THE FRONT PASSENGER SEAT HEATING SYSTEM CAUGHT FIRE AND BURNED THROUGH THE FABRIC AND MY DOG'S SEAT. THE FUMES MADE ME AND MY DOG NAUSEOUS.I HAD TO SWERVE ACROSS THE HIGHWAY AND STOP ON THE SHOULDER TO EXTINGUISH THE SEAT FIRE/BURN. THE FUSE WAS PULLED FOR SAFETY. THE SEAT IS IN STORAGE FOR INSPECTION. MAZDA WILL BE CONTACTED.
The contact owned a 2016 Mazda CX-5. The contact stated while driving 35 MPH, another vehicle going 110 MPH crashed into the rear of the vehicle. The air bags failed to de[ploy. The contact hit his head against the front driver's side window and he became unconscious. The contact stated upon regaining consciousness, he noticed that he was laying flat on his back as the seat had fractured. The contact also noticed that the seat belt had fractured into two. The front of the other vehicle caught on fire and the fire expanded to the rear of the contact's vehicle. The emergency units and State police patrol assisted the contact out of the vehicle through the driver's side door. The emergency units transported the contact to the hospital, where he was informed that he had sustained head, neck, and back injuries. Additionally, the contact was informed that he had sustained a stroke on the left side of his body. A police report was filed. The vehicle was towed to a tow lot, where it was totaled. The dealer and the manufacturer were not notified of the failure. The failure mileage was approximately 58,000. The VIN was unavailable.

MEDICAL ATTENTION WAS NOT YET SOUGHT. A CARDIOLOGIST WAS CONTACTED AND AN APPOINTMENT WAS MADE. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO AN INDEPENDENT REPAIR FACILITY. THE VEHICLE WAS NOT TAKEN TO BE DIAGNOSED. NEITHER THE DEALER NOR THE MANUFACTURER WAS CONTACTED OR NOTIFIED OF THE CRASH. THE VEHICLE WAS NOT REPAIRED. THE APPROXIMATE FAILURE MILEAGE WAS 15,000.

"TAKATA RECALL" ? IN MARCH 2018, I WAS TRAVELING WESTBOUND IN A MAZDA CX-5 ON AN EXPRESSWAY IN LONG ISLAND, NEW YORK. IT WAS APPROXIMATELY 9;45 PM AND THE EXPRESSWAY WAS FAIRLY EMPTY, ROADS WERE COMPLETELY CLEAR, NO RAIN, NO SNOW. OUT OF NOWHERE, A SPEEDING DRIVER SLAMMED INTO THE BACK OF MY MAZDA AT A VERY HIGH RATE OF SPEED. (SHE ADMITTED TO FOLLOWING ME TOO CLOSELY AND NOT BEING ABLE TO STOP IN TIME, SO MY CAR WAS HIT PRETTY HARD. ON POLICE REPORT, SHE WAS CITED FOR UNSAFE SPEED AMONG OTHER THINGS. I WAS ALSO INFORMED THE A POLICE OFFICER THAT SHE DID NOT HAVE A DRIVER'S LICENSE). MY CAR WAS THEN THRUST INTO THE DIVIDER (WHERE THE LEFT SIDE APPROXIMATE CORNER OF THE FRONT OF THE CAR HIT THE MEDIAN) IN THE MIDDLE OF THE EXPRESSWAY AND SPUN QUITE A FEW TIMES. NONE OF THE AIRBAGS EVER DEPLOYED. I HIT MY HEAD AT LEAST TWO TIMES AND I STARTING HAVING NECK AND BACK PAINS AMONG OTHER INJURIES. THOUGH THE CAR HAD EXTENSIVE EXTENSIVE DAMAGES (13K WAS THE LAST TIME I HEARD AND THE CAR IS STILL NOT WORKING FULLY PROPERLY) MY INSURANCE PROGRESSIVE FORCED ME TO TAKE THE CAR BACK WHILE THE OTHER INSURANCE COMPANY DOESN'T UNDERSTAND WHY THE CAR WAS NOT TOTALED. I WAS GIVEN BACK THE CAR A LITTLE OVER TWO WHOLE MONTHS AFTER THE ACCIDENT AND I WAS TOLD THE AIRBAGS WILL NOT DEPLOY IF GOD FORBID SOMETHING ELSE HAPPENS (THE AIRBAG LIGHT IS BLINKING ALL THE TIME), THE HORN DOES NOT WORK, AND NOTHING ON THE STEERING WHEEL CONTROLS WORK ANYMORE. I WAS TOLD I HAVE TO BRING THIS TO MAZDA'S DEALERSHIP DIRECTLY FOR DIAGNOSTICS. I HAVE BEEN TOLD BY MANY PEOPLE THAT THEY BELIEVE MY AIRBAGS MALFUNCTIONED INCLUDING MAXIMUM AUTO BODY IN BOHEMIA WHO FIXED THE CAR AND I AGREE CONSIDERING THE HARD HITS THE CAR TOOK AND MY INJURIES. I AM NOT SURE WHAT MY RIGHTS ARE IN THIS SITUATION BUT I FEEL THIS CAR IS NOT SAFE. I NOW HAVE TO WAIT UNTIL JUNE 4TH TO GET IT TO THE MAZDA DEALERSHIP.
I WAS PULLING INTO A PARKING SPACE AT NATURAL GROCERS. WHEN THE CAR WAS GOING 2-3 MPH INTO THE SPACE GETTING READY I THOUGHT TO STOP, THE ENGINE REVVED UP LIKE A JET ENGINE. THE CAR THEN JUMPED A CONCRETE SIDE WALK, HIT THE BRICK BUILDING, AND BROKE A GAS LINE. THE CAR THEN CAME TO REST BACK IN THE PARKING SPOT. THE AIR BAG DEPLOYED, MY FIRST REACTION WAS "WHAT THE HELL JUST HAPPENED". A MAN WALKING OUT OF A STORE HEARD THE HISS OF THE GAS LINE AND THEN CALLED 911. HE CAME TO THE CAR AND TOLD ME TO MOVE AWAY BECAUSE THE GAS LINE COULD BLOW UP. HE NOTICED THAT MY GLASSES WERE BACKWARDS ON THE BACK OF MY NECK AND ASKED IF I WAS HURT. SHORTLY A FIRETRUCK ARRIVED, TWO POLICE CARS, AND A STORE MANAGER. THE FIREMEN SHUT OFF THE GAS LINE. THE POLICEMAN INTERVIEWED ME AND OBSERVED THE SETTING. AFTER SOME TIME THE POLICEMAN SAID "THIS WAS NOT YOUR FAULT" AND "I AM NOT ISSUING A CITATION". SOMETHING CLEARLY WENT WRONG WITH THE CAR. HE ENCOURAGED ME TO TAKE PICTURES OF BOTH THE SIDEWALK AND THE CAR FOR MY RECORDS. I HAD THE CAR TOWED TO THE DEALERSHIP FROM WHOM I HAD LEASED THE CAR.
2 INCIDENTS OF SUDDEN ACCELERATION WHILE MAKING A RIGHT HAND TURN INTO PARKING SPACES FIRST ON 5/18/17 AND SECOND ON 12/15/17. THE AIR BAGS DEPLOYED THE FIRST TIME BUT NOT THE SECOND TIME. THE INSURANCE COMPANY IS ORDERING AN ENGINEER INVESTIGATE SINCE IT OCCURRED ON 2 SEPARATE OCCASIONS

THE CONTACT'S VEHICLE CONTINUED TO SURGE AND CRASHED INTO A POLE. THE DRIVER AND PASSENGER FRONTAL AIR BAGS DEPLOYED. THE CONTACT SUSTAINED INJURES TO THE CHEST AND ARMS, WHICH REQUIRED MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO AN INDEPENDENT BODY GARAGE. THE MANUFACTURER WAS NOT NOTIFIED. THE FAILURE MILEAGE WAS 28,000.

PARKED MY CAR. OPEN UP DRIVERS DOOR , RADIO AND DASH LIGHTS CAME ON. STANDING ON DRIVEWAY NOW PUSH BUTTON TO TURN OFF RADIO AND DASH. EVERYTHING CAME BACK ON. DID IT THREE TIMES AND ON THE THIRD TIME VEHICLE STARTED TO ROLL BACKWARDS. HAD TO CURLY ON THE FLOOR BARD AND DOOR WIDE OPEN. VEHICLE GAINING SPEED, ENGINE OFF, LOOK AT GEAR SHIFT SAYING IT'S IN PARK STILL. I HAD TO JUMP OUT OF VEHICLE BECAUSE I THOUGHT I WAS GOING TO DIE. NEIGHBOR CAME TO HELP ME OUT AND CALLED 911 WHO SENT AN AMBULANCE AND POLICE. PARAMEDICS ATTENDED TO MY NEEDS AND RUSHED ME TO THE HOSPITAL. POLICE ASK THE NEIGHBORS IF SOMEONE WAS INSIDE THE VEHICLE AND THEY SAID NO. POLICE REPORT SHOWS VEHICLE STILL IN PARK. VEHICLE HIT A TREE AND HIGH CENTER ON A PGE ELECTRICAL BOX. NOW VEHICLE IS AT A DEALERSHIP WHO TOLD MY FATHER UNDER THE TABLE THEY WERE ABLE TO DUPLICATE CONCERNS BUT WILL NOT NOTE IT ON THE REPAIR ORDER. THEY LOOKED AT SENSORS AND KNOW CODES APPEARED. ONCE MY FATHER MENTION HE WORKED AT A DEALERSHIP THEY MENTION THAT I MUST HAVE HAD THE VEHICLE IN REVERSE. IF I STAND OUTSIDE OF THE VEHICLE AND FOR A GOOD MINUTE TO TWO MINUTES AND IT SHOWED IN PARK THE WHOLE TIME, THERE IS SOMETHING ELECTRICAL GOING WRONG WITH THE VEHICLE. IF YOU GOOGLE RADIO IS STAYING ON WHEN VEHICLE IS SHUT OFF REGARDING THESE MAKE AND MODEL THERE ARE SEVERAL CASES OF THIS HAPPENING. PEOPLE SAY THEY HAVE TO SLAM THE VEHICLE INTO PARK TO GET THE RADIO TO WORK. SOME PEOPLE HAD A BRAKE SENSOR REPLACE WHICH FINALLY FIXED THE CONCERN....UPDATED 03/29/17 *BF *CN
Leather heated seats .. safety put at risks because I suffered burns on my bottom (butt) from the driver seat getting too hot to the point where it burned the seat itself as well , and made a brown burn mark on the seat … Both of my cheeks got burned .. this can in the long run cause skin damage and worse case scenario cancer. No it hasn’t been inspected No warning lamps
Parked by the curb at the Ft. Lauderdale International Airport. I put the car in Park and left the car idling with the air-conditioning running.as it was 95 degrees and about 100 at around 8:00 AM. There was a shuttle van parked by the curb about 11 feet in front of me. I had to exit my car to talk to the driver in the car behind me. I walked to the car behind me and started talking to the driver. I glanced back at my Mazda CX5 and it started rolling forward. I ran up to the car and got the door open and stepped on the brake. The brakes did not engage. I repeated stepping on the brake several times and the car would not stop. I got in the drivers seat to shift the car into reverse and keep my foot on the brake pedal. The car would not shift into reverse. and by this time the Mazda had pinned to the shuttle van a lady who was trying to unload her luggage. On the sixth attempt to shift into reverse the shifter finally moved into reverse and by standers came to the aid of the lady that had been pinned to the shuttle van. Airport security and the Broward County police took the statements. The lady retrieving her baggage declined medical attention. When I checked to see what the police were reporting they did not acknowledge that I was attempting to stop a runaway vehicle. The report said that I failed to put the car in park and left it in drive. It is not possible to get out of a car that is idling and in gear without the car moving forward or backward before you can even get all the way out of the car. This incident happened about mid July in 2017. Near the end of June this year while in my driveway I left the car idling and in park, exited the car walking to the rear of the car and it dropped into reverse. Got back in the car and shut off the engine. July 2017 the local Mazda dealer inspected the vehicle and could not find as problem. Not unlikely with an intermittent electrical anomaly. There were no prior warning lamps in either incident.
I noticed smoke filling the inside of my car. I looked over at the passenger seat, and my coat was on fire. I poured my drink on it, pulled over, and threw the coat out of the car. I saw a large hole in the lumbar area of the passenger seat, and it was glowing red and putting out a lot of heat. The seat heater was on, so I immediately turned it off. It was obvious that the fire has started inside the seat, and there was no other fire source in car--ie. lit cigarette, lighter, etc. If someone had been sitting there, they would have serious burns. The seat will need to be replaced.
I was driving down the road and started smelling smoke then looked over and my passenger seat was in flames.

Mazda North American Operations (Mazda) is recalling certain 2016 CX-5 vehicles. The LED Daytime Running Lights (DRL) may fail while driving.
Rimedio: Mazda will notify owners, and dealers will inspect the headlights, and replace the sealing gasket or headlight assemblies, as necessary, free of charge. The recall began March 20, 2020. Owners may contact Mazda customer service at 1-800-222-5500 Option 4. Mazda's number for this recall is 4320A.
Mazda North American Operations (Mazda) is recalling certain model year 2010-2013 Mazda3 and Mazdaspeed3 vehicles manufactured November 18, 2008, to June 8, 2013, 2012-2015 Mazda5 vehicles manufactured October 26, 2010, to June 22, 2015, 2016 CX-3 vehicles manufactured June 1, 2015, to December 26, 2015, and 2013-2016 CX-5 vehicles manufactured December 15, 2011, to December 26, 2015. On the affected vehicles, the lift gate support struts may corrode, possibly causing the struts to break and/or the lift gate to drop unexpectedly.
Rimedio: Mazda dealers will replace the both lift gate supports, free of charge, however parts are not currently available. The recall began April 2017. Owners may contact Mazda customer service at 1-800-222-5500. Mazda's number for this recall is 9916H.
Mazda North American Operations (Mazda) is recalling certain model year 2016 CX-3 vehicles manufactured June 1, 2015, to December 2, 2015, 2013-2014 Mazda2 vehicles manufactured July 3, 2013, to August 8, 2014, 2014-2016 CX-5 vehicles manufactured January 19, 2013, to December 2, 2015, and 2013-2015 CX-9 vehicles manufactured December 6, 2012, to July 31, 2015. The lower mounting nuts and bolts that attach the front strut assembly to the steering knuckle may loosen, allowing the steering knuckle to separate.
Rimedio: Mazda will notify owners, and dealers will inspect the lower mounting nuts and bolts that secure the front strut assembly to the steering knuckle. If the bolts are loose, they will be replaced and then correctly tightened, free of charge. The recall is expected to begin May 20, 2016. Owners may contact Mazda customer service at 1-800-222-5500. Mazda's number for this recall is 9316D.
Mazda North American Operations (Mazda) is recalling certain model year 2014-2016 Mazda CX-5 vehicles manufactured June 27, 2013, to February 2, 2016. The affected vehicles have a fuel filler pipe that may rupture in the event of a crash. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 301, "Fuel System Integrity."
Rimedio: Mazda will notify owners, and dealers will alter the attachment between the fuel filler pipe and the left rear side of the bracket of the fuel filler pipe, free of charge. The recall began on February 25, 2016. Owners may contact Mazda customer service at 1-800-222-5500, option 6. Mazda's number for this recall is 9216B.
Pilkington North America, Inc. (Pilkington) is recalling certain replacement windshields, model FW04026 GTY, manufactured November 23, 2015, to December 16, 2015 for use in Mazda CX-5 vehicles that are equipped with the Smart City Brake Support system. The affected windshields have a bracket mounted on the inside to hold a laser sensor or LIDAR device for the Smart City Brake Support system. These brackets were improperly affixed with only tape instead of tape and urethane and therefore may loosen.
Rimedio: Pilkington will contact the wholesalers and retailers that purchased the windshields to try to identify the affected customers. The affected windshields will be replaced, free of charge. This recall began on February 15, 2016. Owners may contact Pilkington at 1-844-278-7230.
