Was driving my sienna down he highway, as I was passing someone I lightly accelerated and I heard a loud bang sound. Seconds later I see smoke coming out of the hood so I pull over, about 30 seconds before I pulled over the engine bursted into flames. When the vehicle came to a stop it would not unlock and me and my 2 kids were stuck in a burning vehicle, we barley made it out and my arm got burnt from it, this is the 4th time I’ve had a problem with the engine and 6th time the transmission has had problems. I’ve heard this happen on many other siennas yet it’s not recalled…
TRAVELING 45 MPH CROSSING EAST A 3 LANE INTERSECTION WITH A GREEN LIGHT WHEN ANOTHER VEHICLE GOING SOUTH RUN A RED LIGHT. SINCE I HAD THE RIGHT OF WAY AND MY VAN WAS AT STEADY SPEED OF 45 MPH, I HAD NO TIME TO REACT TO PREVENT FROM HITTING THE INCOMING CAR. I HIT THE OTHER VEHICLE'S PASSENGER SIDE DAMAGING BOTH FRONT AND REAR DOOR. THE IMPACT TOTALLY RUINED THE FROM OF MY 2014 TOYOTA SIENNA VAN WITH JUST 6277 MILES. MY INSURANCE COMPANY'S ADJUSTER DECLARED THE VAN A TOTAL LOST. WITH THAT STRONG IMPACT THAT TOTALLY DESTROY THE FRONT OF MY VEHICLE, I WAS SURPRISED THAT THE AIR BAG IN THE DRIVER SIDE DID NOT DEPLOY. SO, I AM REPORTING THIS INCIDENT FOR POSSIBLE PRODUCT DEFECT ON AIR BAGS INSTALLED IN MY 2014 TOYOTA SIENNA LE VAN TO PREVENT FUTURE INCIDENT ON AIR BAG FAILURE. *TR
REAR SEATBELT WOULD NOT RELEASE AND BECAME ONLY TIGHTER WHEN WE TRIED TO RELEASE IT. A CHILD ALMOST SUFFOCATED BECAUSE IT WAS SO TIGHT AROUND HER STOMACH THAT SHE COULD NOT BREATHE. WE HAD TO CUT THE SEAT BELT. NOW, WE ARE MISSING A SEAT BELT AND TOYOTA REFUSED TO ACKNOWLEDGE THAT IT WAS A DEFECT AND/OR REPLACE IT.
AFTER A SHORT 10-MIN DRIVE, PARKED THE VAN IN MY HOUSE'S DRIVEWAY. wHEN I STOPPED, SMOKE STARTED COMING OUT OF THE HOOD. WHICH DEVELOPED INTO FLAMES AND THE FRONT OF THE VAN WAS ENGULFED IN FLAMES. iMMEDIATELY CALLED THE FIRE DEPARTMENT TO PUT THE FIRE OUT. VAN IS TOTALY BURNT. MANAGED TO MOVE TWO OTHER VEHICLES AWAY FROM THE BURNING VAN. ALUMINUM SIDING NEXT TO THE GARAGE SUSTAINED HEAT DAMAGE.
ON OCTOBER 15TH AT APPROXIMATELY 3:30 PM (CT) MY 2014 TOYOTA SIENNA WAS PARKED IN MY DRIVEWAY WITH THE ENGINE ON, UNATTENDED, WHEN IT MOVED FORWARD UP THE SLIGHT INCLINE OF MY DRIVEWAY, STRUCK ANOTHER VEHICLE THAT WAS PARKED IN THE GARAGE AND KEPT MOVING FORWARD UNTIL IT STRUCK THE FRONT WALL OF THE GARAGE. PRIOR TO THE ACCIDENT, I HAD REVERSED THE VAN OUT OF THE GARAGE AND LEFT THE VAN IGNITION ON AND IN PARK AS I EXITED. I KNOW FOR CERTAIN THAT THE VAN WAS IN THE PARK POSITION SINCE THE AUTOMATIC LOCKS LET ME EXIT THE VEHICLE WITHOUT HAVING TO MANUALLY UNLOCK THEM AND THE ALARMS DID NOT SOUND AS I EXITED. THE VAN ALSO DID NOT ROLL FORWARD IR BACKWARD AS I EXITED. I THEN WALKED IN FRONT OF THE VAN TO ENTER MY HOUSE THROUGH THE GARAGE. I WAS IN THE HOUSE FOR LESS THAN 5 MINUTES WHEN I HEARD THE LOUD CRASH OF THE VAN STRIKING THE GARAGE WALL. MY TOYOTA SIENNA WAS MANUFACTURED ON JUNE 6, 2014. AFTER THE ACCIDENT I DISCOVERED THERE IS AN ACTIVE RECALL (RECALL: TRANSMISSION SHIFT CABLE/ROLLAWAY/FMVSS 102) FOR AN ISSUE THAT CAUSES ROLLAWAYS EFFECTING CARS MANUFACTURED BETWEEN JUNE 17, 2014 THROUGH JUNE 24, 2014. I TOOK MY VAN TO TOYOTA EXPECTING MY ISSUE TO BE RELATED TO THIS RECALL. TOYOTA HAD THE BLACK BOX INSPECTED AND THE TRANSMISSION INSPECTED AND DETERMINED THEY WERE NOT LIABLE SINCE THEY WERE NOT ABLE TO REPLICATE THE ISSUE AND DID NOT FIND ANY DEFECTS. WHEN I TOOK MY CAR IN ORIGINALLY THE TECHNICIAN STATED THAT THERE WAS NOT A GOOD WAY TO DETERMINE IF THE SHIFT CABLE WAS DEFECTIVE AND THEY WERE SIMPLY REPLACING THE PART AS PART OF THE RECALL, WHICH MY CAR DID NOT QUALIFY FOR SINCE IT WAS MANUFACTURED 11 DAYS PRIOR. A CAR SHOULD NEVER MOVE ITSELF FROM THE PARK POSITION AND SOMETHING HAS TO BE FAULTY WITH THE CAR IN ORDER FOR IT TO DO SO.
ON 8/2/18, WHILE DRIVING MY 2014 TOYOTA SIENNA VAN, BOUGHT NEW FROM A LOCAL DEALER WITH APPR. 89,000 MILES AND METICULOUSLY MAINTAINED FOR ALL SERVICE, I WAS DRIVING ON A CITY STREET WHEN THE BRAKES FAILED AFTER PUMPING THEM THREE TIMES. THE BRAKE FAILURE CAUSED MY VEHICLE TO REAR-END THE VEHICLE I WAS FOLLOWING. NO INJURIES, BUT THE VAN WAS TOTALED. NO CITATION WAS ISSUED BY NORMAN, OK. POLICE, ACCIDENT REPORT #2018-57682. PASSENGERS IN MY VAN WITNESSED THE BRAKE FAILURE.
WHILE IN MOTION THE STEERING LOCKED UP AND MADE THE VEHICLE UNSTEERABLE LEADING TO A COLLISION
I WAS DRIVING DOWN I-95 TO FLORIDA FROM NC, TRAVELING 70 MPH IN THE LEFT LANE WHEN I NOTICED A CAR COMING UP FROM BEHIND DRIVING ALMOST ON THE CENTER LINE. I STAYED IN MY LANE AND THE CAR CAME UP AND IMPACTED THE RIGHT SIDE OF MY VEHICLE, ROTATING THE CAR 90 DEGREES AND SENDING ME AND MY FAMILY OFF THE INTERSTATE ONTO THE RIGHT SHOULDER. THE SIDE AIRBAG DID NOT DEPLOY EVEN THOUGH THE POINT OF IMPACT RESULTED IN DAMAGE TO THE PASSENGER AND SIDE DOORS OF THE MINIVAN. *TR
I PARKED MY CAR ON AN UPWARD INCLINING DRIVEWAY AND LEFT IT IN IDLE. I WENT INSIDE THE HOUSE TO GO AND GET MY DOG AND SPEAK TO MY IN-LAWS. I BROUGHT THE KENNEL OUT TO THE CAR, LIFTED THE TAILGATE AND PUT THE KENNEL IN. I WENT BACK IN THE HOUSE, GOT THE DOG FOOD, PUT THE DOG FOOD IN THE TAILGATE. I WENT BACK INSIDE AND WAS SPEAKING TO MY IN-LAWS WHEN ALL OF A SUDDEN WE HEARD A LOUD CRASH. WE WENT OUTSIDE AND MY CAR HAD ACCELERATED AND CRASHED INTO THEIR GARAGE. THERE WAS NO ONE IN THE CAR AT THE TIME OF THE ACCELERATION. THE DOG WAS STILL IN THE HOUSE WITH US. THE CAR WAS IN PARK. THE TAILGATE WILL NOT LIFT UNLESS THE CAR IS IN PARK WHILE IDLING. WHEN THE CAR IS IN DRIVE, THE DOORS LOCK. SO, WE KNOW IT WAS IN PARK. THE CAR CAUSED EXTENSIVE DAMAGE TO THE GARAGE FROM THE FORCE OF THE ACCELERATION. The fuel lines that are connected to recall 14V576000 need to be replaced and so that recall needs to be expanded as the parts are present in my vehicle but the connected to my VIN

The contact owns a 2014 Toyota Sienna. The contact stated that while driving at various speeds the vehicle had experienced unintended acceleration. During the failure the driver forcely pressed the brake pedal to stop the vehicle. The contact indicated that the failure had once occurred while a transporting a Uber passenger. The cause of the failure was not determined. The manufacturer and local dealer were not contacted. The failure mileage was 120,000.
Odometer Fraud. The contact purchased a 2014 Toyota Sienna. The contact discovered a mileage discrepancy after the purchase. The vehicle was a private sale. At the time of the purchase, the mileage was 59,500. It was later discovered upon checking the paperwork that the mileage was 176,047.
The vehicle died while driving and when I try to start again it will crank but can't start.
Dear sir/ madam, I am writing to report an issue with recall status reporting. All open recalls for my vehicle (VIN: [XXX] ) were completed at an authorized dealership on [01/27/2026]. I have documentation from the dealer confirming that the recall repairs were successfully performed. However, the recall status has not been updated in the NHTSA database and still appears as “open.” I request assistance in ensuring the recall completion is properly reported and reflected in the NHTSA system. Vantage Mobility International, LLC (Vantage) is recalling certain 2012-2018 AMS Genesis wheelchair vans, built on a Toyota Sienna vehicles. The rubber brake hoses that attach to the rear wheel assembly may be too short, causing them to rub against the rear axle trailing arm.
Remedy: Vantage will notify owners, and dealers will install a set of rear brake hoses that will provide the proper clearance, free of charge. The recall began December 16, 2019. Owners may contact Vantage customer service at 1-800-488-9082.
Braun Corporation (Braun) is recalling certain 2014-2019 Toyota Sienna mobility van conversions. The brake lines may have incorrect tube nut fittings installed, which may cause a pressure drop within the braking system.
Remedy: Braun has notified owners, and dealers will install an adaptor to correct the issue, free of charge. The recall began June 25, 2019. Owners may contact Braun customer service at 1-800-488-0359.

Remedy: Toyota will notify owners. Depending on the vehicle model, dealers will replace the front passenger air bag inflator, or replace the air bag assembly. The recall began February 15, 2019. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2011-2016 Toyota Sienna minivans manufactured January 4, 2010, to August 12, 2016. If the power sliding door is unable to be opened when commanded, such as if the door is frozen shut, the door may subsequently open unexpectedly, possibly while the vehicle is moving.
Remedy: Toyota will notify owners, and dealers will replace the junction block, rear sliding door wire harnesses, and install two sub wire harnesses, free of charge. The recall began on July 28, 2017. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is G04.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2014 Toyota Avalon, Camry, Sienna, and Highlander and model year 2015 Lexus RX350 vehicles. Fuel may leak from the one of the fuel delivery pipes in the engine compartment.
Remedy: Toyota will notify owners, and dealers will replace any of the suspect fuel delivery pipes free of charge. The recall began November 17, 2014. Owners may contact Toyota customer service at 1-800-331-4331.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2014 Sienna vehicles manufactured June 17, 2014, through June 24, 2014. The affected vehicles have a transmission shift control cable whose protective cable sleeve may have been damaged during assembly. This could cause the control cable to bind or separate during gear shift operation causing incorrect gear selection. As such, these vehicles fail to comply with the requirements of the Federal Motor Vehicle Safety Standard No. 102, "Transmission Shift Lever Sequence, Starter Interlock, and Transmission Braking Effect."
Remedy: Toyota will notify owners, and dealers will inspect and replace the transmission shift control cable, as necessary, free of charge. The recall began in early August 2014. Owners may contact Toyota customer service at 1-800-331-4331.
