Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2010-2016 4Runner, 2003-2006 Tundra, 2003-2013 Corolla, 2009-2010 Corolla Matrix, 2004-2005 RAV4, 2002-2007 Sequoia, 2011-2013 Sienna, 2008-2012 Scion xB, 2008-2009 Lexus IS-F, 2007-2012 Yaris and Lexus ES350, 2010-2017 Lexus GX460, 2002-2010 Lexus SC430, 2006-2012 Lexus IS250 and IS350 and 2010-2015 Lexus IS250C and IS350C vehicles. These vehicles had their driver or passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.
Remedy: Toyota will notify owners. Depending on the vehicle model, dealers will replace the front driver inflator, front passenger airbag inflator, the airbag assembly, or the front passenger airbag sub-assembly and inflator. The recall began November 20, 2019. Owners may contact Toyota customer service at 1-888-270-9371 or Lexus customer service at 1-800-255-3987. Toyota's numbers for this recall are J0A, J0B, and J0C. Toyota is also recalling certain 2010 Scion xB, Toyota Corolla, Corolla Matrix, Yaris, 4Runner, Lexus IS250, IS350, IS250C, IS350C, ES350, IS-F, GX460, and Pontiac Vibe vehicles sold, ever registered, in the states of AZ, AR, DE, DC, IL, IN, KS, KY, MD, MO, NE, NV, NJ, NM, NC, OH, OK, PA, TN, VA, and WV or "Zone B." Toyota Motor Company (Toyota) is recalling certain model year 2009-2011 Tundra, Sequoia, Corolla, Corolla Matrix and Scion xB, 2008-2011 Highlander and Highlander Hybrid, 2007 Camry and Camry Hybrid, 2009 Camry and Camry Hybrid, 2006-2011 RAV4, 2006-2010 Yaris, and 2009-2010 Scion xD and Pontiac Vibe vehicles. During the manufacturing of the Power Window Master Switch (PWMS), grease lubricant may have been inconsistently applied to the sliding electrical contacts.
Remedy: Toyota will notify owners, and dealers will inspect the switch, applying a lubricant if no abnormality is found. If abnormality is found, the PWMS circuit board will be replaced, free of charge. The recall is expected to begin December 20, 2015. Owners may contact Toyota customer service at 1-800-331-4331. Toyota is recalling certain model year 2006-2010 Yaris vehicles manufactured August 22, 2005, through May 12, 2010, and 2008-2010 Scion xD vehicles manufactured April 4, 2007, through May 12, 2010. In the affected vehicles, the springs used for the seat rails on the driver's seat and the seat rails of the front passenger seat of three-door models may break.
Remedy: Toyota will notify owners, and dealers will inspect the seat rail, and replace it with a new one, if necessary, free of charge. The recall began on July 3, 2014. Owners may contact Toyota at 1-800-331-4331.
Toyota is recalling certain model year 2006-2008 RAV4, 2006-2010 Yaris, 2008-2010 Highlander, and 2009-2010 Tacoma, Corolla, Corolla Matrix and General Motors Pontiac Vibe vehicles. In the affected vehicles, the steering column assembly contains electrical connections to the driver's airbag module housed in a spiral cable assembly, which includes a Flexible Flat Cable (FFC). Due to the shape and location of the FFC's retainer, the FFC could become damaged when the steering wheel is turned.
Remedy: Toyota will notify the Toyota vehicle owners, and General Motors will notify the Pontiac Vibe owners. Toyota sent owners an interim notification letter in May 2014 to advise owners of the recall. Toyota will mail owners a second letter when remedy parts are available. General Motors sent Pontiac Vibe owners an Interim letter on June 20, 2014 notifying them that parts are currently unavailable. General Motors sent notices to the Pontiac Vibe owners on September 15, 2014, that a remedy is available. The recall began on October 27, 2014. Owners may contact Toyota at 1-800-331-4331. WAS GOING APPROX. 35 MPH WHEN MY FRONT DRIVERSIDE TIRE BLEW OUT, SENT CAR IN TO A RIGHT TURNING MOTION IN TO CEMENT DIVIDERS. AIRBAG DEPLOYED BUT DID NOT AIR UP. AIRBAG CAME OUT LIKE TUBE RIGHT INTO MY CHEST. WAS TAKEN TO HOSPITAL FROM ACCIDENT. WHERE THEY CONCLUDED THAT I HAD 3 BROKEN RIBS.

I WAS DRIVING AT THE REGULAR SPEED LIMIT AND WHEN THE CAR IN FRONT OF ME WAS STOPPING TO MAKE A LEFT HAND TURN. I STEPPED ON MY BRAKE AND THE CAR WOULD NOT STOP. I WAS PRACTICALLY STANDING ON THE BRAKE PEDAL AND THE CAR WOULD NOT STOP AND I HIT THE CAR IN FRONT OF ME. THE AIRBAGS DEPLOYED. I REMEMBER THAT A POLICEMAN TOLD ME TO PUT ON THE EMERGENCY BRAKE BECAUSE THE CAR WAS STILL SLIDING. I KNOW I TRIED TO STOP AND THE CAR WOULDN'T STOP. BASED ON THEIR REPORTS OF TOYOTA'S STICKING ACCELERATOR, I THOUGHT THIS SHOULD BE REPORTED AND INVESTIGATED. I JUST CAME HOME LAST WEEK. I BROKE 4 RIBS IN THE ACCIDENT AND THE CAR IS TOTALED. I WAS IN REHABILITATION FOR 2 1/2 WEEKS. I WOULD LIKE TO SEE THIS ACCIDENT INVESTIGATED. I HAVE A CLEAN DRIVING RECORD AND I BELIEVE THERE MAY HAVE BEEN SOME TYPE OF MECHANICAL ISSUE THAT WOULD NOT LET ME STOP THE CAR. GOING WESTBOUND ON PORT SAINT LUCIE BLVD, FL 34953, A WHITE TRUCK WITH A SIX TO EIGHT FOOT TRAILER MADE A TURN FROM EASTBOUND TO WESTBOUND, CUTTING MY 2009 TOYOTA YARIS OFF, BECAUSE OF THE CLOSE PROXIMITY I WAS NOT ABLE TO STOP TRAVELING THE SPEED LIMIT, THEREFORE, HAVING A FRONTAL CRASH WHICH NONE OF MY FOUR AIRBAGS DEPLOYED. I WAS TAKING TO THE SAINT LUCIE HOSPITAL TO CHECK FOR INJURIES, WHICH I HAVE WHIPLASH, AND CONTUSION OF MY LOWER LIMB, AND BRUISES WHERE MY SEAT BELT WAS. *TR
I WAS INVOLVED IN A HEAD ON COLLISION INTO A UTILITY POLE ON A TWO LANE ROADWAY. I WAS FOUND UNCONSCIOUS IN MY CAR. I HAVE NO RECOLLECTION OF THE COLLISION ITSELF BUT NO OTHER CAR WAS FOUND TO BE INVOLVED. THE DRIVERS SIDE OF THE VEHICLE EXPERIENCED THE MOST DAMAGE. I HAD MY SEATS BELT ON BUT NOT A SINGLE AIRBAG IN THE ENTIRE VEHICLE DEPLOYED, DESPITE THE DISABLING DAMAGE THAT OCCURRED TO THE VEHICLE. I HAVE REACHED OUT TO TOYOTA MULTIPLE TIMES SINCE THE INCIDENT OCCURRED YET ABOUT THE CONCERN OF NO AIRBAGS DEPLOYING YET WAS NEVER NOTIFIED THAT THIS WAS EVEN AN OPTION TO REPORT THE FAILURE OF THE AIRBAG TO DEPLOY IN AN ACCIDENT, ESPECIALLY WHERE THE HEAD ON COLLISION TOTALED THE CAR. THE PICTURES OF THE INCIDENT ATTACHED TO THIS REPORT WERE OBTAINED BY MY FATHER IN THE DAYS FOLLOWING THE ACCIDENT AND INCLUDE THE ROAD OF THE ACCIDENT AND THE CAR. THE STEERING WHEEL IS VISIBLY DEFORMED FROM THE IMPACT MY BODY HAD ON IT DURING THE CRASH. I FRACTURED THE C2 SEGMENT IN MY NECK AND TO THIS DAY STILL SUFFER FROM COMPLICATIONS FROM THE CONCUSSION AND INJURIES OF THIS CRASH ON A DAILY BASIS. I HAVE BEEN THROUGH NEUROLOGICALLY TESTING AND THERAPIES RELATED TO THE INJURIES. THESE INJURIES HAVE IMPACTED MY LIFE GREATLY AND I FEEL AS THOUGH MY CONCERN IS BEING DISREGARDED BY TOYOTA. I STRONGLY FEEL AS THOUGH MY INJURIES WOULD NOT HAVE BEEN NEARLY AS SEVERE IF THE AIRBAG DEPLOYED AND I WOULD REALLY APPRECIATE IT IF YOU COULD LOOK INTO THE CASE. TL* THE CONTACT OWNS A 2009 TOYOTA YARIS. WHILE DRIVING APPROXIMATELY 25 MPH, THE DRIVER NOTICED SMOKE AROUND THE FRONT DRIVER SIDE DOOR WINDOW. THE CONTACT COASTED THE VEHICLE TO THE SIDE OF THE ROAD AND TURNED OFF THE ENGINE. A CERTIFIED INDEPENDENT MECHANIC CAME OUT TO INSPECT THE VEHICLE AND NOTICED THAT THE CIRCUIT BOARD WAS SMOKING. THE MECHANIC DISCONNECTED THE CIRCUIT BOARD TO PREVENT THE VEHICLE FROM CATCHING ON FIRE. THE VEHICLE WAS DRIVEN TO THE CONTACT'S RESIDENCE. THE VEHICLE WAS NOT TAKEN TO A DEALER. THE VIN WAS NOT INCLUDED IN NHTSA CAMPAIGN NUMBER: 15V689000 (VISIBILITY). THE MANUFACTURER WAS NOT CONTACTED. THE APPROXIMATE FAILURE MILEAGE WAS 70,185.
