The car accelerated brakes did not work or did not function. The. “Brakes “word (brown color ) flashed 2 x. I was going to work I parked on the upper level of the parking structure. AS I was doing rear parking I adjusted the car forward to align with the parking slot . As I moved the car forward it jolted and accelerated. The brakes was not working. The word “brake”. Flashed. 2 times . The car fell to the lower floor. I was fortunate as it was a Sunday no car nor pedestrian was in the lower level but the car beside where I fell suffered damages. I was brought to the emergency department . Paramedics and police were called. My ct of head neck abdomen and pelvis were negative. Ct of chest showed band like contusion comparable with seatbelt injury. I suffered from bruises to my left leg. Right breast lower abdomen. Left clavicle area. History wise: my Honda pilot lost all power on [XXX]. I had it towed to the Honda service center in Cerritos. I was told it was battery issue. Battery was changed A ,month after I crashed. I informed my insurance. I called Honda. They gave me a case number. But said it’s the insurance who will investigate. Per insurance Honda should do the investigation. Up to this day I have no update from Honda. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

1.The vehicle’s airbag unexpectedly deployed while the vehicle was in Park and the driver was attempting to reverse out of a parking space. Multiple airbags deployed, including the driver-side airbags, the front airbag near the windshield area, and the long side/curtain airbag extending toward the passenger side. There was no collision or impact that would reasonably explain the deployment. The vehicle and deployed airbag/SRS components are available for inspection upon request. 2.The unexpected deployment of multiple airbags created a serious safety risk. The driver was struck in the head by the deploying airbag, resulting in dizziness, and was also struck in the chest, causing chest pain. A toddler was also present in the vehicle at the time of the unexpected deployment, creating an additional risk of injury to the child. The sudden deployment also created a risk of distraction, impaired visibility, and loss of control while the driver was attempting to maneuver the vehicle. There was no collision or impact preceding the airbag deployment. 3. No. The unexpected airbag deployment has not yet been reproduced or confirmed by a dealer or independent service center. The vehicle is available for inspection, including the airbag/SRS components and vehicle systems associated with the deployment. 4.Yes. The vehicle was towed to a dealer following the incident and was inspected by the dealer. The dealer provided an estimate of at least 10,000 to repair the damage associated with the airbag deployment. The vehicle has not, to my knowledge, been inspected by the manufacturer, police, or an independent service center. 5. No. There were no known warning lamps, warning messages, or other symptoms indicating an airbag/SRS problem before the incident. The airbags deployed suddenly and unexpectedly while the vehicle was in Park and the driver was attempting to reverse out of a parking space. There was no collision or impact immediately before the airbags deployed.

The state investigator indicated that while the 2022 Honda Pilot was being driven by the owner, the accelerator pedal had malfunctioned and the vehicle suddenly experienced unintended acceleration. During the incident, the vehicle's speed had increased above 100 MPH. The driver was unable to stop or slow the vehicle when pressing the brake pedal or when activating the emergency brakes. A State Trooper Cruiser vehicle was used to forcefully ram into the side of the vehicle to make the vehicle reduce speed and eventually stop. During the incident, the driver sustained undetermined minor injuries. No further information was available. A police report was taken and the vehicle was towed to the local dealer. No further information was available. The manufacturer was not contacted by the investigator. The failure mileage was unknown.

The car has used Automatic braking 3 times for no reason. No one is front of us. It also used automatic braking and then made the car die? Or lose all power and there was no object in front of us, in the middle of the road. We were almost rear ended and the vehicle would not let us move. It was horrible.

My 2022 Honda Pilot failed to engage Drive after reversing, despite the Drive button being pressed. This resulted in an unintended backward motion & a collision with another vehicle. 1.What component or system failed or malfunctioned…? The Electronic Transmission Shift-by-Wire system (push-button shifter) failed to respond to a gear change request. Additionally, related electrical systems including the dashboard infotainment system & the integrated USB charging ports have malfunctioned. The vehicle & its components are available for inspection upon request. 2.How was your safety or the safety of others put at risk? The vehicle experienced unintended motion. While attempting to move forward in a parking lot, the vehicle remained in Reverse despite selecting Drive. This failure led to a low-speed collision with a truck’s tow hitch. This defect creates a high risk of crashes, injuries to pedestrians, or property damage because the vehicle does not move in the direction the driver has selected. 3.Has the problem been reproduced or confirmed by a dealer or independent service center? The vehicle was previously serviced at a Honda dealership for a radio/infotainment recall. Since that service, the transmission has failed to engage Drive multiple times at random. 4.Has the vehicle or component been inspected by the manufacturer…? The vehicle was inspected during the initial recall fix. And brought back because after the fix because the charging port stopped working/continued screen glitches. Following the recent collision, it is awaiting further inspection by a Honda dealership to determine the extent of the mechanical failure. 5.Were there any warning lamps, messages prior…? Symptoms began after the recall fix & included intermittent electronic failures of the charging ports/screen. There were no specific transmission warning lights prior to the gear selection failure; however, the persistent electrical issues in the dash area suggested a systemic electronic malfunction.
I was operating a 2022 Honda Pilot and had just made a left turn into a parking lot. I was slowly pulling into a parking space directly in front of a dance studio with a large floor-to-ceiling window. There were four children and a dance instructor standing inside near the window. As I was coming to a stop to park, my foot was on the brake. At that moment, the engine suddenly revved loudly and the vehicle accelerated forward unexpectedly without any additional throttle input. There were no warning lights, messages, or prior symptoms before this occurred. Despite braking, the vehicle continued forward and struck the exterior of the building. I was able to avoid the window area where the children were standing. The airbag deployed, and the front passenger side of the vehicle was damaged. The building was not structurally damaged, and no one was injured. There were no obstructions such as floor mats interfering with the accelerator, and there was no unintended pedal input. My foot was on the brake at the time of the incident. This appears to be a case of sudden unintended acceleration. The issue has not been reproduced or confirmed by a dealer or service center, and there were no prior warning signs before the event. The vehicle is available for inspection upon request. It has been towed to the body shop. The problem has not been reproduced or confirmed by a dealer or independent service center at this time. The vehicle was inspected by insurance and they did not total it. It is going to be repaired. The vehicle has been inspected by insurance representatives. The manufacturer has not inspected the vehicle at this time. I did not call police because I was not hurt, the kids were not hurt and the building only had a couple of scratches on the brick.
When I started my 2022 Honda Pilot Special Edition (VIN: [XXX] ) in a parking lot, the vehicle immediately behaved abnormally. As I attempted to turn into a space, the car unexpectedly accelerated and moved in an unintended direction. Despite pressing the brake pedal firmly, neither the standard brakes nor the automatic braking system responded. Shifting into Park did not stop the vehicle, and the transmission did not engage to hold the car. The vehicle continued moving through the parking lot, striking property and other vehicles. The entire event felt like a loss of braking and control, with both the brake system and gear selector failing to function as intended. This was a sudden and dangerous incident that could have caused serious injury. The Honda dealer has confirmed there are no open recalls or campaigns for my VIN, but the event aligns with reports of unintended acceleration and brake failure in other 2022 Honda Pilot vehicles. My vehicle is equipped with Honda's Auto Idle Stop system. While driving, the vehicle has chronically suffered from a slow crank symptom. On two occasions, after coming to a complete stop at an intersection and the engine shutting off automatically, the vehicle completely failed to auto-restart when required, taking considerable time to restart and enter Drive. No warning lights or error codes were displayed on the dashboard during or leading up to or after these failures. One of these auto-restart failures occurred in active traffic and nearly resulted in a crash. I mentioned the slow start/crank to the dealer on several occasions (before the "no restarts" occurred); they were dismissive of the issue, stating that "All pilots with Auto Idle Stop feature do this. I've gotten to the point where I try to disable the feature everytime I get in the car but don't always remember. The vehicle remains a safety risk to myself and other drivers on the road. I would love a method of shutting this feature off perminently as to not have to disable it every time I get in the car or accidentally forget!

My pilot is suffering from the known safety defect where the engine stalls and force shifts into neutral during Auto Idle Stop. The dealership advisor handed me Service bulletin 23-010 admitting my vehicle matches the exact failure criteria. However, they are refusing to cover the failed starter motor under the warranty extension, even though Honda’s own technical documents state that the starter failure is a direct consequence of the low cranking torque and the tight engine valves. I need a corporate case manager to open an urgent case and issue a goodwill authorization to cover the starter the 23-010 valve adjustment. We have purchased 8 Hondas in the last 15 years, service has been a main reason for our return but after the experience today I’m not sure we can continue. The problem is a hazard.
