. Stated in their Recall: "We are sincerely concerned about your safety and reiterating scheduling a repair". After two calls to the Customer Service at the company and 3 trips to the dealer, we are no closer to having this repair schedule. I thank you in advance for any assistance you can provide in helping us expedite this repair.

Auto stall at stop is not working safely on my vehicle there are two issues that can cause safety problems .A) when system restarts engine the vehicle has jumped forward several feet quickly causing very nearly hitting the car in front of me as a traffic light turned green . B) when I slow not quite to a stop making a left turn I have experience very low or no power and could be hit if a car is coming towards me . Both these issues do stop if I remember to turn off the auto stall system but I do not always remember ! I did make a compliant at a Chrysler Dealer they found no issues and did not reproduce the problem no warning lights come up when the problem happens . I have not had damage to the vehicle but have had two close calls one nearly rearending a vehicle at a intersection as my Pacicifca lurched forward and the other as an oncoming car needed to brake when I made a left turn in an intersection and had a loss of power .

The brake pedal position sensor capacitor may be cracked, which can cause a loss of electronic stability control. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 126, "Electronic Stability Control Systems."
Solution : Dealers will inspect and replace brake pedal position sensor, as necessary, free of charge. Owner notification letters are expected to mailed September 29, 2026. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 226, "Ejection Mitigation."
Solution : Dealers will replace the side curtain air bags, as necessary, free of charge. Owner notification letters were mailed May 14, 2026. Owners may contact Chrysler customer service at 800-853-1403. Chrysler's numbers for this recall are 06D, 10D, and 11D. Vehicle Identification Numbers (VINs) involved in this recall will be searchable on NHTSA.gov beginning April 2, 2026. This recall expands and replaces previous NHTSA recall numbers 25V302 and 25V573. Vehicles previously repaired under a prior recall will need to have the new remedy completed.
