Airbags, seats, and headrest all failed. State Farm insurance currently has the vehicle. I do not know if the people has been inspected by anybody. The vehicle had no warning lamps message or other symptoms of problems or failure before the collision. I was traveling in the far left lane when traffic came to abrupt stop and I was able to stop 10 to 15 ft from the vehicle in front of me. Two vehicles in my line behind me swerved out of the way. Then a semi truck with a trailer rear-ended me. The front end of the semi truck struck the rear end of my vehicle getting lodged inside my vehicle. My car was then pushed into the car in front of me and dragged 100 to 200 yards down The highway by the semi. The seat belts did not lock. The airbags did not deploy. Both front seat frames were bent and ripped apart. The rear seat that's became detached. The headrest for the front seats flew off. All four seats were occupied by adult males all weighing around 190 pounds or above. All four occupants were wearing seat belts. I the driver was knocked unconscious and received a concussion plus facial damage/injuries. Rear passenger side occupant received a broken jaw and a skull fracture. The other two passengers were also injured. All four passengers of the vehicle were transport to emergency room via ambulance.

The contact stated that both vehicles were towed away from the scene of the crash. The contact stated that their vehicle was repaired. The manufacturer was not informed of the failure. THE CAR WENT OFF THE ROAD AND INTO TREES TOTALING THE VEHICLE - AIRBAGS DID NOT DEPLOY.
My pregnant wife is parking her car at her work when she accidentally step on the gas pedal instead of the brakes. She panicked and she drove all the way to the wall. Airbags were not activated. 911 was dispatched. Police report made. Only minor injury finger fracture and chest bruise. There was a severe frontal damage to the car. Insurance deemed it totaled due to frame damage. I also complained to Toyota. They hired a software engineer to investigate. Just received official letter from Toyota there weee no defects in their safety system. The issues are the airbags not being deployed at the time of impact and pre collision auto braking non existent. The impact on the wall according to the people inside the nursing she works felt like an earthquake inside. But our main concern is the auto brake feature did not function. This accident could have been worse. Good thing seatbelt was not an inch lower from her chest preventing injury to our unborn child. The impact could have been worse if it was made of wood and could have injured other people in the nursing home. The auto brake feature should have worked because there were many obstacles at her front including first the bushes and eventually the wall. Their response letter is frustrating for us because they are claiming there were no problems in the car. But our concern is their safety system is not reliable compare to other car companies. Kia and Honda safety system is more reactive based from my driving experience. I think there are lapses in their system.

The contact’s left wrist was burned due to the air bag deployment. The contact received medical treatment for the injuries at the scene. A police report was filed. The vehicle was towed to an independent mechanic. The manufacturer was made aware of the failure and a case was filed. The failure mileage was approximately 5,500.
Novemver 18, 2025 "I was driving my 2024 RAV4 .. suddenly - I only heard the glass shatter, not an impact on my sunroof glass. After 10mins driving (7:10AM) Speed 65 mph in freeway It was not raining when I started driving but a little windy and splash of rain started No impact or debris and I am far away from cars in from of me The remaining glass edges (show outward shattered)
On 9/06/2026 my 2024 Toyota RAV4 Hybrid was involved in a severe collision where the impact occured on the drivers side at the passenger door. The vehicle spun several times before coming to a stop. During the spins the front passenger door flew opening. My wife was sitting in the passenger seat her seatbelt successfully restrianted her preventing ejection and injury. However the unlatching of a non impacted door under rotational forces represents a catastrophic structural or mechanical latch failure that poses an imminent risk occupant ejection.

Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.
Rimedio: Dealers will update the instrument panel software over-the-air (OTA) for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2024 RAV4 and 2025 Lexus NX vehicles. Some of the bolts that secure the front brake caliper and the wheel hub bearing may be loose, which can result in a damaged caliper brake hose or possible wheel detachment.
Rimedio: Dealers will inspect and tighten the front brake caliper mounting bracket and hub bearing bolts, as necessary, free of charge. In addition, any damaged components will be replaced, free of charge. Owner notification letters were mailed January 23, 2025. Owners may contact Toyota customer service at 1-800-331-4331.
