There was a front-end collision into a stone wall. It appears the driver seatbelt failed and ripped upon impact. The driver died as a result of injuries sustained. There was a front passenger in the vehicle as well who was belted. The seatbelt for the front passenger worked correctly. That passenger had serious injuries but survived the collision. The contact stated that the air bags failed to deploy upon impact or during the crash. The vehicle was not drivable. The contact and his wife were transported to the hospital and were both treated for whiplash, body pain, and pain to the head and neck. The contact also had a throbbing headache from the crash. The contact and his wife both received CT scans that showed no signs of injuries. The other driver also spun out of control and struck the metal guardrail located in the middle of the highway. The contact stated that the other driver had not received medical attention at the scene. A Police Report was filed. The vehicle was towed to a collision yard where it remained. The insurance company had not inspected the vehicle; however, the contact expected that the vehicle would be deemed a total loss. The contact stated that the driver who caused the accident was cited by the Police for causing the crash. Neither the dealer nor the manufacturer was notified of the failure. The failure mileage was approximately 71,400.

2019 Subaru Forester was stopped with left directional signal on to make a left turn. Waiting for oncoming traffic to pass. Vehicle was hit multiple times from the front, back AND side. Airbags did NOT deploy. Driver of the vehicle which hit us said he was “distracted” when he hit us and that it was his fault.

TL* THE CONTACT OWNED A 2019 SUBARU FORESTER. THE CONTACT STATED THAT ALL THE AUTOMATIC SAFETY FEATURES FAILED ON THE VEHICLE. WHILE DRIVING BETWEEN 50-55 MPH AND CHANGING LANES, THE LANE DEPARTURE WARNING FAILED. AS A RESULT, THE CONTACT REAR ENDED ANOTHER VEHICLE. THE AIR BAGS FAILED TO DEPLOY. THE CONTACT SUSTAINED A FRACTURED STERNUM THAT REQUIRED MEDICAL ATTENTION. THE CONTACT WAS ABLE TO EXIT THE VEHICLE BEFORE IT CAUGHT FIRE AND BURNED TO THE GROUND. THE FIRE DEPARTMENT EXTINGUISHED THE FIRE. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO A TOW YARD AND DEEMED DESTROYED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND DID NOT ASSIST. THE FAILURE MILEAGE WAS APPROXIMATELY 14,000. THE VIN WAS UNKNOWN.
On 9/2/2016, my 2019 Subaru Forester was involved in a right-front collision. During the collision, the driver's frontal airbag did not deploy. Actually none of the airbags deployed. As a result, the driver's chest and upper abdomen struck the steering wheel.And sustained significant bruising and other injuries and required evaluation and treatment at an emergency department. The vehicle has substantial damage and may be considered a total loss. I am reporting this because the driver's frontal airbag did not deploy during an injury-producing collision. Actually no airbags deployed. I would like NHTSA to document the incident and determine whether the airbag/SRS system may have malfunctioned or whether there may be a safety defect affecting this vehicle or other vehicles of the same type. I can provide photographs of the vehicle and damage, medical documentation, the crash/police report if available, the vehicle identification number (VIN), mileage, and additional information about the collision if needed. I am not making a determination about why the airbag failed to deploy; I am requesting that NHTSA investigate the circumstances and determine whether a safety defect may be involved.

The insurance company towed the vehicle to a tow lot, where it was totaled. The dealer was notified of the failure. The manufacturer was notified of the failure and sent a field engineer who retrieved the EDR from the vehicle. The manufacturer later informed the contact that the information retrieved from the EDR indicated that there was no vehicle malfunction and that the air bags had not deployed because the impact was not significant. The failure mileage was approximately 34,000.

WHEN THE CONTACT PUSHED THE SOS BUTTON INSIDE HER VEHICLE, SHE SPOKE WITH STARLINK AND WAS INFORMED THAT SHE WOULD BE CONNECTED TO 911. INSTEAD, THE "YOUR CALL CANNOT BE COMPLETED AS DIALED, PLEASE TRY YOUR CALL AGAIN LATER" MESSAGE WAS HEARD. ALSO, THE STARLINK APP INDICATED THAT THE VEHICLE WAS FUNCTIONING AT 100%; HOWEVER, THE BLIND SIDE DETECTION HAD DETACHED FROM THE VEHICLE DUE TO THE CRASH. THE FOLLOWING DAY, THE CONTACT CALLED SUBARU STARLINK AND WAS INFORMED THEY WOULD LOOK INTO THE FAILURES. THE CONTACT HAD NOT HEARD BACK AND THEN CALLED SUBARU AMERICA. THE MANUFACTURER STATED THAT THEY WOULD OPEN AN IT TICKET. THE CONTACT FOLLOWED-UP WITH THE MANUFACTURER AND FOUND THAT NOTHING HAD BEEN DONE AND A TICKET WAS NOT OPENED. THE CONTACT INJURED HER BACK AND SUSTAINED A SPRAINED ANKLE AND WHIPLASH. IMMEDIATE MEDICAL ATTENTION WAS NOT RECEIVED. A POLICE REPORT WAS FILED. THE VEHICLE WAS DRIVEN TO NORTH READING SUBARU (978-386-6090, LOCATED AT 260 MAIN ST, NORTH READING, MA 01864) AND THEN TAKEN TO A BODY SHOP. THE ENTIRE REAR END OF THE VEHICLE HAD TO BE REPLACED. THE CAUSE OF THE FAILURE WAS NOT DETERMINED. THE FAILURE MILEAGE WAS 21,419.

I WAS DRIVING MY 2019 SUBARU FORESTER ON THE INTERSTATE WHEN A CAR STOPPED IN FRONT OF ME. I ATTEMPTED TO BRAKE, BUT DID END UP REAR ENDING THE CAR IN FRONT OF ME, WHICH PUSHED HER INTO THE CAR IN FRONT OF HER. THE IMPACT WAS AT LEAST 30-35 MPH AND THE CAR IS TOTALED. AIRBAGS FAILED TO DEPLOY.*JB

Subaru of America, Inc. (Subaru) is recalling certain 2018-2019 Subaru Crosstrek, 2019 Crosstrek Hybrid, and 2019 Forester vehicles. The rear stabilizer bracket bolts may loosen and detach.
Solución: Subaru will notify owners, and dealers will retorque and replace any missing bolts, free of charge. Owner notification letters were mailed May 17, 2021. Owners may contact Subaru customer service at 1-844-373-6614. Subaru's number for this recall is WRD-21.
Subaru of America, Inc. (Subaru) is recalling certain 2019 Crosstrek, Forester, and Ascent vehicles. The aluminum Positive Crankcase Ventilation (PCV) valve can separate, allowing the separated components to enter the engine, resulting in a loss of power while driving.
Solución: Subaru will notify owners, and dealers will inspect and replace the PCV valve, free of charge. If the PCV valve has separated and the components cannot be found, the short block engine will be replaced, free of charge. The recall began January 31, 2020. Owners may contact Subaru customer service at 1-844-373-6614. Subaru's number for this recall is WUW-08.
Subaru of America, Inc. (Subaru) is recalling certain 2019 Forester and Crosstrek vehicles. A connector inside the Electronic Power Steering (EPS) unit may short circuit resulting in a loss of electric power steering assistance.
Solución: None of the affected vehicles have been sold yet. Subaru has notified dealers and they will inspect the steering gearbox, replacing it as necessary, free of charge. The recall began February 7, 2019. Owners may contact Subaru customer service at 1-844-373-6614. Subaru's number for this recall is WUC-88.
