09/17/23 Was driving 65mph, car was slowing Down in front of me and turned cruise control off. Immediately car started stalling. Was in middle lane with cars on both sides. Turned on flashers and no power in gas pedal. At about 30mph, was struck from behind by Jeep! Third car behind struck Jeep and Jeep struck me again. There were injuries and a death. My car was towed. 10/02/23; Now car is in body shop. Contacted Chrysler, they inspected car, but did not drive it. 10/12/23 Just received email stating that they found no problem. They stated that they are not responsible as they found no evidence of manufacturing defect or recall having contributed to this accident. Insurance companies and police are still investigating.
Traveling down a country road on a clear afternoon, the vehicle traveled approximately 35 mph while cresting a small hill. On the other side of the hill was a "T" shaped intersection where another vehicle stopped in the intersection from a stop sign several feet behind. The other driver failed to complete the turn when they saw oncoming traffic. The driver noticed the intersection had an obstructed path and attempted to move into the oncoming traffic lane since no oncoming traffic was visible. Just then, the onboard computer installed in the control panel of the drivers seat had acted upon either the obstruction or the switch in lanes and assumed complete control of the braking and steering systems. The driver was unable to retrieve control from the computer systems artificial intelligence before a direct crash occured with a telephone pole. Rendering the vehicle dangerous as live wires draped the saturated earth below the tires and feet of passengers exiting the vehicle. The manufacturer was contacted, but no follow up has been made. Neither the police or insurance representatives have been helpful or any investigation made into this account. There were several lights, sensors, noises, and messages that were flashing and beeping but none were able to be understood before the sudden impact.
I was in an accident. Where the van rolled 17 times. On or about the 12th roll. The retaining lock on the shoulder strap released. This allowed me to slide out of the seatbelt. This caused me more complicated injuries. The vehicle is currently at copart impound lot in Oklahoma City, OK. Will be available for inspection upon your request. The Oklahoma highway patrol report indicated. I was wearing my seatbelt but the retaining lock on shoulder strap had broken. The Oklahoma highway patrol report # OHP25-024333. There were no warning lights or messages to indicate that there was a problem.
The contact owned a 2022 Chrysler Pacifica. The contact stated that while driving approximately 55 MPH, a very loud snapping noise had suddenly occurred, and the message "Stability System Activated" displayed before the steering wheel suddenly turned to the right, causing the driver to lose control of the vehicle to crash and roll over multiple times. The police and first responders were called to the scene, and the occupants were transported to the hospital. During the crash, the driver sustained multiple neck and back fractures and a right leg injury from the seat belt strap. The front passenger sustained multiple rib fractures. The vehicle was destroyed and later towed away. The cause of the failure was not determined. The manufacturer and local dealer were not notified of the failure. The contact also indicated that due to the injuries sustained in the crash, his right hand is unusable. The VIN was not available. The failure mileage was 5,000.
The contact's mother had rented a 2022 Chrysler Pacifica Hybrid. The contact stated that upon his mother entering a gas station at a low rate of speed, she hit a divot in the road which triggered the vehicle's phantom braking to engage without warning. As the vehicle came to a sudden halt, the air bags deployed without incident. Due to the failure, his mother was taken to the hospital where she was treated for a concussion as well as multiple injuries to her neck, hips, nose, and right thumb; she also sustained severe burns and bruising to both hands and arms. A police report was filed. The vehicle was towed to an independent mechanic; however, the whereabouts of the vehicle are unknown. The dealer nor the manufacturer had yet to be notified of the failure. The failure mileage was unknown.
The sliding driver side rear door malfunctioned - either a sensor did not go off, or the door did not register the sensor. My 4-year-old female child pressed the button to automatically open the sliding door, and attempted to climb in the vehicle as it was sliding open. She accidentally hit the button again as she entered, and the automatic sliding door began to close. Her head became trapped between the sliding door and the vehicle frame, and the door continued to attempt to shut, crushing her head. She screamed and I pressed the button again, and the door began to open, letting her loose. She was in significant pain and did not settle quickly; she has a large knot and bruising on her head where the automatic sliding door trapped her, and has a headache. The door should have registered that her body was in the way of closing, and additionally should have stopped and re-opened when it made contact with her head.
The vehicle caught fire earlier morning of 7/9/2026. It was sitting in front of house unoccupied. The alarm went off twice which pressing the key fob shut it off until the third time when the alarm would continue until it died down. I went to my door to check and noticed the car was on fire.
The contact owns a 2022 Chrysler Pacifica. The contact stated that while driving at 80 MPH with the cruise control activated, the vehicle experienced phantom braking. The following day, while driving at an undisclosed speed, the vehicle was vibrating and the brakes failed to immediately stop the vehicle. The contact stated that the brakes caught fire, and there were flames coming from the brake calipers. The contact was able to put out the fire with a hose. The vehicle was towed to the dealer, where it remained for twenty-one days; however, no cause of failure was found. The failure recurred, and the vehicle was taken to an independent mechanic, who recommended the contact stop driving the vehicle as it was unsafe on the roadway. The vehicle was not repaired. The manufacturer was made aware of the failure and dispatched an inspector to the dealer to inspect the vehicle, but the report was not yet available. The failure mileage was approximately 13,000.

Subject: Urgent Safety Concern – Chrysler Pacifica Hybrid (VIN: [XXX] ) Dear NHTSA, I am writing to report a serious safety issue with my 2022 Chrysler Pacifica Hybrid (VIN: [XXX] ). The vehicle experiences sudden power loss due to the following error codes: P079A-00 (transmission issue), P0A43-00 (hybrid battery issue), and P0A7D-00 (battery voltage issue). These problems have nearly caused accidents, including: On the highway: Power loss while overtaking, leaving me in a dangerous situation. In city traffic: Repeated power loss, almost causing collisions. The issues are well-documented among Pacifica Hybrid owners in the USA. NHTSA has received numerous complaints, and there are active class-action lawsuits due to unresolved problems. My vehicle was involved in a minor collision in the USA, classified as "clear title" (cosmetic damage only, no impact on the drivetrain or battery). Repairs were done professionally, and there is no connection to the current issues. I have contacted Chrysler and the authorized service center in Berlin, but received no response. This issue poses a direct safety risk, and I urge NHTSA to investigate and ensure a permanent solution. Attached: Error codes and incident descriptions. I was driving my Pacifica with my special needs children to thier therapy appointments. The power steering light came on all of a sudden going down the highway at 50-55mph. Steering became extremely difficult. I was barely able to take the the turn to pull into the parking lot. This is even more dangerous as their is a drop-off on the other side if i had not managed to turn aggressively enough. It scared both of my children and myself. The dealership wanted $206 diagnostic fee which we cannot afford. The light will go off and we regain steering once it has been off for a few minutes then light kicks on and we lose steering again.
I am filing a safety complaint regarding Chrysler Corporate's handling of recall campaign 06D (NHTSA 26V-189) for my 2022 Chrysler Pacifica.I received an official corporate notification via mail explicitly stating that parts were now available to remedy this active safety recall. However, when I immediately contacted my authorized Chrysler dealership to schedule the critical safety repair, I was turned away and informed that the parts are completely unavailable and backordered with no estimated release date.Sending out official notices claiming an open safety risk can be fixed when the necessary components are entirely missing from dealer inventory is a highly deceptive and unsafe practice. It causes vehicle owners to waste time coordinating service under false pretenses while forcing us to continue operating family vehicles with known, unaddressed safety hazards. Chrysler must be held accountable for coordinating its corporate communications with actual supply chain availability so owners are not driving compromised vehicles under the false impression that a fix is ready. It is alarming that the post card they just sent me had big letters "URGENT SAFETY RECALL" headline and the recall site says the parts are in and the dealership says they only gets parts to fix 2 a week and it will still be close to 6 months before the parts are available.
Vehicle suffered sudden loss of motive power due to Diagnostic Trouble Code P0A43 (Drive Motor A Position Sensor Circuit Intermittent). The failure is caused by thermal and chemical degradation of the internal transmission wiring harness, resulting in an internal electrical short submerged in transmission fluid. The vehicle is able to be powered on again and driving can be continued however certain speeds trigger the issue. The check engine light appears and remains for a short period. The vehicle was purchased through CarMax of Naperville, IL [XXX]. The vehicle began to experience the defined issue the last week of May 2026. The vehicle was brought to CarMax [XXX]. CarMax transferred the repair of the vehicle to Castle Chrysler of Naperville/Stellantis IL, and they conducted the initial repair. Stellantis attempted to mask this hardware failure via software recalibrations under NHTSA Recall 03A (23V-010) rather than replacing physical parts. The vehicle was returned and appeared to be repaired. [XXX] the same issue occurred while on a cross country road trip. The vehicle was able to be driven but under specific speed conditions as not to activate the transmission failure. [XXX] the vehicle was brought to an independent technician at Stephens Automotive in Palatine, IL and they were able to verify the issue and documented it. The vehicle was then brought to Castle Chrysler on [XXX], and the vehicle has been there since. Stellantis later acknowledged the true physical failure via Warranty Extension XE1 and TSB 21-014-26, requiring Electric Rebuild Kit #68729719AB. The vehicle has been continuously grounded at Castle Chrysler since [XXX] (30 cumulative days) because Part #68729719AB is on indefinite backorder. Stellantis's failure to supply hardware repairs leaves the vehicle inoperable and creates a severe safety hazard on public roads. Stellantis Case #[XXX]. Having the same issues as earlier models of Chrysler vans with the steering rack tension when going around curves. It’s been doing this for a couple of months now.
The contact owns a 2022 Chrysler Pacifica. The contact received notification of NHTSA Campaign Number: 26V189000 (Air Bags); however, the part to do the recall repair was not yet available. The local dealer was contacted; however, the vehicle was not diagnosed or repaired due to parts being on backorder. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. In addition, the contact was referred to another local dealer as per the manufacturer; however, parts were not yet available. The manufacturer was made aware of the issue. The contact had not experienced a failure.

Chrysler (FCA US, LLC) is recalling certain 2020-2022 Pacifica Plug-In Hybrid Electric (PHEV) vehicles. The battery pack may cause a vehicle fire, even when the vehicle is parked with the ignition in the "Off" position.
Remedy: Owners are advised not to charge their vehicles, and to park outside and away from structures, until their vehicle is repaired. Dealers will update the high voltage battery pack control module software. In addition, dealers will inspect and replace the battery pack assembly, as necessary. All repairs will be performed free of charge. Owner notification letters were mailed beginning June 23, 2026. Owners may contact FCA US LLC customer service at 1-800-853-1403. Chrysler (FCA US, LLC) is recalling certain 2022-2026 Pacifica and Voyager vehicles. The right and left side curtain air bags may have insufficient pressure retention from improperly sealed seams. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 226, "Ejection Mitigation."
Remedy: Dealers will replace the side curtain air bags, as necessary, free of charge. Owner notification letters were mailed May 14, 2026. Owners may contact Chrysler customer service at 800-853-1403. Chrysler's numbers for this recall are 06D, 10D, and 11D. Vehicle Identification Numbers (VINs) involved in this recall will be searchable on NHTSA.gov beginning April 2, 2026. This recall expands and replaces previous NHTSA recall numbers 25V302 and 25V573. Vehicles previously repaired under a prior recall will need to have the new remedy completed.
Vantage Mobility, LLC (VM) is recalling certain 2021-2026 Honda Odyssey, 2021-2025 Chrysler Pacifica, and Toyota Sienna vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. The retractors may not lock, preventing the wheelchair from being properly secured.
Remedy: Dealers will inspect and replace the retractors, free of charge. Owner notification letters was mailed January 27, 2026. Owners may contact Vantage Mobility customer service at 1-800-488-9082.
Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.
Remedy: Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

Chrysler (FCA US, LLC) is recalling certain 2020-2023 Pacifica and 2020-2021, 2023 Voyager vehicles. An improperly secured connector for the side curtain air bags may prevent the air bags from deploying. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard numbers 214, "Side Impact Protection" and 226, "Ejection Mitigation."

Chrysler (FCA US, LLC) is recalling certain 2021-2022 Dodge Durango, 2021-2023 Chrysler Pacifica, Jeep Grand Cherokee L, 2022 Ram 1500, 2500, 3500, Chrysler Voyager, 2022-2023 Jeep Compass, Grand Cherokee, Wagoneer, Grand Wagoneer, and Ram Promaster vehicles. The radio software may prevent the rearview image from displaying. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

Chrysler (FCA US, LLC) is recalling certain 2017-2023 Pacifica Plug-In Hybrid Electric vehicles (PHEVs). An internal transmission wiring connector may short circuit, resulting in an unexpected engine shutdown.
Remedy: Dealers will update the Power Inverter Module (PIM) software and, if necessary, update the instrument panel cluster (IPC) and the electronic control module (ECM) software, free of charge. FCA US will notify dealers and begin notifying owners on or about: 03/21/2023 for 2021 model year vehicles, 05/02/2023 for 2019-2020 model year vehicles, and 06/15/2023 for 2017-2018 model year vehicles. Chrysler (FCA US, LLC) is recalling certain 2022 Voyager and Pacifica vehicles. The windshield wiper arm nuts may be improperly tightened, which can cause the wipers to not function properly.

Chrysler (FCA US, LLC) is recalling certain 2021-2022 Pacifica and 2021 Voyager vehicles. The second-row seat-to-floor mounting latch may bind and stick open, preventing the seat from properly securing to the floor. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 225, "Child Restraint Anchorage Systems."
