I AM AN EXCELLENT DRIVER. I PURCHASED THIS PRIUS IN THE SUMMER OF 2011. WE (MY WIFE AND YOUNG DAUGHTER) WERE PROCEEDING ON JUANITA DRIVE N.E. IN KIRKLAND, WA. MY WIFE AND I AGREE THAT OUR SPEED WAS APPROXIMATELY 30-33 MPH AND THAT THERE WERE APPROXIMATELY 3+ CAR LENGTHS BETWEEN US AND THE VEHICLE IN FRONT OF US. THE WEATHER WAS DRY AND SUNNY. SUDDENLY, A VEHICLE THREE VEHICLES AHEAD OF US STOPPED TO MAKE A LEFT TURN, AND THE TWO VEHICLES BEHIND IT WERE ABLE TO STOP. I IMMEDIATELY APPLIED MY BRAKES TO THE FLOOR AS SOON AS I OBSERVED THE STOPPED VEHICLE AHEAD. I FELT NO RESPONSE - NO GRIPPING OF THE BRAKES OR GRIPPING OF THE WHEELS TO THE ROAD, NO SQUEALING, NO SKIDDING, NO SENSE OF THE BRAKES SLOWING THE VEHICLE IN THE THREE SECONDS I APPLIED THEM. DUE TO VEHICLES IN THE OPPOSING LANE, A NARROW ROAD AND NO SHOULDER (A DITCH WITH TELEPHONE POLES ON THE RIGHT), I WAS FORCED TO DESIGN THE LEAST HARMFUL IMPACT - MY FRONT RIGHT CORNER TO THE REAR LEFT CORNER OF THE VEHICLE IN FRONT. THE IMPACT TORE OPEN THE RIGHT FRONT OF THE VEHICLE, SHATTERING THE WINDSHIELD NEXT TO MY WIFE AND CAUSING HER LEGS TO FLY UP AND STRIKE THE DASHBOARD, INJURING HER LEGS IN SEVERAL PLACES. HER AIRBAG DID NOT DEPLOY. MY 7-YEAR-OLD WAS IN HER CAR SEAT IN THE BACK SEAT. THE LARGER SPLIT BACK WAS BEHIND HER. THE BACK BECAME UNHINGED AT IMPACT AND FLIPPED FORWARD, FORCING MY DAUGHTER'S UPPER BODY FORWARD. SHE SUSTAINED MINOR FACIAL INJURIES. The contact owned a 2011 Toyota Prius. The contact stated while driving at an undisclosed speed in an intersection, a Ford F-150 vehicle crashed into the front driver's side of the vehicle. The contact stated he was pinned between the door and the infotainment system. The airbags failed to deploy. No warning lights were illuminated. The contact was taken to the local hospital by a friend in a private vehicle, and the contact received medical assistance. The contact sustained internal bruising and external bruising and cuts. Additionally, the contact sustained a major injury was on the neck, on the laryngeal prominence area of the neck and on the collar bone. The medical Doctor informed the contact that the major injury could heal in 4 weeks, or in 7-8 months. The contact was also informed that the major injury could become a permanent condition, causing cramps and stiffness depending on the weather. The contact was experiencing trouble sleeping due to the injuries. The vehicle was towed to a local tow lot and was totaled. A Police report was filed. The leasing company the vehicle was purchased from was notified of the failure. The contact related the failure to an unknown air bag recall. The manufacturer was not made aware of the failure. The failure mileage was approximately 325,000.

ON Sept 9th, 2022, I was involved in a collision, traveling approximately 30 - 40 MPH and struck a vehicle that pulled into my path. I struck the other vehicle perpendicularly (T-Boned). My vehicle impacted on the front, initially, and then on the driver's side at full force. The result of the accident caused the front bumper to be damaged and the entire left side of the car was crushed. The force of the accident caused the driver's window and port-hole to shatter. The AIR BAGS DID NOT DEPLOY. None of the air bags deployed. The vehicle is still in the auto repair lot and has yet to be inspected by anyone. There we NO prior indicators of any problems with the airbag system.

The contact owns a 2011 Toyota Prius. The contact stated that she was hit by another vehicle while turning left onto a highway at 5 MPH. The contact was hit on her driver-side door at approximately 60 MPH and the airbags did not deploy upon impact. The contact stated that the driver seat had collapsed backward onto the rear driver seat which sent her over the steering wheel and into the windshield. The contact lost consciousness upon impact and when she came to, her hands were dangling over the steering wheel with her head pinned to the windshield glass. The contact was removed from her vehicle by authorities and was transported to the hospital by ambulance. The contact was treated for a broken sternum, four broken ribs, two on both the left and right side of her body, and severe bruising on both the left and right side of her entire body. The contact stated that five occupants inside the other vehicle were all transported to the hospital including a four-year-old child whose car seat broke upon impact. The whereabouts and injuries of the occupants are unknown. A police report was filed at the scene. The vehicle was towed to an independent tow yard but had yet to be destroyed. The dealer nor the manufacturer had yet to be notified of the failure. The failure mileage was 117,000. The VIN was not available.

MY NOSE HAD A SIGNIFICANT RIP IN TWO LOCATIONS AND WAS TRANSPORTED BY AMBULANCE FOR PLASTIC SURGERY. WITHIN 24 HRS MY ENTIRE FACE WAS SWOLLEN WITH A MAJORITY OF DARK BRUISING FROM MY EYES TO MY JAWS. AND MULTIPLE STITCHES IN MY NOSE. MY SEATBELT FAILED TO RESTRAIN ME BACK. THE AIRBAGS DIDN'T DEPLOY. ACCORDING TO THE AUTO SHOP THE AIRBAGS DIDN'T DEPLOY BC IT HAD MISSED THE SENSOR BY AN INCH. THE DAMAGE TO THE VEHICLE WAS THE FRONT CENTER OF THE VEHICLE AND THE ENTIRE PASSENGER FRONT END. THE HOOD ROLLED UP AND EVEN PARTIALLY CAME THRU THE WINDSHIELD. THE CAR WAS TOTALED FROM THE IMPACT.THEY STATED TO THEM THERE IS NO REASON THE SEAT BELT DIDN'T RESTRAIN. HOWEVER AFTER CONTACTING TOYOTA THEY DID AN INSPECTION ROUGHLY THE MIDDLE OF JANUARY AND I RECEIVED A COPY OF THE DATA REPORT THAT SHOWED MY MPH AND PROOF I WAS RESTRAINED AND MORE INFORMATION THAT AN AVERAGE PERSON WOULDN'T UNDERSTAND. THE END OF MARCH 2013 I RECEIVED A RESPONSE STATED THE SEATBELT WORKED WITHIN THE LEGAL GUIDELINES. MY THOUGHTS WERE NO WAY, WHAT WOULD BE THE PURPOSE TO WEAR THE SEATBELT IF YOU CAN STILL BE SERIOUSLY INJURED AND THEN THANKING GOD THAT MY DAUGHTER WASN'T IN THE FRONT SEAT AND NEVER HAD BEEN IN THE CAR. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2010-2014 Toyota Prius and 2012-2014 Toyota Prius V vehicles. Excessive voltage in the Intelligent Power Module (IPM) within the inverter may cause the hybrid system to shut down, causing the vehicle to stall while being driven.
Abhilfe: Toyota will notify owners, and dealers will update the software for the motor/generator control electronic control unit (ECU) and the hybrid control ECU as necessary, free of charge. The recall began January 22, 2019. Owners may contact Toyota customer service at 1-888-270-9371. Toyota's number for this recall is J0V. Note: The software updates in this recall are needed even if the vehicle had the software previously updated under recalls 14V-053 or 15V-449.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2010-2012 Toyota Prius, 2010 and 2012 Toyota Prius Plug-in Hybrid, and 2011-2012 Lexus CT200h vehicles. A weld in the curtain shield air bag inflator may crack causing one or both portions of the inflator to eject into the passenger cabin.
Abhilfe: Toyota will notify owners, and dealers will install retention brackets to the curtain shield air bags, free of charge. The recall began on October 17, 2016. Owners may contact Toyota Customer Service Department at 1-800-331-4331. Toyota's number for this recall is Toyota G0U, and Lexus GLJ.

Southeast Toyota Distributors, LLC (SET) is recalling certain 2005-2011 4Runner, Highlander, Sequoia, and Sienna, 2005-2010 Avalon and Tundra, 2006-2008 Camry Solara, 2006-2010 Highlander hybrid, 2006-2011 Rav4 and Tacoma, 2007-2010 FJ Cruiser, 2007-2011 Camry hybrid, 2009-2010 Venza, 2010-2012 Prius, 1988-1990 and 2005-2010 Camry, and 1989 and 2009-2010 Corolla vehicles equipped with aftermarket accessory seat heaters with a copper strand heating element. The electrical wiring in the seat heaters may be damaged when the seat cushion is compressed.
Abhilfe: SET will notify owners, and dealers will disconnect the seat heaters, free of charge, and refund the purchase price of the seat heater accessory. The recall began on July 14, 2016. Owners may contact SET customer service at 1-866-405-4226. SET number for this recall is SET16B.

HYBRID PROPULSION SYSTEM: INVERTER
Toyota is recalling certain model year 2010 through 2014 Prius vehicles. In the affected vehicles, the Intelligent Power Module (IPM) inside the inverter module (a component of the hybrid system) contains transistors that may become damaged from high operating temperatures. If this occurs, various warning lamps will be illuminated on the instrument panel and the vehicle will have reduced power allowing it to only drive a short distance.
Abhilfe: Toyota will notify owners and dealers will update the software for both the motor/generator control electronic control unit (ECU) and the hybrid control ECU, free of charge. If an owner experiences a failure of the inverter before the vehicle receives updated software, the dealer will repair or replace the inverter assembly with a new one at no charge. Southeast Toyota Distributors, LLC (SET) is recalling certain models interspersed through model years 2009 through 2013 as follows: model year 2009-2012 Tacoma, 4Runner, Camry, Camry Hybrid, Prius, and RAV4; model year 2009-2010 Avalon, FJ Cruiser, and Highlander Hybrid; model year 2010-2013 model year Corolla, Sienna and Tundra; model year 2009-2013 Highlander and Venza; model year 2012 Prius V; and model year 2010-2012 Sequoia. During modification by SET to include accessories such as leather seat covers, seat heaters or headrest DVD systems, these vehicles may not have had the passenger seat occupant sensing system calibration tested. Without passing the calibration test, the occupant sensing system may not operate as designed.
Abhilfe: Southeast Toyota will notify owners, and dealers will test the sensitivity of the occupant detection sensors, and recalibrate them as necessary. The recall began on March 21, 2013. Owners may contact Southeast Toyota at 1-800-301-6859.
