"I am reporting a fatal crash involving a 2024 Toyota Corolla (VIN [XXX] I do not know the exact mechanical cause, but the following symptoms occurred: 1) The driver reported dashboard lights (red and blue) flickering on and off, windows and locks going on or off. 2) There was hesitation in acceleration after taking off, 3) The vehicle failed to stop and went airborne causing a FATAL accident. Insurance Co. failed to investigate porperly and sold the car To Co-part without waiting for Probate forms to be complete for this vehicle and the Estate. We have discovered this car was an Avis fleet vehicle sold as 'new' at Beaver Toyota. I am requesting a federal investigation to determine if a mechanical or electronic defect caused this loss of control." There was a recall on my vehicle for the skid and emergency braking and was told it was taken care of Feb 2025 at my service appointment. I was involved in a car accident on Aug 11, 2025, everything that was supposed to be fixed in the recall did not operate during my crash. I reconfirmed with the dealership that they did the recall repair and they again said yes but if they did then the repair completely failed. There was no emergency braking available, the car skid a long distance, no airbags deployed even though all sensors in the front were engaged and collision center confirmed they should have gone off. Vehicle is currently at collision center and is available to be inspected.

Component/system failed: The SRS Airbag System malfunctioned. After a rear-end collision on October 3, 2025, in Gwinnett County, GA, the airbags did not deploy. The vehicle displayed an alert: “SRS Airbag System – Malfunction detected” at 16:02. Safety risk: My safety was put at risk because the airbags did not function during a significant impact. This failure could have resulted in severe injury if the collision had been more forceful or from another angle. Inspection/reproduction: On October 6, 2025, I brought the vehicle to Atlanta Toyota. Instead of documenting the diagnostic trouble code (DTC) and providing a report, the dealer cleared the fault code and recalibrated the system without my authorization, effectively erasing critical evidence of the malfunction. The invoice confirms this. Inspections by others: The vehicle was inspected only by the Toyota dealer. It has not yet been inspected by the manufacturer, police, or insurance representatives, but I have legal counsel and am preserving the evidence for further independent inspection. Warning lamps/messages: Yes. The SRS Airbag System Malfunction message appeared immediately after the collision on October 3, 2025. I have photographic evidence of this alert.

[XXX] In reading the above article, Toyota recalled Corolla Cross Hybrid vehicles with many complaints and situations mirroring our accident. Before the accident, the vehicle displayed no warning signs. Despite the driver pressing the brakes and the assisted braking feature seemingly turning on (e.g., signal lights on the computer system), neither seemed to work in this situation. The damage was far too severe for the assisted braking feature to have activated, along with the driver pressing the brakes. After viewing the matter with those involved in the accident, many of us concurred that we believe either the braking system or the computer system failed. In the aftermath of the accident, the police, manufacturer, or third party hadn't reviewed the vehicle's status. On top of that, our insurance refused to investigate, urging us to discuss the matter with Toyota. Thus, we'd propose that the Corolla SE may experience an issue similar to the vehicles in the article. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

On 6/7 while driving 65 mph I hit a deer on the driver front. My car detected the crash and deployed every airbag inside the car. I received an injury to my legs from the driver's airbag under my steering wheel. If this airbag were not there, I would have had no injuries from this accident. I have missed work and was seen at the local ED for treatment.

During a routine transmission fluid service on a 2024 Toyota Corolla Hybrid, I discovered that the transmission model (PA10) is completely unlabeled and cannot be identified anywhere under the hood, in the owner’s manual, or on any visible tag. The only label is buried behind the firewall and not reasonably accessible to consumers or technicians. As a result, I initially attempted to service the wrong fluid port because Toyota provides no visible or clear identification of the transmission model or fill/drain points. Additionally, the factory-installed transmission drain and fill bolts were tightened with extreme torque well beyond normal service specifications, requiring application of high heat and excessive force to loosen. This resulted in personal injury and risk of property damage. The combination of (1) inaccessible transmission identification, (2) inadequate service guidance, and (3) extreme bolt torque creates a foreseeable safety hazard to vehicle owners or technicians attempting basic maintenance. Toyota’s official service documentation (Toyota Technical Information System, “TIS”) that clarifies the correct fill ports is paywalled and not included with the vehicle or manual, making it impossible for consumers to safely perform essential maintenance without paying daily access fees. This lack of accessible labeling and over-torqued components presents an avoidable risk of physical injury, vehicle damage, and mis-servicing. I believe Toyota should be required to provide visible transmission identification and safe torque specifications for consumer maintenance.

The contact owned a 2024 Toyota Corolla. The contact stated that while the vehicle was stopped on a curb with another vehicle approximately 8 -10 feet ahead, the brake pedal was depressed to shift into park; however, the vehicle independently accelerated. The contact stated that because of the failure, the contact's vehicle rear-ended the Honda CR-V vehicle that was parked in front of the contact's vehicle. The contact stated that the vehicle's speed was only 3–4 MPH. No air bags deployed. The other vehicle sustained damage to the rear bumper. The contact's vehicle was towed to a local tow yard. The contact's vehicle was condemned as a total loss by the insurance adjuster. No police report was filed. No injuries were sustained. The manufacturer was made aware of the failure. The failure mileage was approximately 10,000.

Dear Better Business Bureau, I am filing a formal complaint against Toyota Motor North America regarding a serious and unresolved safety defect in my 2024 Toyota Corolla LE (VIN [XXX]). The vehicle experienced two incidents of sudden unintended acceleration—the second, on [XXX], resulted in a collision. Despite my repeated efforts to resolve the issue, Toyota has failed to respond adequately or provide critical diagnostic data. In both incidents ([XXX] and [XXX]), while stopped at a red light, the vehicle suddenly surged forward despite my foot being on the brake. On [XXX] this caused a collision. My vehicle has remained undrivable since. Toyota conducted a diagnostic inspection on May 29 under Case #[XXX]. I’ve repeatedly requested access to the raw data from this inspection to understand the defect, but Toyota has not responded. Emails to the assigned case manager, Ms. Cynthia Ware, have gone unanswered. When I was directed to reply to a support email, it bounced back as a “do-not-reply” address, redirecting me to a login page which falsely claimed I had no active cases. This created an endless communication loop. Toyota’s lack of transparency and support is unacceptable—especially given the severity of the issue and the fact that the vehicle is still under warranty. I am requesting: Immediate release of the raw diagnostic data from the May 29 inspection A prompt resolution, including full repair, replacement, or buyback Clear, responsive communication from an authorized representative This is not only a financial burden, but a serious safety risk. I respectfully request BBB’s assistance in securing a resolution and holding Toyota accountable. Thank you for your time.
On 11/29/23 after 11:30pm, I was in the process of clearing an exit and at the tail end of the exit it felt like the car was "correcting" itself and I was unable to stay turning right. The steering wheel felt locked and I was unable to control the car. The car impacted the guardrail and I was able to regain control after the initial impact. -This is a BRAND NEW car with less than 100 miles at the time of the accident. - Prior to the collision, there were no warning signals from the dashboard. - I was not injured from the accident as I was only able to go around 27-33mph due to a tractor trailor clearing the exit in front of me. No others were involved. - The manufacturer sent an inspector to look at the vehicle and test drive it. However, they did not test drive in the same conditions I was in or at. - The issue was not reproduced by another party. (Photos were taken the next day)

In a 2024 Corolla the spiral cable (clock spring) is defective. It controls the airbag, cruise control, horn and more, so it is an important safety mechanism electrical part. Toyota USA is on back order for this part for at least a month, maybe two. This suggests to me that there is a widespread problem with defective Toyota Corolla spiral cables (clock spring) inside the steering wheel. People are driving around with this defective part while waiting for the part to arrive in the US, and will need to pay upwards of 1000 to have the defective part replaced.

A severe, pre-existing life-safety defect and unauthorized structural modification were discovered on my 2024 Toyota Corolla . Following a routine diagnostic check for vehicle pulling, certified Toyota technicians discovered that the vehicle's primary fuel line had been dangerously altered and was held together underneath the chassis by an unsecured wire clothes hanger. The vehicle was immediately grounded as an active fire and explosion hazard. A forensic vehicle inspection by a major insurance carrier (Traveler's) subsequently denied standard repair coverage, formally documenting that the wire-rigged fuel system was a pre-existing mechanical defect resulting from previous work done prior to my purchase of the vehicle from McCluskey Automotive. The selling dealership actively concealed an unroadworthy condition that exposed myself and my minor daughter to a catastrophic vehicle fire.

Subject / Issue: Child car seat caused vehicle seat belt to become permanently jammed Complaint: I am reporting a safety-related issue involving an Evenflo Revolve 360 child car seat. During normal use, the vehicle seat belt became slightly twisted inside the car seat base, and the seat stopped rotating properly. We were then unable to remove the child car seat from the base following the standard removal instructions. While attempting to safely remove the seat, the vehicle seat belt became completely jammed inside the base. The seat belt is now fully immobilized and cannot move in either direction. As a result, the child car seat is permanently stuck in the vehicle, and the seat belt system in the left rear seat is unusable. We had the vehicle inspected at a Toyota service center, and they confirmed that the only way to remove the child seat is to cut and replace the left rear seat belt. The estimated repair cost is approximately $680. This situation has rendered a vehicle safety restraint system inoperable and requires cutting the seat belt to resolve the issue. I am concerned about the safety design and potential risk if this occurs during normal consumer use. Photos of the issue are available and can be provided if needed.

A flaw in the vehicle's combination meter programming causes it to deteriorate its memory. Due to this the display of the monitor may not work, removing access to the speedometer and warnings. This increases the risk of accidents. The product does not comply with the Regulation on the approval and market surveillance of motor vehicles and their trailers, and of systems, components and separate technical units intended for such vehicles.
Insufficient pressing of parts in the high pressure fuel pump can cause movement of the cylinder in the fuel pump through which the piston moves. This can create wear between the cylinder and the pump body, resulting in increased stress on valves and/or movement valve seat inside the pump body. This increased stress can cause a crack in the weld of the affected valves, which could lead to a fuel leak, increasing the risk of fire. The product does not comply with the Regulation on the approval and market surveillance of motor vehicles and their trailers, and of systems, components and separate technical units intended for such vehicles.
Due to the programming of the skid control's electronic control unit software, the brake fluid pressure can be incorrectly controlled when the brake pedal is applied during cornering. This can lead users to experience a hard brake pedal with reduced braking force, increasing the brake stopping distance and creating a risk of accidents and injuries. The product does not comply with the Regulation on the approval and market surveillance of motor vehicles and their trailers, and of systems, components and separate technical units intended for such vehicles.
