TL* THE CONTACT OWNED A 2015 TOYOTA RAV4. WHILE DRIVING AT AN UNKNOWN SPEED, THE CONTACT NOTICED THAT ANOTHER VEHICLE WAS APPROACHING HEAD ON TOWARDS HIM. THE CONTACT ATTEMPTED TO AVOID A CRASH AND MOVED OVER TO THE OTHER SIDE OF THE HIGHWAY; HOWEVER, THE OTHER VEHICLE DID THE SAME THING. THE OTHER VEHICLE STRUCK THE CONTACT ON THE PASSENGER SIDE OF THE VEHICLE. A POLICE REPORT WAS FILED. THE AMBULANCE WAS CALLED AND FOUR OUT OF THE FIVE OCCUPANTS WERE TRANSPORTED TO THE HOSPITAL. A TEN YEAR OLD OCCUPANT WAS AIR LIFTED TO THE HOSPITAL AND LATER DIED. THE VEHICLE WAS DESTROYED AND TOWED FROM THE SCENE. PRIOR TO THE CRASH, THE CONTACT WAS ALERTED THAT THE REAR PASSENGER SIDE SEAT BELT WAS DEFECTIVE. THE CONTACT TOOK THE VEHICLE TO THE DEALER (NORTH HOLLYWOOD TOYOTA, 4606 LANKERSHIM BLVD, NORTH HOLLYWOOD, CA 91602) FOR REGULAR MAINTENANCE. THE TECHNICIAN INFORMED THE CONTACT THAT THE SEAT BELT WAS DEFECTIVE AND AN APPOINTMENT WAS SCHEDULED FOR FEBRUARY 17, 2018. THE CRASH OCCURRED THE DAY BEFORE THE APPOINTMENT. THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE. THE FAILURE MILEAGE WAS 45,000.

THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE FAILURE MILEAGE WAS 20,000.
BRAKES DID NOT RESPOND WHILE DRIVING AND AN ACCIDENT OCCURRED.
MYSELF, MY DAUGHTER AND MY DAUGHTER'S FATHER WERE TRAVELING SOUTHBOUND ON INTERSTATE 89 TOWARDS BURLINGTON, VT. WE HEARD AN EXTREMELY LOUD BANG. IT SOUNDED LIKE AN EXPLOSION OR A GUN SHOT. THE SUNROOF HAD EXPLODED, NOT FROM ANY DEBRIS OR ROCK'JUST EXPLODED. IT WAS A TERRIFYING EXPERIENCE. LUCKILY WE ONLY GOT MINOR CUTS FROM THE GLASS. THIS HAPPENED ON A BUSY HIGHWAY. THANKFULLY I MAINTAINED CONTROL OF THE CAR DESPITE THE EXPLOSION AND PULLED TO THE SIDE OF THE HIGHWAY. WE COULD HAVE BEEN KILLED OR KILLED SOMEONE ELSE IN ANOTHER CAR. NOW I AM FINDING OUT HOW OFTEN THIS TYPE OF THING HAPPENS AND WHY IS NOTHING BEING DONE ABOUT THIS. SUNROOFS NEED TO BE RECALLED. IT IS HORRIFYING THAT THIS HAS NOT BEEN ADDRESSED. IN ALL HONESTY, THIS PROBABLY WON'T CHANGE UNTIL SOMEONE DIES AS A RESULT OF THIS.
The recall for my RAV4 was for the battery, and it may cause a fire. The safety of my family members and other families was affected by this fire. We are all displaced. My mom parked our car under our apartment, and 15 minutes later, the vehicle was engulfed in flames. The battery was replaced in 2019, and the rep told us of farmers that they can't confirm that the recall may be the root of the fire. I was told by the farmers rep that a certain battery is responsible for the fire. The car was parked and no lights were on.
The contact owns a 2015 Toyota Rav4. The contact stated that the mother was driving at a slow rate of speed while in a parking lot and when the brake pedal was pressed the vehicle suddenly experienced unintended acceleration causing the driver to lose control of the vehicle and crash into a tree. During the incident, the driver sustained a right leg and right arm injury and was transported to the hospital and treated for multiple fractures. A police report was taken at the scene and the vehicle was towed away. The cause of the failure was not yet determined. The local dealer was notified of the failure and referred the contact to the manufacturer. The manufacturer was not yet contacted. The failure mileage was 100,000.
On November 4 2022 I was involved in a car accident The opposing driver hit me nearly head on but to the right on the drivers side. Crushed the left front tire and axle The car is totaled My air bags did not go off. Front side or otherwise. My life was put in serious danger, I am lucky to be alive; suffering chest back and knee pains due to the strong impact. I had the officer note and hospital note that the car bags did not go off. The opposing insurance made note of the air bags not going off. Unknown if there were warning lights. I am the only driver of that car and original owner. I am in Puerto Rico now and the local dealer in San Juan has not returned phone call.

The manufacturer was notified of the failure and informed him that they would investigate the failure. The vehicle had yet to be repaired. The failure mileage was approximately 79,000.
I WAS STOPPED AT A LIGHT. IT WAS MY TURN TO GO, I PROCEEDED TO GO STRAIGHT WHEN ANOTHER CAR RAN THE STOP LIGHT AND I WAS T-BONED ON THE DRIVERS SIDE WHEN A DRIVER RAN A LIGHT. THE CAR WAS TOTALED. THE NONE OF THE AIR BAGS DEPLOYED. I WOULD HAVE EXPECTED THE SIDE AIRBAG TO DEPLOY IF THE HIS WAS BAD ENOUGH TO TOTAL THE CAR. I WAS TOLD BY THE DEALER THE AIR BAGS KNOW WHEN IT IS A LIFE THREATENING SITUATION.
HEAD ON COLLISION AT 48MPH AFTER BEING SIDESWIPED. NEITHER FRONT OR SIDE AIRBAGS DEPLOYED.THIS INCIDENT OCCURRED ON A 4 LANE HIGHWAY.
VEHICLE WAS MAKING A LEFT TURN, WHEN ANOTHER VEHICLE WENT THROUGH A 'RED' LIGHT AND HIT IT ON THE RIGHT FENDER. FENDER CAME OFF. OTHER VEHICLE HAD FRONT SMASHED IN AND AIRBAGS DEPLOYED. THE AIR BAGS DID NOT DEPLOY IN THE TOYOTA. DRIVER WAS WEARING SEATBELT, BUT LACK OF AIRBAG CAUSED DRIVERS ARM AND CHEST TO HIT STEERING WHEEL AND LEGS TO HIT OTHER COMPONENTS.

VEHICLE DID NOT RESPOND TO BRAKING (POSSIBLE STALLING, LOSS OF CONTROL OF VEHICLE), RESULTING IN A SEVERE CRASH INVOLVING TWO PARKED CARS AND PROPERTY, INJURY TO DRIVER, AND TOTAL LOSS. THE VEHICLE ACCELERATED AT A HIGH RATE OF SPEED GOING DOWN HILL DUE TO HAVING NO CONTROL OVER THE VEHICLE. BECAUSE DRIVER WAS UNABLE TO STOP, THE VEHICLE RAN TWO STOP SIGNS. THE CRASH RESULTED IN THE VEHICLE CRASHING INTO TWO PARKED CARS, "JUMPING" THOSE VEHICLES AND RUNNING INTO A HOME. THE VEHICLE WAS TWICE PREVIOUSLY REPAIRED UNDER WARRANTY FOR STALLING. OCCASION 1 (14K MILES): VEHICLE STALLED AT STOPLIGHT AND KEY LIGHT WAS FLASHING. ERROR CODES P1605 AND P219A AT THE TIME. OCCASION 2 (16K MILES): VEHICLE STALLED TWICE WHILE STOPPED, ON THE SAME DAY. THE VEHICLE WAS SHAKY AT THE STOPLIGHT AND MULTIPLE WARNING LIGHTS CAME ON, CODE P1605 (2ND REPAIR). *TR
TAKATA RECALL. I WOULD LIKE MORE INVESTIGATION REGARDING WHEN A CAR IS REPAIR PER SPECIFICATION AND STATE LAW. HOW IS THAT A COMPANY TAKE CONTROL OF THE VALUE AND HISTORY OF THE CAR. BUT IS NOT REPORTED THE CORRECT WAY WHEN THE REPAIR IS DONE. AFFECTING THE OWNER OF THE VEHICLE.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2013-2018 RAV4 vehicles. Certain replacement 12-volt batteries may not fit properly in the battery tray, allowing the battery to move and contact the hold-down bracket, possibly causing a short circuit.
Solution : Dealers will replace the battery clamp sub-assembly, battery tray, and positive terminal cover, free of charge. Owner letters were mailed December 27, 2024, to 2013-2014 RAV4 owners. Additional owner notification letters will be mailed in phases, phase 2 will begin on May 14, 2025, and phase 3 in late July 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 23TB13 and 23TA13.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2015 Camry, Camry Hybrid, Highlander, and Highlander Hybrid, and 2014-2015 Rav4 vehicles. A component of the electric power steering (EPS) electronic control unit (ECU) may have been damaged during the manufacturing process. Over time, this damage may result in failure of the electric power steering system.
Solution : Toyota will notify owners, and dealers will inspect the serial number of the EPS ECU or steering column assembly. If the number is within the affected range, the EPS ECU will be replaced, free of charge. The recall began on May 7, 2015. Owners may contact Toyota customer service at 1-800-331-4331.
Southeast Toyota Distributors, LLC (SET) is recalling certain model year 2013-2015 Toyota Rav4 vehicles manufactured June 1, 2013, to December 29, 2014, and equipped with an accessory trailer light module. The software within the module may incorrectly detect an electrical short and preventively turn off the electrical current and the trailer lights.
Solution : SET will notify owners, and dealers will replace the trailer light module with a new unit with corrected software, free of charge. The recall began on March 17, 2015. Owners may contact SET customer service at 1-866-405-4226. SET's number for this recall is SET-15A.
Southeast Toyota Distributors, LLC (SET) is recalling certain model year 2014-2015 Toyota 4Runner, Tacoma, FJ Cruiser, Land Cruiser, RAV4, Scion FRS, and XB vehicles. The affected vehicles may have accessories installed by SET, such as running boards or other items, that were incorrectly installed. The accessory attaching fasteners were not tightened with the proper torque, possibly causing the accessory to detach from the vehicle.
Solution : SET will notify owners, and dealers will inspect and tighten the affected bolts, free of charge. The recall began February 23, 2015. Owners may contact SET customer service at 1-866-405-4226. SET's number for this recall is SET-14B.
