I WAS ON A CITY STREET MOVING AT ABOUT 45 MPH. A TRUCK TURNED IN FRONT OF ME AND WE COLLIDED HEAD TO HEAD. DAMAGE ON MY VEHICLE WAS PRIMARILY ON THE FRONT DRIVERS SIDE.

I was not made aware of the recall. The driveshaft separated while I was driving, causing significant damage to other parts of the vehicle, including the transmission, motor and transmission mount, cooling, electrical and brake and transmission lines, oil and fuel parts, and much more underneath the van. This happened after a very brief hard vibration moments before it “exploded” and sent me flying into the air and coming to a very quick stop, which also caused medical issues. Ford is refusing to repair all that their recalled part damaged claiming they “notified me” but I haven’t received such notice. I stay up on all maintenance and repairs for my van, and the dealership acknowledged that when looking at the vehicle history reports.

AFTER ARRIVING HOME IN OUR 2016 FORD TRANSIT VAN, WE PARKED IN OUR DRIVEWAY AND PLACED THE TRANSMISSION INTO THE PARK POSITION AND SET THE EMERGENCY BRAKE. OUR FAMILY OPENED THE DOORS AND PROCEEDED TO EXIT THE VEHICLE. OUR FOUR YEAR OLD SON, HOWEVER, HAD UNBUCKLED AND TWISTED AROUND IN HIS SEAT SUCH THAT HE BECAME TRAPPED IN HIS SEAT BELT. THE MORE HE STRUGGLED TO GET FREE THE MORE IT RATCHETED TIGHT... TO THE POINT THAT HE COULDN'T BREATH. DESPITE US TRYING EVERYTHING TO FREE HIM, THE SEAT BELT WOULD NOT RELEASE HIM. FEARING THAT HE WAS IN REAL MEDICAL DANGER, I CUT HIM FREE THEREBY RENDERING THAT SEAT BELT UNUSABLE. I AM CONCERNED THAT THE SEAT BELTS ARE DEFECTIVE IN THAT THEY FAILED TO RELEASE PROPERLY WHEN THE VAN WAS PLACED IN PARK AND THE DOORS WERE OPENED.
I was going up hill and I heard a pop and smoke started to come out form the center console and then the truck burst into flames

The rearview camera on my 2016 Ford Transit 150 intermittently fails while the vehicle is in reverse. At times, the rearview mirror display shows a blue/blank screen instead of the rear camera image. At other times, the camera image appears reversed/inverted. Ford Safety Recall 25S89 includes certain 2016 Ford Transit vehicles for a rearview camera condition that can result in a distorted, inverted, or blank image. My vehicle is exhibiting these same symptoms, but VIN [XXX] is not currently included in the recall. I am concerned that the population of vehicles included in recall 25S89 may not include all 2016 Ford Transit vehicles affected by this rearview camera defect. The intermittent loss or incorrect orientation of the rearview image reduces rear visibility while backing and creates a safety concern.
