I am filing a formal safety complaint regarding a severe electrical failure and fire hazard inside the fuel tank of my 2015 Nissan NV200, directly related to Safety Recall PMA56 (NHTSA 25V676000). The dealership performed the recall service but refused to replace the fuel pump module, claiming no diagnostic trouble codes (DTCs) were present. Experiencing ongoing symptoms, I purchased an OEM replacement fuel pump and extracted the original module myself. Upon disassembly, I discovered: 1. Severe Electrical Arcing/Thermal Damage: The fuel level sender ceramic card, contacts, and internal harness connectors were completely charred, carbonized, and melted due to an active short circuit/extreme thermal event inside the module. 2. Debris Contamination: The internal short circuit melted the module's plastic housing, dropping burnt debris directly into the reservoir and clogging the fuel strainer. Running an active short circuit with exposed arcing contacts inside a volatile environment saturated with fuel vapors creates an imminent risk of tank explosion or vehicle fire. The dealership's protocol of only inspecting external wires or relying on DTCs is inadequate. I urge the NHTSA to audit Nissan's remedy procedures for Campaign 25V676000, as the current inspection protocol fails to detect severe internal module destruction

The contact owns a 2015 Nissan NV200. The contact stated that the climate control mode failed to defrost the windows. Approximately 3 years after the first failure, the vehicle was repaired; however, the failure reoccurred. The vehicle was taken to the dealer at the first failure, and it was determined that the heater box had failed. The heater box and climate control bracket were replaced. The vehicle had not been taken to the dealer for a second diagnostic test or repair. The manufacturer was made aware of the failure and a case was opened. The approximate failure mileage was 10,000.

The vehicle safety switch is bad and Nissan dealers will not sell the part unless you buy the entire transmission. I bought this transmission in Nov 2020 from Nissan Dealer and the part is malfunction allowing it to start in all gears. The warranty on the transmission is 12,000 miles or 12 months. At this point, I am forced to drive this vehicle which can make it hazardous. Nissan is insistent that will not sell the part unless I buy a whole new transmission. How can that be?

THE FIRST TIME THIS HAPPENED, WE NEARLY HIT THE CAR IN FRONT OF US BECAUSE WE HAD TO BRAKE QUICKLY (ORIGINAL COMPLAINT STATED PEDAL WENT TO THE FLOOR -- I THINK THIS WAS IN ERROR DUE TO EXCITEMENT OF THE MOMENT). THIS BRAKE MALFUNCTION HAS HAPPENED TWICE IN THE PAST 2 MONTHS. DEALER CAN'T DUPLICATE AND SO FAR NISSAN CONSUMER AFFAIRS SAYS THEIR PROCESS DOESN'T ALLOW THEM TO DO ANYTHING UNLESS THE DEALERSHIP CAN DUPLICATE THE PROBLEM. WHILE DRIVING IN TRAFFIC ON A DRY, STRAIGHT ROAD AT ABOUT 35MPH, TRAFFIC STARTED TO SLOW FOR A RED LIGHT. THE BRAKE PEDAL WENT TO THE FLOOR WITH NO RESISTANCE AND HAD NO EFFECT ON VEHICLE SPEED. FRANTICALLY PUMPING THE BRAKE PEDAL GRADUALLY INCREASED BRAKE PEDAL RESISTANCE. AFTER GAINING BRAKE PEDAL RESISTANCE, THE VEHICLE CAME TO A SCREECHING HALT, ACTIVATING ABS AND NARROWLY AVERTING AN ACCIDENT. THIS HAS ONLY OCCURRED ONCE, ON 19 DEC 2018 AND, TO DATE, HAS NOT REOCCURRED. WE TOOK THE VEHICLE TO THE DEALERSHIP AND THEY WERE NOT ABLE TO REPRODUCE THE PROBLEM AND SENT US AWAY WITH "NO PROBLEM FOUND AT THIS TIME". ANECDOTALLY, THE DEALERSHIP DID MENTION ANOTHER FAMILY THAT REPORTED THE SAME PROBLEM AND THEY DECLARED IT A FLUKE BECAUSE IT HAS NOT REOCCURRED. THE PROBLEM HERE IS THAT IF IT DOES REOCCUR, IT'S QUITE POSSIBLE NEITHER MYSELF NOR MY FAMILY WILL BE AROUND TO REPORT IT. CONTINUING TO DRIVE WITH A STRATEGY OF HOPE THAT THIS DOES NOT REOCCUR SEEMS A POOR STRATEGY CONSIDERING THE RISK TO MY FAMILY AND OTHERS ON THE ROAD. Nissan North America, Inc. (Nissan) is recalling certain 2013-2021 NV200 Van, 2014-2017, 2019 NV200 Taxi, and 2015-2018 Chevrolet City Express vehicles. The Fuel Tank Temperature (FTT) sensor harness may have been incorrectly routed, damaging wires and causing a short circuit in the fuel pump fuse.
Solution : Dealers will inspect and reroute the FTT sensor harness or replace the fuel pump assembly as necessary, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Nissan's customer service at 800-647-7261. Nissan's number for this recall is PMA56. Vehicle Identification Numbers (VINs) involved in this recall became searchable on NHTSA.gov October 10, 2025.

AIR BAGS:SENSOR:OCCUPANT CLASSIFICATION
Nissan North America, Inc. (Nissan) is recalling certain model year 2016-2017 Nissan Maxima, 2013-2016 Nissan Altima, NV200, LEAF, Sentra, and Pathfinder, 2014-2016 Nissan NV200 Taxi, Infiniti QX60, QX60 Hybrid, and Q50 Hybrid, 2014-2017 Nissan Rogue and Infiniti Q50, 2015-2016 Nissan Murano, Murano Hybrid, and Chevrolet City Express, 2014-2015 Nissan Pathfinder Hybrid, and 2013 Infiniti JX35 vehicles. In these vehicles, the front seat passenger Occupant Classification System (OCS) may incorrectly classify an adult passenger as a child or classify the seat as empty despite it being occupied. As a result, the passenger frontal air bag may be turned off and not deploy in the event of a crash.
Solution : Nissan will notify their owners. Chevrolet City Express owners will be notified by General Motors. Dealers will reprogram the Air Bag Control Unit (ACU) and OCS Electronic Control Unit (ECU) in Altima, Maxima, Murano, Rogue, and Sentra vehicles, and replace the OCS ECU in LEAF, NV200, NV200 Taxi, Pathfinder, Infiniti Q50, JX35, and QX60 and Chevrolet City Express vehicles, free of charge. Interim notices were sent to owners on May 31, 2016. Owners will receive a second notice when remedy parts become available. Owners may contact Nissan customer service at 1-800-867-7669, Infiniti customer service at 1-888-833-3216 or Chevrolet customer service at 1-800-222-1020.
