The contact owned a 2021 Honda Pilot. The contact stated that while her friend was driving at an undisclosed speed, with the brake pedal depressed all the way to the floorboard, the vehicle failed to stop as intended. As a result, the vehicle left the roadway, hitting a stop sign before coming to a complete stop after colliding with a concrete wall. The driver and passenger side air bags did deploy. A police report was filed, and injuries were reported. The vehicle occupants transported themselves to an area hospital for treatment. The driver was treated for a head contusion and pain in her legs and lower back; the contact was treated for chest pain. No warning lights were illuminated. The vehicle was towed to a tow yard, pending inspection. After investigating the failure, the contact related it to NHTSA Campaign Number 23V458000 (Service Brakes, Hydraulics), but the VIN was not included. The manufacturer was notified of the failure, and a case was opened. The failure mileage was 5,500.
I am submitting this letter as an official complaint regarding a potential safety defect involving the airbag system in my 2021 Honda Pilot. I was involved in a motor vehicle accident on February 3, 2026 in Atlanta, GA. The collision was a direct frontal impact with another vehicle at approximately 35 mph. The vehicle sustained damage with repair estimates over $8,000. Despite the nature of the crash, the airbags did not deploy. However, post-accident diagnostic information reported by the auto body repair shop indicates that the vehicle’s computer system recorded the airbags as having deployed. This discrepancy raises serious safety concerns, as the system appears to have registered a deployment event that did not physically occur. This could indicate a malfunction in the airbag control module, sensors, wiring, or software. A failure of this kind prevented the proper airbag deployment in a crash where it was needed, posing a significant risk to my safety. To my knowledge, the airbag system had no prior warnings or indicators of malfunction before the accident. Directly following the accident an error appeared on the screen behind the steering wheel stating "Airbag System (SRS) Problem." The vehicle is currently at the body shop and available for inspection.
The contact owns a 2021 Honda Pilot. The contact stated that while the engine was idling and with the gear shifter in the park position, the contact reached into the vehicle from the front driver's door, and suddenly the vehicle began to slowly roll backwards. The contact was pushed to the ground, causing the front driver's side wheel to roll over the contact's right leg. The vehicle continued to roll backwards before making a U-turn and then rolling forward down the inclined street and increasing speed. To avoid having the vehicle roll down hill into a crowded street with pedestrians, the front passenger inside the vehicle turned the steering wheel, causing the vehicle to crash into the rear of another parked vehicle. During the incident, the contact suffered an injured right leg and was transported to the hospital. A police report was taken at the scene, and the vehicle was towed away. The both air bags had deployed, and the vehicle was destroyed. The contact indicated that during the incident, the emergency brake was not activated. The cause of the failure was not determined. The manufacturer and local dealer were not notified of the incident. The failure mileage was 20,000.
On October 18, 2023, at about 11 am, I was approaching to enter our driveway with my foot on the brake system when all of the sudden the car sped up at a very high speed that I was unable to control. The speed was so forceful that the car crashed unto the house resulting in a lot of damage to our house, all this happened in less than one second. I had a next door neighbor in the car with me and thankfully neither of us was hurt, I like to mention at this point that neither air bag deployed to issue protection to us. Of course the safety of both of us was put at risk. We called the insurance with no luck to send an adjuster to inspect the car and/or damage done to the house, so we had to do everything on our own. The car has not been inspected by a representative of the manufacturer nor the police. I did not see any warning nor messages on the screen until the accident happened that said "Crash has been detected" Since we were left to do everything on our own we called Caliber Collision to haul the car to their location to assess the damage done to the car. As of this writing the car is still in their possession and on Friday we received a call from them to inform us what they have found and we were told that as of that day they were sure that the car is 75% totaled. Of course the damage done to the house and car is nothing compared to the damage that it was done to me, because now every time I drive our rental car and approach the driveway I had in my mind if the car is going to do the same thing.
2021 Honda Pilot is subject to "rapid acceleration". We had no notification of this issue until the vehicle drove itself into our home on its own causing damage to the car and our home. Only after we did some research did we know Honda had this issue.
The side curtain airbags deployed while I was sitting in the passenger's side of the car looking into the glove compartment for about ten minutes with the car being completely off and parked in the driveway. The Ignition was off and there was glitching and clicking sounds seconds before the airbags deployed. This could have happened at any point while driving on the street or the freeway. It could have caused an accident and could have killed me if it happened elsewhere. It could have been the passenger's side airbag that deployed and I could have been seriously injured. This incident happened on a Sunday, so I had the car towed to Honda the following day. Honda's response after investigating: "We contacted American Honda Technical Support. Our American Honda District Rep informed us to have the customer contact her insurance provider for repairs." Aside from this official response, I was told that they investigated what they could and found no reason as to why the airbags would have deployed. I was told I had two options. Pay for repair with them or report it to the insurance. I stated that the release of the vehicle was a liability if I decided to not pay for repairs with Honda. After some dialogue, they helped with a loaner car and did not release the car to me. There were no warning lights or messages on the dash whatsoever. About a month prior to this incident, the car started alarming randomly. The random alarming would stop when disconnecting the hood latch sensor. I took the car in for a diagnosis and Honda reported that the cause of the alarm was the keyless access unit. The car (screen and dash) would still glitch after the hood latch sensor was disconnected. My hope is that Honda & American Honda admit responsibility & provide a solution not only for me but for all Honda Pilot owners that may be in danger. Investigating should be a priority, but even if repairs are done to my vehicle, I no longer feel safe driving a my vehicle.
Car slammed on breaks for no reason. Good thing I was paying attention and was able to keep my vehicle under control. My neck hurt for the rest of the day and is Sore today. Now my cars system says the system isn’t working correctly after my car slammed on the breaks for basically no reason. I know this is an ongoing problem certain Honda models already. I believe every Honda with this system should be recalled.
Vehicle spontaneously caught fire. There were no known/indicated issues with the vehicle. Vehicle was eventually extinguished by the fire dept. It took them 45 mins to render it extinguished. Vehicle was parked in my driveway at the time of the incident.
The car's steering wheel got locked out and air bags came out suddenly while car is still moving on the road and driver blocked and not able to do anything and hit the parked car on the street due to car's system malfunction. Since incident happened in the housing community area with speed 20-25 miles and not majorly hit anyone but reported to and inspected by city police inspected. This was the first time this situation happed and however, had other incidents with the car auto feature malfunction, when stop on the red signal and suddenly the card shutdown with lot of beeps. This mentioned to the service center where normally go for servicing activities like oil change etc.

The contact confirmed that the accelerator pedal was not depressed accidentally. No air bags deployed. The contact crashed into unknown private property in a parking lot. No injuries were sustained. No police report was filed. The vehicle was towed to the dealer where the diagnosis was unknown. The vehicle was not repaired. The manufacturer was notified of the failure, offered no assistance, and referred to the NHTSA Hotline. The approximate failure mileage was 47,395.
Car in front of me stopped. My vehicle did not causing a rear end collision. I do not recall the brake warning flashing to alert me to brake. The automatic braking system did not seem to engage. Even though I pressed the brake pedal the vehicle did not seem to want to stop. It reminded me of a vehicle without power brakes. Nor did the wheels lock up or leave any skid marks from hard braking.
The accelerator on my vehicle malfunctioned and got stuck. While making turn with no acceleration, the acceleration of the vehicle got stuck and the brakes did not respond causing the vehicle to hit a sidewalk and breaking the passenger side suspension and control arm and axel of the vehicle. Airbags deployed. Controlled the vehicle until it stopped completely. When vehicle was completely stopped the passenger side still sounded like that gas pedal was fully pressed when the other tires were not moving.
The contact owns a 2021 Honda Pilot. The contact stated that while reversing, the rearview camera failed to turn on, causing the vehicle to hit another unoccupied parked vehicle. There was no warning light illuminated. The manufacturer was notified of the failure and informed the contact that they could not assist because the VIN was not included in a recall. The contact stated that the failure recurred while reversing on another occasion, and the vehicle hit another unoccupied parked vehicle. The contact stated that both incidents were fender-benders. There was no injury sustained. The contact later received notification of NHTSA Campaign Number: 23V-431000 (Back Over Prevention) that she related to the failure. The dealer was contacted and an appointment was scheduled for recall repair. The vehicle was not diagnosed nor repaired. The manufacturer was notified of the failure. The failure mileage was approximately 25,000.
The controls froze while driving and I was unable to safely maneuver the vehicle and it crashed in nearby bushes. I don’t see any recall for the VIN, Also we asked the motor repair team run scan though nothing was found. But would like to report this as we still feel it is some mechanical failure
Auto braking applies randomly cause brakes to lock up happens when driving in any conditions. This problem has already caused a minor accident with my vehicle due to brakes locking up and no throttle

The rear subframe may corrode at the suspension mounting points, which can result in rear suspension component failure.

Interim letters notifying owners of the safety risk were mailed July 27, 2026. Additional letters will be sent once the final remedy is available, anticipated in the fall of 2026. The tie rod fastener that connects the brake booster and the brake master cylinder may have been improperly assembled during manufacturing, which can cause the brake master cylinder to separate from the booster assembly.

Due to a faulty Media Oriented Systems Transport (MOST) communication coaxial cable connector, the rearview camera image may not appear on the display. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."
Abhilfe: Dealers will replace the MOST cable harness and install a straightening cover over the vehicle cable connector, free of charge. Owner notification letters were mailed July 24, 2023. The heating pads behind both side-view mirrors may not be bonded properly, allowing the mirror glass to detach. American Honda Motor Co. (Honda) is recalling certain 2021 Pilot vehicles equipped with Continental CrossContact LX Sport 245/50R20 102 H tires with DOT serial number A376 D3K9 2920 and mold numbers S-421920 and S-421921. The tires may have been cured for a time beyond the specification limits.
Abhilfe: Honda will notify owners, and dealers will inspect all four tires and replace the tires that have the affected DOT serial number and mold number, free of charge. Due to a faulty circuit board in the passenger seat weight sensor, the passenger seat’s occupant detection system may identify a Steering Wheel Sensor (SWS) error and the Supplemental Restraint System (SRS) warning light could switch on. This can cause a faulty triggering of the airbags, creating a risk of injuries for a child occupying the passenger seat. The product does not comply with the Regulation on the approval and market surveillance of motor vehicles and their trailers, and of systems, components and separate technical units intended for such vehicles.
