The contact owns a 2024 Honda HR-V. The contact stated while her daughter was driving 30 MPH, the driver attempted to switch lanes to prevent another vehicle from crashing into her vehicle. The contact stated that the steering wheel seized inadvertently, causing the vehicle to drive off the road. The vehicle hit the sidewalk and drove into the ditch, where the vehicle hit a small hill. The air bags did not deploy. The driver hit her head against the steering wheel and sustained neck and head pain. The contact stated that the front driver's side seat belt failed to retract and restrain her daughter; however, the front passenger's side seat belt retracted even though the seat was unoccupied. Additionally, the rear driver's side and passenger's side seat belts had extended abnormally. The vehicle automatically dialed 911, and police officers arrived at the scene. A police report was filed. The contact's daughter was able to exit the vehicle and did not seek medical assistance. The vehicle was towed to Tipton Motor Sales, LLC(3840 North Expy, Brownsville, TX 78526), where the front bumper, all the seat belts, the front driver's side tire, and unknown suspension parts were replaced due to the damage the vehicle had sustained during the incident. The contact became aware that the VIN was included in NHTSA Campaign Number: 24V744000(Steering), which the contact related to the failure; however, the part to do the recall repair was not yet available. The local dealer was contacted. The vehicle was not diagnosed or repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was notified of the failure and informed the contact that the insurance company would need to hire an investigator to determine the cause of the accident because the local dealer was not capable of doing so. The failure mileage was approximately 4,441.

When we came back, we started seeing smoke coming out from a front hood, there were also flame on the right front side of the car. I immediately called 911 to describe the situation. I own a 2024 Honda HR-V. Since I purchased the vehicle, I have experienced recurring warning messages stating: "Collision Mitigation Braking System Problem - See Your Dealer" and "Road Departure Mitigation System Problem - See Your Dealer." I brought the vehicle to the Honda dealer and reported these problems. The dealer scanned the vehicle and told me that no fault codes were found. Despite this, the problem has continued. On one occasion, while driving at approximately 5 mph, the vehicle unexpectedly applied the brakes on its own. There was no vehicle, pedestrian, cone, rock, debris, or other obstacle in front of the vehicle. I did not intentionally apply the brakes to cause the sudden stop. I am concerned that the Collision Mitigation Braking System or another Honda Sensing safety system may be incorrectly detecting an obstacle and applying the brake hen there is nothing in front of the vehicle. This is a safety concern, it could save lives and stop accidents from happening if this safety issue gets investigated and find a solution.

*The vehicle exhibited a profound and worrisome vibration under the driver's seat ( I was also told by a Honda Serviceman in the passenger seat during the pre-service test drive, that he felt it, also), when accelerating at speeds of 53 mph and above on the highway. This systematic issue was replicated by the dealership across multiple model years (2023 and 2026 HR-V's), suggesting a fundamental mismatch between the heavy SUV body and the lighter Civic chassis platform. The vehicle is NOT available for inspection upon request because I was forced to trade it in for a competitor's vehicle due to structural concerns. *The safety risk was two-fold: structural unpredictability and driver distraction/anxiety. The profound vibration directly beneath the driver's seat felt fundamentally unsafe and unstable at highway speeds, creating a constant fear of a catastrophic mechanical or structural failure while driving at 53 mph+ speeds. Dealing with this profound, undiagnosable vibration caused considerable psychological stress, heartache and anxiety every time I entered the freeway, severely impacting my peace of mind and distraction levels while attempting to navigate high-speed traffic. *Yes, the issue was brought to a Honda dealership's Service Department, where they dismissed the shaking by stating "that is just how the HR-V drives." To confirm the systematic nature of the defect, comparative freeway test drives were conducted with a member of the Service Department and a salesman, using both a separate 2023 HR-V and a brand new 2026 HR-V on the exact same route. Both vehicles exhibited the identical, unsettling vibration at 53 mph, confirming it is a widespread engineering defect across the vehicle line. *Yes. My independent mechanic of 20 years did a full inspection but incorrectly blamed "the road". This delayed my dealer visit, where staff also refused to inspect it, saying "that's" just how the HR-V drives". Both facilities failed to address the risk. *No.

Rear liftgate window spontaneously cracked no signs of damage to the rear of vehicle rear glass was mostly intact but had tempered glass fracturing throughout while car was parked. In terms of safety, glass breaking spontaneously can cause damage to eyes/ect. Police report filed, Dealership denies that it is an issue for the 2024 HRV model only 2023. No warnings, or symptoms just the rear window cracking.

Lights that went off were : collision mitigation braking system, road departure mitigation system, auto high beam, adaptive cruise control, some driver assist systems cannot operate (clear front windshield or poor viewing distraction) - all at the same time, even when it's no visible windshield issues. My wheels will not stay aligned. I’ve had all the property services done to the car, and extra, by a Honda Dealer. I have 8 repair records to alignment on a new car, in under 18 months. Under my state of Virginia laws, only three safety fixes on a car authorize the car as a ‘dud’ and should be a buy back. Honda Corp has in their possession all documents and all notices needed under the law, since November 2025. Honda Corp is not responding. Hello, who holds them to the law? Who is above Honda? It’s been since months since I notified Honda and they won’t follow the law. Who overseas Honda? Hello?

As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard numbers 208, "Occupant Crash Protection," 209, "Seat Belt Assembles," and 210, "Seat Belt Assembly Anchorages."
