I was arriving for an appointment and preparing to parallel park on the street. I was parking on a street in front of the office building, which is located on a corner. I observed the second and third vehicles parked from the corner were preparing to depart. I moved my vehicle slightly to the shoulder and activated my blinker to wait for the vehicles to leave. I idled next to the car parked at the corner to allow room for the departing vehicles to exit and for myself to pull in. Both vehicles pulled out of their parking spaces and into the street. I then proceeded to accelerate to pull into a parking spot. As two spots had opened, it was large enough for me to pull in forward without needing to parallel park by backing in. As I started to pull in, the vehicle’s automatic emergency braking system malfunctioned and erroneously engaged the brakes with violent force. An alarm sounded in the vehicle, a red warning flashed on the screen, and I was launched violently forward in my seat. The force with which the vehicle stopped made it seem as though I had hit something hard, like a wall. I quickly put the vehicle in park -- only halfway pulled into the parking spot -- and exited the vehicle to survey the situation. After circling the vehicle, it was clear that nothing had been hit and there were no obstructions in the road whatsoever. I returned to the driver’s seat and resumed parking. After pulling into the space, I put the vehicle into reverse to straighten out. Again, the vehicle’s braking system engaged without reason, though this time less violently and without the sound alarm and red warning. After parking, I exited the vehicle and took photographs of the area to document that there were no obstructions or anything else in the environment that would cause the system to believe there was an obstruction. Due to the malfunction, I sustained injuries consistent with whiplash -- pain, tightness, and inflammation along the left side of my neck and left shoulder.

My primary safety complaint is that the automatic emergency-call function failed during an actual airbag-deployment collision even though the SOS indicator was green and appeared to show that the system was operational. The vehicle is a 2023 Volkswagen ID.4, VIN [XXX]. On XXX, at approximately 8:04 a.m., while traveling southbound on Columbia Pike near Trader Joe’s, the vehicle was involved in a collision that deployed the airbags. The vehicle automatically initiated the emergency-call sequence but announced words to the effect of, “Your call cannot be completed.” The Montgomery County Police Department investigated the collision under Incident Number XXX. Fortunately, a passerby witnessed the collision and assistance was available. Without that person, emergency assistance could have been delayed, increasing the risk of serious harm. An injured, disoriented, trapped, or unconscious occupant may not be able to locate a phone and call [XXX]. The vehicle had a prior history of recurring GPS, SOS, and connected-services failures. It was presented to authorized Volkswagen dealers four times while under warranty, and a Volkswagen TAC case was opened, but no root cause or permanent repair was documented. Shortly after Volkswagen was notified that a lemon-law filing would proceed, the GPS, green SOS indicator, and some connected functions appeared to return for about two days without any explanation of a repair, software update, reset, remote action, or configuration change. The GPS then became inaccurate again, while the SOS indicator remained green. I request that NHTSA investigate why the indicator remained green when the automatic crash call could not connect, whether Volkswagen verifies actual voice and location functionality before displaying the green indicator, and whether affected vehicles require a safety recall or other corrective action. Date/Time of Incident: 1/29/36 at approximately 5:44 PM Location: Small school parking lot Vehicle: 2023 Volkswagen ID.4 On 1/29/36 at approximately 5:44 PM, I was pulling out of a parking space at low speed in a small school parking lot in my 2023 Volkswagen ID.4. As I began to move, the vehicle suddenly accelerated on its own, the steering would not turn left, and the car did not respond to braking, moving at an abnormal rate of speed over a very short distance. Prior to this, there had been no warning messages or alerts, except a routine notification to service the car in 1,000 miles. The collision involved three vehicles: my VW, the main car I hit, and a rental car. My car spun and veered sideways, the parked SUV on the right was pushed sideways, forming a rough “H” pattern, and the rental car went up onto a rail ramp where children walk. My grandson observed the car’s tires spinning at impact, and he hit the seat behind me. Without the parked cars, the vehicle would have continued directly toward the school building, creating a very serious risk of fatal injuries to parents, staff, and children. Injuries: Myself — pain and emotional trauma; grandson — soft cast on foot/leg, crutches, pain; his mother — pain. The vehicle is awaiting Volkswagen guidance for inspection. Likely involved systems include Speed Control, Fuel/Propulsion System, and Electrical, but the exact cause is UNKNOWN. The incident has been reported to State Farm (claim #0596B819B), Volkswagen (Case #07294060), and police.
On 7/4/2024 at approximately 6pm, I disconnected my 2023 VW ID4 from an electrical charging station. I moved my vehicle 2 blocks away to an apartment building parking lot, which I had parked in previously. While pulling into the parking spot, I was coming to a complete stop, perhaps going 1-2 miles per hour and as I came to a stop, the car accelerated out of nowhere and crashed into a few small trees and a chain link fence for the neighbor's home. I had a passenger in the front passenger seat and neither of us could understand what had just happened. It made no sense and I have no idea what happened to cause the acceleration.

The contact owned a 2023 Volkswagen ID4. The contact stated that while parking the vehicle and attempting to remove the key from the ignition the vehicle erroneously accelerated forward driving over a curb and crashed into a traffic pole. The vehicle continued to accelerate forward into an intersection and crashed head-on into a second vehicle. During the crash, both vehicles were destroyed. The driver sustained a left arm laceration from the air bag deploying. The front passenger sustained a left arm laceration and abdomen contusion. The dog located in the rear passenger seat sustained an eye injury. All required medical treatment. No injury was reported from the second vehicle. A police report was taken at the scene and the vehicles were towed away. The cause of the failure was not determined. The manufacturer and local dealer were not contacted. The failure mileage was 4,000.

When I was backing from a parking space, another car drove behind mine and the car's maneuvering braking kicked in and decelerated the vehicle. When I stepped on the brake, the car then accelerated backwards and caused me to collide with the crossing vehicle.
The driver of my vehicle stated that my car suddenly lurched forward striking a cross passing vehicle. He did not accelerate and the emergency brake system failed to engage in advance of an obvious and avoidable crash.
I was driving with Travel Assis on, the car steered left into the center divider then swerved right into other lanes… I was unable to regain control of steering wheel or break to deactivate travel assist for about 10 seconds, the steering wheel felt stuck in the direction that Travel Assist was steering to.
My cars backup cameras have never worked properly and the dealer keeps changing me $485.00 to run a complete diagnostic every time I take it in. My car will erratically pulled to the right for no reason and caused the right front tire to blow forcing me to have it towed to the dealer and they had to replace both front tires which cost me $900.00 to replace. The car locked up the brakes and turned it to the right causing me to sever my left outer thumb which has cost me over $700.00 so far and is going to require me to have 4+ hours of surgery on my thumb by a surgeon and they haven't given me the cost of the surgery yet but it is estimated at over $7,000.00 and I am left handed and doesn't allow me to use that hand to hold the steering wheel or anything else.

On 9/17/2024 roughly at 10:00PM, while in the drive-thru picking up food, my Volkswagen ID.4 suddenly self-accelerated out of nowhere as I was slowly driving towards the window to pay for food. I was completely shocked—I’ve never experienced anything like this. I tried to react quickly, but the car surged forward and slammed into a large cement light post. The impact was hard, and the car is now severely damaged. What really shook me was that none of the safety features I’ve come to rely on worked—there was no automatic braking, late front collision warning, and despite the heavy impact, none of the airbags deployed. I ended up feeling all bruised and now have some severe mid-back pain. The thought that I could have easily been hurt more seriously, or worse, could have harmed someone else if this had happened in a crowded area, is traumatizing. This has made me completely lose trust in the vehicle’s safety systems. If the car can do this in such a low-risk situation, I can’t imagine what might happen in a more dangerous scenario. I brought up other safety concerns to Volkswagen that were completely dismissed.

I am writing to file a formal complaint with the National Highway Traffic Safety Administration (NHTSA) regarding a critical safety concern with my recently purchased vehicle, a VW ID 4 model from 2021-2023. Vehicle Information: VIN: [XXX] Make: Volkswagen Model: ID 4 Year: 2021-2023 Date of Purchase: January 5th, 2024 Incident Details: On [XXX], I experienced a hazardous incident involving sudden acceleration when releasing the brake pedal as I was coming to a stop and pulling into a parking space. Despite the fact that the pedal was not fully released, the vehicle accelerated forward, colliding with another vehicle at an approximate speed of 40-50 MPH. Shockingly, neither the airbags deployed nor did the safety automatic braking system engage. The incident raised significant safety concerns, especially considering similar reports from other VW ID 4 owners. Actions Taken: I promptly took the vehicle to VW Marin Dealership in San Rafael, CA, where their report claimed no issues with the vehicle. However, further research on my part revealed that this issue is prevalent among VW ID 4 models from 2021 to 2023. Attached to this letter, you will find pictures of both vehicles involved in the incident for your reference. Request for Investigation: I urge the NHTSA to thoroughly investigate this safety issue with VW ID 4 models from 2021-2023. I have never been in an accident in my entire life, never got pulled over by a cop, and have only ever driven one car prior to the vw. I always have my car in “B” mode. During the crash on 4/02 I was going about 40 to 35 mph coming off the highway tried to slow down more to 25 to go on the roundabout but it wouldn’t, I had the brake collision assistance on, but unfortunately it did not assist me in trying to stop, even when I pressed down on the brake pedal completely. When I tried to turn the steering wheel, that had locked and I was unable to avoid hitting the sign and curb and going over the cement median which cause both my left tires to go flat. I got a call from the Volkswagen SOS and at that time the person was talking to me, I had to maneuver my car off the cement median to move it to a safer road as I was in the middle of a dangerous place and it needed to be towed. The road I had the incident on is the one I take to work every day and I have never had an issue. There was no issues with the weather on that day ether. I have only driven this car 703 miles.

I have never been in a car accident in my entire life. This was my first ever electric vehicle, and I really loved driving it for the one week I owned it before the brakes failed to stop it. On 03/31 I had to restart the software several times because it froze when i was trying to connect it to Apple Carplay, but it was fine the next day. During the crash, I had the brake collision assistance on, but unfortunately it did not assist me in trying to stop, even when I pressed down on the brake pedal completely. When I tried to turn the steering wheel, that had locked and I was unable to avoid hitting a sign and curb. The road I had the incident on is the one I take to work every day and I have never had an issue. There was no issues with the weather on that day ether. Not only did the brakes not work but the the brake assist failed, the collision avoidance system, the steering wheel locked, and the airbags failed to deploy. Both my left side tires became flat. If I would have known how unsafe this was, I would have never selected it in the first place. I chose this vehicle because I thought I would be safe and I wanted to save money on gas. I am so extremely terrified to drive this car and the dealership service center claimed there was nothing wrong with the car.

The front seats go back and recline to an almost flat position randomly. This has hurt my kids' knees while they've been seating in the back seat. It is also very scary and they yell and scream in pain and panic. There is no pattern to the moving, it happens while stationary. Dealers have refused to look at it because they cannot replicate it at the time of the service call. I have pictures and videos of some instances but they are random. No warning or alerts pop up. This has been happening randomly since I bought the car one year ago.

The message went away immediately and the car continued to work normal. My neck is still hurting hours later from the very hard brake. During the whole incident there was no other car moving and no person or other object anywhere in the path of the car. I have not yet brought the car to the dealer but will do so next week.
