Hello, I was involved in a accident o March 11 2025 in [XXX] on [XXX] and [XXX] It happened around 2pm cst. There was substantial damage to my vehicle, and my Air Bags did Not Deploy. There are no recalls for this on the vehicle, but Toyota has sent recalls out on other 2022 and 2023 Toyota Corolla Cross's, but not ours. The vehicle was towed by Anderson Towing and Collision. I reported this to my insurance company Progressive. I was rear-ended and the Vehicle Alert displayed airbag malfunction. This was a car rental I reported to insurance company. It advised to inspect car immediately.
I was involved in a frontal collision in which I rear-ended another vehicle. The impact was significant enough to cause a fractured sternum due to the seat belt restraint. However, the front airbags did not deploy. I had a passenger in the vehicle, and we were both wearing our seatbelts at the time of the crash. I am concerned that the airbags failed to deploy despite the severity of the impact and the resulting injury. I believe this may indicate a defect in the airbag system or the airbag control unit (ACU). I am submitting this report to ensure the issue is investigated and to help prevent potential harm to other drivers. After the collision the car was not drivable due to an airbag error message on the dash of the vehicle. It had to be towed away from the scene and will not be repaired. The phone charger stopped working. During driving, the dashboard lights up and then break failed and steering became out of control. As a result, car crushed to another car and went to different lane and hit another car. All airbags deployed. Car is total loss. Police was at the scene. All drivers have minor injuries. Reported the issue to the insurance.

I experienced 4 near-death experiences by this car. 1. 10/14/23 Car caught on fire. Car was parked and turned off. Everything inside was shut off. When I stepped out the car, the engine was still running but it sounded loud like a lawnmower. I went back into the car and turned it back on. Several lights and system malfunction messages displayed on dashboard. I turned off the car and everything inside shut off. When I stepped out the car, the engine was still on, sounded like a lawn mower, and caught on fire. Estimator from my insurance couldn't identify the issue and had car taken to dealership services. I contacted Toyota corporate who claimed they would send an investigator, but he never came. Dealership replaced engine and engine harnesses that burned, and discharged car to me on 12/05/23. 2. 01/09/24 while driving on the highway at 65 mph, my engine shut off and steering wheel locked. I had a difficult time getting into the left shoulder to safety. Several system malfunction msgs displayed on dashboard. Car was towed to dealership services that evening. I had an appt for 8:15AM the next day. Servicer from Toyota dealership called me at 8:34 AM and told me to come pick up my car because they couldn't find anything wrong with it. I refused and explained the dangerous situation. Master mechanic ran more tests and couldn't find the issue so discharged the car back as is on 1/13/24. 3. 2/5/24, driving on highway at 65 mph, engine shuts off and steering wheel locked. I had a difficult time getting into the right shoulder to safety again. Several system malfunctions displayed on dashboard. Car was towed to dealership services. Servicer from Toyota calls again and tells me to pick up vehicle. They said they didn't find any issues with car. I begged them to run more tests. 2/14/24 They found "starter" part to be defected. Starter replaced and car discharged on 2/27/24. 4. 2/28/24 Driving on highway again, system malfunction msgs displayed. Car now in dealership. 4.

The vehicle's "Engine Maintenance Required" dashboard message illuminated due to a total failure of the Engine Coolant Bypass Valve, throwing Diagnostic Trouble Code (DTC) P268115. A certified Toyota dealership diagnostic inspection confirmed that an internal seal leak allowed pressurized engine coolant to short out the valve's electrical circuit, rendering the component completely inoperable. This component is responsible for regulating engine temperature and routing coolant to prevent the engine from overheating. Because the valve has suffered an internal electrical short, it cannot redirect coolant appropriately, introducing a severe risk of sudden engine overheating and catastrophic power loss while driving at highway speeds. Furthermore, Toyota has officially acknowledged this exact premature part failure on identical engine platforms under Customer Support Program 24TE04, but has excluded the Corolla Cross model. Car shuts off randomly saying low steering power, will restart after a few minutes. This a safety issue that can lead to a crash.
I received the engine maintenance required notification on 7/11. On 7/13, I called my Toyota to make an appt to bring it in at 9 AM. The ran diagnostics for $180 and then the test results said the engine coolant valve is stuck open or closed but either is dangerous because my car could overheat. This is a $850 repair. It sounds like this same part was used on another Toyota suv and the warranty was extended for those so this part needs to be recalled or warranty extended for the 2022 Toyota Corolla Cross as well. I was waiting for this repair to be done for 5 hours total.
While driving at highway speeds 65mph in HOV lane (wall was to driver side), the back driver side window exploded. There was no external objects on the road that could explain the damage. No scratches or other body damage to suggest something was kicked up or impacted it. The window spontaneously exploded sending shards of glass inside the back of the vehicle. Upon inspection of the back inside of the vehicle there is no foreign objects to explain the explosion. The windows were not down or cracked, temperature outside was 83F with no winds or other weather. This is a hazard especially to back passengers and god forbid drivers. Please take this account to make Toyota fix this issue, it’s unsafe.
VEHICLE: 2022 Toyota Corolla Cross L FWD MILEAGE: 40,000 miles DATE OF FAILURE: [Insert date] DEFECT DESCRIPTION: My vehicle's coolant bypass valve (part #16671-F2010) has failed, causing coolant leaks and triggering error codes. An independent shop diagnosed the issue as a defective coolant bypass valve before I took it to the Toyota dealership for confirmation. SAFETY CONCERN: This is a known defect affecting multiple Toyota models from 2019-2023. Coolant bypass valve failures can lead to engine overheating, potential engine damage, and loss of vehicle control if the engine fails while driving. This creates a serious safety hazard. MANUFACTURER RESPONSE: Toyota issued Customer Support Program 24TE04 covering this exact defect for 2020-2021 Corollas and certain RAV4 models. However, they arbitrarily excluded 2022-2023 Corolla Cross vehicles despite using THE IDENTICAL DEFECTIVE PART (F0010 coolant bypass valve) in all gas models through early 2024. Toyota dealership charged me $99 for a diagnostic to confirm what was already known. The repair quote was nearly $1,000. Toyota Corporate denied goodwill assistance, stating they only provide it to "loyal customers" who have spent significant money on servicing or own multiple Toyotas. CONSUMER IMPACT: This is the same defect that led to the class action lawsuit Barrientos v. Toyota. Toyota settled privately and created CSP 24TE04, but deliberately excluded newer model years with the same defective part. Consumers with 2022-2023 vehicles are forced to pay $900-1,300 out-of-pocket for a manufacturing defect that Toyota has already acknowledged in other model years. REQUESTED ACTION: I am requesting NHTSA investigate why Toyota's Customer Support Program excludes 2022-2023 model years when they contain the identical defective coolant bypass valve. Toyota should be required to extend CSP 24TE04 to cover ALL vehicles with part #16671-F2010 or F0010, regardless of model year.
Bracket for Right rear differential cracked and broke in two separate places. causing a severe banging noise when turning right or left and or going from backing up to forward motion. have video and Toyota report . never went over any pot holes and was in driveway when noise started backing up. vehicle currently at freehold Toyota dealership service dept. component item was seen by service mngr, service tech and several others stating they never saw anything like this before. the whole wheel could have broken off while driving causing death or severe injury to my family. it broke in two separate places on either side of bolts holding rest together . Have Toyota Premier vsa Protection plan-Toyota states not part of protection??This could have killed us on the highway if the whole rear wheel fell off and we flipped over.

The instrument panel may have been produced without the required perforation, which could result in the front-passenger air bag not deploying properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."
Solution : Dealers will inspect and replace the instrument panel as necessary, free of charge. Owner notification letters were mailed January 26, 2024. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 23TA16. Vehicles previously inspected under 23V-384 will need to return to the dealership for additional inspection.
