Vehicle Left rear wheel detached on Interstate 94 in Washtenaw County, Michigan. Resulted in death of another motorist. Michael James Brooks was operating the Black Dodge 4 Door, 2013 vehicle, traveling westbound on I-94. The vehicle tire detached and traveled into a vehicle operated by Janice Southall. The incident resulted in a rollover collision and death of Janice Southall.
ON DEC. 15, 2014 I RECEIVED A RECALL ON MY 2013 DODGE RAM 2500 DIESEL. THE RECALL WAS FOR A INLINE FUEL HEATER THAT MAY CATCH FIRE, IT HAD NO APPARENT FIX, NO PARTS, NO INSTRUCTIONS AND NO TIMELINE AS TO WHEN IT WOULD OR COULD BE FIXED. I CALLED THE DEALERSHIP AND SPOKE TO THE SERVICE MANAGER ABOUT MY CONCERNS, HE TOLD ME THAT IT WAS A REMOTE POSSIBILITY NEXT TO NONE, THAT THE VEHICLE WOULD CATCH FIRE. I ASKED HIM IF I COULD PARK IT INSIDE AND HE SAID YES, AND THAT THE RECALL WAS JUST BEING SENT OUT AS A PRECAUTION. ON THE EVENING OF JAN 28TH 2015 AT APPROXIMATELY 0:920 PM SMOKE DETECTORS ALERTED MY DOG AND HE WOKE ME UP. I WOKE MY WIFE UP YELLING (FIRE) I PANICKED I TOOK IN A DEEP BREATH COVERED MY MOUTH WITH MY FOREARM, OPENED THE PERSONAL ENTRY DOOR AND STEPPED INSIDE THE GARAGE NOT REALIZING HOW BAD IT WAS. THE AMOUNT OF HEAT BEING GENERATED FROM THE DIESEL FUEL AND THE STRUCTURE WAS SO HOT THE BACK DRAFT PULLED THE DOOR OUT OF MY HAND TRAPPING ME IN THE GARAGE AND BECAUSE IT WAS A SCHLAGE COMBINATION STYLE THERE WAS NO TURNING BACK, ALL I COULD SEE WAS COMPLETE BLACK AND RED, RED FLAMES WAS SHOOTING OUT FROM UNDER MY TRUCK. MY NEIGHBOR WAS KICKING ON MY GARAGE DOOR YELLING TO WAKE UP. I PANICKED AGAIN AND HIT THE UP BUTTON ON THE MAIN GARAGE DOOR HOPING TO MAKE A RUN FOR IT, THE GARAGE DOOR WENT UP ABOUT A FOOT AND FELL HARD BECAUSE IT PULLED THE DOOR MOTOR FROM THE MOUNTING BRACKETS. I KNEW THE ONLY WAY OUT WAS THE FRONT DOOR I USED THE SIDE OF THE DODGE AS A GUIDE AND BURNT MY LEFT HAND, MADE IT OUT THE DOOR AND MY NEIGHBOR HANDED ME THE GARDEN HOSE HANGING ON THE FENCE. ALL FIRE REPORTS INDICATE THE FIRE STARTED UNDER THE HOOD OF THE DODGE, THE FIRE INVESTIGATOR FROM CHRYSLER SAID IN FRONT OF LARAMIE COUNTY ARSON INVESTIGATORS THAT IT ORIGINATED AT THE FUEL HEATER. STATE FARM MAY HAVE SETTLED WITH DODGE. PLEASE READ ATTACHMENT.
THE ALTERNATOR CAUGHT ON FIRE. TRUCK HAS 62000 MILES ON IT. THE BATTERY LOW VOLTAGE CAME ON. AFTER THE TRUCK RAN FOR A MINUTE IT WENT OUT. I RAN THE TRUCK FOR TEN MINUTES. AFTER I PARKED THE TRUCK AND WENT INSIDE I COULD SMELL SOMETHING. WHEN I STEPPED OUT MY DOOR AND LOOKED SMOKE WAS ROLLING OUT FROM UNDER THE HOOD. I GRABBED THE EXTINGUISHER AND PUT IT OUT. I THEN SPRAYED IT DOWN WITH WATER TO COOL IT. IT TOOK SOME TIME.
TL* THE CONTACT OWNS A 2013 RAM 3500. THE CONTACT STATED THAT WHILE DRIVING AT 55 MPH, THE VEHICLE ABS WARNING LIGHT ILLUMINATED AND THE VEHICLE VIOLENTLY SHAKES. THE CONTACT VEERED TO THE SHOULDER OF THE ROAD, GOT OUT THE VEHICLE AND SEEN SMOKE COMING FROM FRONT PASSENGER SIDE TIRE. THE CONTACT NOTICED THAT THE WHEEL WAS SEPARATED FROM THE AXLE. THERE WERE NO INJURIES SUSTAINED. NO POLICE/FIRE REPORT WAS FILED. THE CONTACT CONTACTED HIS INSURANCE TOWED THE VEHICLE TO THE DEALER RIVER OAKS CHRYSLER JEEP DODGE RAM 4807 KIRBY DR, HOUSTON, TX 77098 (888) 284-5254 WHERE A DIAGNOSTIC TESTING WAS PENDING. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND INFORMED THE CONTACT THAT NO RECALLS WERE UNDER THE VIN. THE FAILURE MILEAGE WAS 103,000.
I WENT TO START MY VEHICLE IN MY DRIVEWAY ON 5/23/2019. THE VEHICLE GAVE ME A LOW BATTERY INDICATION AND IMMEDIATELY SMELLED BURNING PLASTIC. SHUT THE VERTICLE OFF, HAD EVERYONE EXIT THE VEHICLE AND POPPED THE HOOD UP. SMOKE WAS POURING OUT OF THE ALTERNATOR THAT HAD BEEN "INSPECTED " INSTEAD OF BEING REPLACED ACCORDING TO THE S68 RECALL BULLETIN 3MONTHS EARLIER. I QUICKLY GRABBED SOME TOOLS TO DISCONNECT BOTH BATTERIES TO PREVENT AND FURTHER DAMAGE OR FIRE. *DT*JB *DT *DT *TR
TL* THE CONTACT OWNS A 2013 RAM 3500. WHILE ATTEMPTING TO START THE VEHICLE, THE SERVICE ENGINE INDICATOR ILLUMINATED. ADDITIONALLY, THERE WAS AN ABNORMAL BURNING ODOR IN AND OUT OF THE VEHICLE. THE VEHICLE WAS TAKEN TO CLASSIC CHRYSLER DODGE JEEP (1102 WALTON DRIVE, TEXARKANA, TX 75503) WHERE IT WAS DIAGNOSED THAT AN UNKNOWN SYSTEM WAS CONTAMINATED AND NEEDED TO BE CLEANED. THE VEHICLE WAS REPAIRED. THE MANUFACTURER WAS NOT NOTIFIED. THE FAILURE MILEAGE WAS APPROXIMATELY 86,000.
ON JUNE 1, 2013, I WAS DRIVING THE DODGE RAM 3500 AT HIGHWAY SPEED (APPROX. 60 MPH) WHEN THE STEERING COMPLETELY LOCKED UP AND 4-WAY EMERGENCY FLASHERS FAILED. I DRIFTED TO THE SIDE OF THE HIGHWAY AND CAME TO A STOP. I IMMEDIATELY CALLED ROADSIDE ASSISTANCE. MY PHONE LOG SHOWS THIS CALL STARTED AT 8:40AM AND LASTED 9 MINUTES. DURING THE CALL, WASHINGTON DEPARTMENT OF TRANSPORTATION INCIDENT RESPONSE STOPPED AT THE SCENE AND NEITHER OF US REALIZED THE EXTENT OF THE PROBLEM AND WA DOT LEFT THE SCENE. NEAR THE END OF THE CALL, I NOTICED SMOKE POURING FROM UNDER THE HOOD AND EXITED THE VEHICLE. AT THAT TIME, I NOTICED BURNING MATERIAL ON THE GROUND UNDER THE TRUCK'S ENGINE. I HUNG UP WITH ROADSIDE ASSISTANCE AND CALLED 911 AT 8:49AM. AT THAT TIME, AT LEAST 4 LAW ENFORCEMENT, 2 FIRE ENGINES, AND WA DOT ARRIVED ON SCENE AND SHUT DOWN THE ENTIRE STATE HIGHWAY DUE TO THE SIZE OF THE FIRE. ONE LANE OF THE HIGHWAY WAS REOPENED JUST BEFORE 9:20AM. WHEN I LEFT THE SCENE, THE FIRE DEPARTMENT WAS STILL ACTIVELY WORKING TO CONTROL THE BLAZE AND HAD CUT OPEN THE TRUCK'S HOOD TO BETTER ACCESS THE FIRE. AFTER THE FIRE, I RE-READ THE RECALL NOTICE FOR THE VEHICLE'S ALTERNATOR AND BELIEVE THIS PROBABLY CAUSED THE NEAR-CRASH AND FIRE.

THE CONTACT STATED AFTER THE REPAIRS WERE COMPLETED WHILE HIS WIFE WAS DRIVING SHE EXPERIENCED THE DEATH WOBBLE AND SKIMMED THE CEMENT WALL OF A BRIDGE. THE CONTACT STATED NO WARNING LIGHT WAS ILLUMINATED. THE CONTACT STATED NO ONE WAS INJURED OR SEEKED ANY MEDICAL ATTENTION. NO AIR BAGS DEPLOYED. THE CONTACT STATED HE HAD ALSO EXPERIENCED THE FAILURE WHILE DRIVING. THE VEHICLE WAS TAKEN BACK TO THE SAME DEALER AND WERE MADE AWARE OF THE FAILURE. THE CONTACT STATED HE HAD ALL 4 TIRES CHANGED BUT CONTINUED TO EXPERIENCE THE FAILURE. THE MANUFACTURER WAS CONTACTED BUT WAS UNABLE TO SPEAK TO ANYONE. THE FAILURE MILEAGE WAS 119,175.
The contact owns a 2013 Ram 3500. The contact stated that while attempting to sell the vehicle, the contact discovered there was a discrepancy in the mileage. The contact stated that the Carfax mileage was 47,029, and on March 10, 2015, the mileage was recorded at 49,873, and nine days later the mileage was recorded at 47,019.
The contact owns a 2013 Ram 3500. The contact stated while driving 65-68 MPH over a bumpy road surface, the steering wheel and the front end of the vehicle shuddered significantly. The contact decelerated, exited the freeway, and pulled over to the shoulder of the road. The contact was able to continue driving. The vehicle was taken to the dealer, where the contact was informed that the vehicle had experienced the death wobble. The dealer diagnosed the vehicle and determined that the upper ball joints and control arms needed to be replaced. The dealer replaced the upper control arms. The vehicle was then taken to an independent mechanic, where the upper ball joints were replaced. The contact stated that the failure recurred several times while driving. The contact stated that the upper and lower ball joints, the control arms, the bushings, and other unknown parts were replaced several times; however, the failure persisted. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 100,000.
I HAVE BEEN HAVING PROBLEMS WITH A RECALL ISSUED BY NHTSA. THE RECALL WAS V06 DRAG LINK. WHEN THE RECALL WA PREFORMED, THE ALIGNMENT NUTS ON THE DRAG LINK WERE WELDED SO THE COULDN'T COME LOOSE. I'VE HAD TO HAVE MY TRUCK REALIGNED 2 TIMES SINCE THE DRAG LINK NUTS WERE WELDED. AS OF RIGHT NOW THE NUTS ARE NOT WELDED AND THE INTEGRITY OF THE DRAG LINK IS COMPROMISED. I'VE HAD IT INTO 3 DIFFERENT INDEPENDENT REPAIR SHOPS AND EACH SAY THAT WELDING THE NUTS SHOULD ONLY BE A TEMPORARY FIX NOT A PERMANENT FIX. SINCE THE WELDING, I'VE HAD STEERING ISSUES WITH MY TRUCK WHICH IS WHY I'VE HAD IT IN THE REAPIR SHOPS. I FEEL THAT THE DRAG LINK NEEDS A TOTAL REPLACEMENT AND NOT A BANDAID FIX. I'VE CONTACTED FCA 2 TIMES AND HAVE A CASE NUMBER, #[XXX]. THE CALLS MADE WERE ON OCT. 10 & OCT 24. I'VE ALSO CONTACTED THE THE RAM DEALERSHIP IN MY AREA WHERE I GET SERVICE DONE AND HAVE "0" RESPONSE FROM THEM. I WOULD LIKE TO KNOW WHAT I CAN DO TO GET THIS ISSUE RESOLVED. THE WELDING OF A STEERING COMPONENT (DRAG LINK) IS NOT A FIX. I HAVE HAD STEERING ISSUES AND DEATH WOBBLE OCCUR SINCE THIS RECALL WAS COMPLETED. I'VE HAD IT IN FOR 2 ALIGNMENTS. I'M WORRIED FOR THE SAFETY OF MY FAMILY NOT HAVING THIS ISSUE TAKEN CARE OF. PLEASE ADVISE ON MY NEXT STEPS THANKS FOR YOUR ATTENTION. When having to brake hard, the lining of the pressure line from the pump to the hydro booster comes apart and causes the brakes to not work and the power steering to act up. The plastic lining shoots into the booster causing it to fail. They don’t make a replacement line so I was forced to fabricate one. Very expensive fix.
Truck was repaired June 2019. They welded the jam nut which made it impossible to get an alaignment.Been having a open claim with Chrysler Jeep and Dodge going on 2 years per claim #78469040 Front Suspension Track Bar Frame Bracket ( vehicle recently started to do the death wobble again)

Chrysler (FCA US, LLC) is recalling certain 2012-2021 RAM 3500, 3500 Cab Chassis, 4500 Cab Chassis, and 5500 Cab Chassis vehicles. Incorrect information in the Service and Owner's Manuals may cause the flanged lug nuts to be over-tightened, which could result in a broken wheel stud and possible wheel separation.
Abhilfe: Dealers will update the torque specification in the owner's manual and other published service documents. Dealers will also inspect the vehicle's wheel studs, replacing them if necessary, free of charge. Owner notification letters were mailed on November 30, 2021. Campaign Y36 owner notification letters were mailed on November 30, 2021. Campaign Y26 owner notification letters were mailed on December 1, 2021. Chrysler (FCA US LLC) is recalling certain 2015-2017 Ram 1500, 2500 and 3500 pickup trucks equipped with an 8 foot bed and a power locking tailgate. Chrysler is also recalling all 2013-2014 and 2018 Ram 1500, 2500 and 3500 pickup trucks built before April 1, 2018 with a 6 or 8 foot bed and a power locking tailgate. On August 29, 2019 Chrysler expanded the recall to include an additional 693,128 model year 2013-2014 Ram 1500, 2500 and 3500 pickup trucks. The tailgate actuator limiter tab may fracture and cause the tailgate to unlatch and open while driving.
Abhilfe: Chrysler will notify owners, and dealers will repair the tailgate latch, free of charge. The recall began May 23, 2019. Chrysler (FCA US LLC) is recalling certain 2013-2018 Ram 3500 and 2014-2018 Ram 3500 Chassis Cab and Ram 2500 vehicles. The outboard steering linkage jam nut may loosen, allowing one end of the drag link to separate.
Abhilfe: Chrysler will notify owners, and dealers will check the steering linkage jam nut tightness, replacing the drag link assembly or welding the nut to the adjuster sleeve, free of charge. Streetcar ORV LLC (Streetcar) is recalling certain American Expedition Vehicles (AEV) 3" Dualsport aftermarket suspension kits, part numbers N0436000 and N0436001, sold for installation on 2013-2018 RAM 2500 and 3500HD trucks. Improper installation and machining of the original equipment steering knuckle may weaken the joint between the steering drag link and knuckle.
Abhilfe: Streetcar will notify owners and dealers will inspect and install a new steering knuckle, free of charge. The recall began December 3, 2018. Owners may contact AEV customer service at 1-248-926-0256, extension 500.
Chrysler (FCA US LLC) is recalling various Dodge, Chrysler, and RAM vehicles equipped with Kidde Plastic-Handle or Push Button 'Pindicator' Fire Extinguishers. A list of the affected trailer models is available at: https://static.nhtsa.gov/odi/rcl/2017/RMISC-17V824-0103.pdf. These extinguishers may become clogged, preventing the extinguisher from discharging as expected or requiring excessive force to activate the extinguisher. Additionally, in certain models, the nozzle may detach from the valve assembly with enough force that it could cause injury and also render the product inoperable. Chrysler (FCA US LLC) is recalling certain 2016-2017 Ram 3500 Cab Chassis with a Gross Vehicle Weight Rating (GVWR) of less than 10,000 lbs, 2011-2017 Ram 3500, 4500 and 5500 Cab Chassis vehicles, 2009-2017 Ram 1500 and 2010-2017 Ram 2500 and 3500 pickup trucks, all equipped with a column shifter. Pushing the brake pedal for prolonged periods when the vehicle is running and in PARK may cause the Brake Transmission Shift Interlock (BTSI) pin to stick in the open position. With the pin in the open position, the transmission can be shifted out of PARK into any gear without pushing the brake pedal or having the key in the ignition.
Abhilfe: Chrysler will notify owners, and dealers will update the vehicle software, inspect the BTSI, and, as necessary, replace it, free of charge. The recall began February 26, 2018. Owners may contact Chrysler customer service at 1-800-853-1403. Chrysler's number for this recall is T79.
Chrysler (FCA US LLC) is recalling certain 2013-2017 RAM 2500, 3500, 3500 Cab Chassis, 4500 Cab Chassis, and 5500 Cab Chassis vehicles, equipped with Cummins 6.7L Turbo Diesel engines that have Concentric-brand water pumps without a vent hole. These water pumps may leak coolant.

Streetcar ORV LLC, dba American Expedition Vehicles (AEV) is recalling certain 2013-2017 RAM 2500 and 3500 heavy duty trucks, modified by AEV to be equipped with cast aluminum Katla 8.5" road wheels. The outer surface of the wheel may fracture, resulting in rapid air loss.
Abhilfe: AEV will notify owners, and dealers will replace the wheels with wheels of similar value, free of charge. The recall began on August 30, 2017. Owners may contact AEV customer service at 1-248-926-0256 extension 500.
Chrysler (FCA US LLC) is recalling certain model year 2011-2014 Dodge Charger Police vehicles, 2007-2013 RAM 3500 Cab Chassis, 2008-2013 RAM 4500 and 5500 Cab Chassis, and 2013 RAM 2500 and 3500 vehicles. The affected vehicles have a 220 amp alternator that may suddenly fail.

Chrysler (FCA US LLC) is recalling aftermarket Mopar Canvas Seat Covers for certain 2012-2016 RAM 1500, 2500, and 3500 trucks. These Mopar canvas seat covers were sold through Chrysler dealerships beginning October 15, 2015. The seat covers can impede the deployment of the seat thorax airbags.

Chrysler (FCA US LLC) is recalling certain model year 2014 Ram 2500 trucks and 2013-2014 Ram 3500 trucks and cab chassis manufactured October 9, 2012, to July 29, 2014. The affected vehicles may have inadequate welds securing the front track bar frame brackets, which may result in the bracket components separating from the frame.

Chrysler (FCA US LLC) is recalling certain model year 2013-2015 Ram 1500, 2500, 3500, 4500, and 5500, 2015 Chrysler 200, Chrysler 300, Dodge Charger, and Dodge Challenger, 2014-2015 Jeep Grand Cherokee, Cherokee, and Dodge Durango, and 2013-2015 Dodge Viper vehicles. The affected vehicles are equipped with radios that have software vulnerabilities that can allow third-party access to certain networked vehicle control systems.
Abhilfe: Chrysler will notify and mail affected owners a USB drive that includes a software update that eliminates the vulnerability, free of charge. Optionally, owners may download the update to their own USB drive from http://www.driveuconnect.com/software-update/ or take their vehicle to a Chrysler dealer for immediate installation. In an effort to mitigate the effects of this security vulnerability, Chrysler has had the wireless service provider close the open cellular connection to the vehicle that provided unauthorized access to the vehicle network. This measure may not have been implemented on all vehicles and does not address access by other means that will be remedied by the software update. The recall began on August 25, 2015. Chrysler (FCA US LLC) is recalling certain model year 2013-2015 Ram 1500, 2500, and 3500 trucks manufactured June 20, 2012, to January 26, 2015. In the affected vehicles, the side impact sensor calibrations may be overly sensitive, and as a result, the side air bag inflatable curtains and seat air bags may unexpectedly deploy and the seat belt pre-tensioners may activate.

The affected vehicles have a steering wheel wiring harness that may rub against the driver air bag module retainer spring.
