Driving on the highway was side swiped then rearended at high speed causing my driver seat to fly backwards. So the locking mechanism on the manual seat adjuster either failed or never locked
Car was involved in a head on collision [XXX] with a concrete exit barrier after being pushed into the lane by a driver trying to in front of them trying to make a last second dash for the exit. Not sure if the driver was in blind spot, but the vehicle trying to merge did use turn signal or respond to horn honking from myself. In order to avoid the hitting the other driver, who was unaware of my car while aggressively merging, I tried driving into the safety lane, but at 65 MPH could not stop in time from hitting the barrier My safety was put at risk because upon impact the driver airbag did not deploy. The entire front end of the car crumbled and the car was declared totaled by the driver's insurance. The airbag deployment could have prevented more serious injury to the driver, by did not deploy. The driver was wearing their seat belt and suffered a bruised sternum and PTSD. Driver called police who did not file a report due to no damage to another vehicle or property or several injury to any persons. Airbag was part of the takata airbag recall for Subaru. Airbags are on record of being replaced at a Subaru dealership Redwood City, California- driver verified post crash. There were no warning lamps indicated the airbag was disabled. Driver alerted Subaru's quality control hotline who inspected the car on [XXX]. No word has been returned on why the airbag did not deploy during the crash. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
TL* THE CONTACT OWNS A 2012 SUBARU OUTBACK. THE CONTACT STATED THAT WHILE THEIR SON WAS DRIVING APPROXIMATELY 10 MPH, WHEN THE DRIVER ATTEMPTED TO APPLY THE BRAKE PEDAL HOWEVER THE BRAKES FAILED. THE DRIVER HAD TO DROP THE VEHICLE INTO FIRST GEAR THEN CRASHED INTO THE CURB WHERE IT CAME TO A STOP. THE AIR BAGS DID NOT DEPLOY. THE CONTACT STATED THAT THE DRIVER RECEIVED A CHEST INJURY HOWEVER THERE WAS NO MEDICAL ATTENTION NEEDED. THE REAR PASSENGERS ALL OF WHICH WERE CHILDREN WERE NOT INJURED BUT ALL CAR SEATS NEEDED TO BE REPLACED. THE VEHICLE WAS TOWED. A POLICE REPORT WAS NOT FILED. THE VEHICLE WAS TAKEN TO THE DEALER (WALKER MOTORS INC 265 RIVER ST, MONTPELIER, VT 05602; (802) 229-0391) WHO DIAGNOSED THE BRAKE LINE HAD CORRODED. THE VEHICLE'S BRAKE LINE WAS NOT REPAIRED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND CONFIRMED THAT THEIR VIN WAS NOT INCLUDED IN ANY RECALL. THE APPROXIMATE FAILURE MILEAGE WAS 112,000.
SUBARU OUTBACK (2012), WHEN STOPPED FOR 1-2 MINUTES, WITH THE ENGINE AND IGNITION OFF, AND THE GEAR SHIFT IN PARK, ON A SLIGHT INCLINE (TO CHECK A POTENTIAL SAFETY HAZARD) SUDDENLY ROLLED BACKWARDS AS I WAS LEANING IN TO RETRIEVE MY BAG. I WAS DRAGGED ACROSS THE NORMALLY BUSY ROAD, BARELY ABLE TO HANG ON (BY BOTH HANDS GRABBING ANYTHING I COULD INSIDE THE CAR). MY LEGS SCRAPED ON THE GROUND VIOLENTLY BURNING THOUGH MY SNEAKER THEN ANKLE FLESH AND GRAZED THE BONE BADLY. MY LEGS ON THE ROAD TURNED THE 1.5 TON VEHICLE A FEW DEGREES AS IT CAREENED UNDER RAPID ACCELERATION REACHING AN ESTIMATED 8-10 MPH BUMPING OVER 2 KERBS PULLED ME EVEN FURTHER OUT OF THE CAR, BUT STILL HANGING ON. LUCKILY THE OPEN DRIVER'S DOOR ENGAGED WITH A HEDGE WHICH RIPPED THE DOOR ALMOST OFF THE CAR AND TURNED THE CAR ANOTHER 35 DEGREES OR SO, AVOIDING A VIOLENT CRASH INTO THE NEIGHBOR'S HOUSE. INSTEAD THE CAR GRAZED A WOODEN POST (FURTHER SLOWING IT) THEN HIT A SECOND POST WHICH BROUGHT THE CAR TO A HARD STOP. THIS PUSHED ME OUT OF THE CAR ONTO A PATCH OF GRASS, BACK TO THE CAR. AN ENGINEERING ASSESSMENT LATER REVEALED THE SHIFT-INTERLOCK HAD FAILED (INVOLVING A COUPLE OF PARTS) SO THAT THE GEAR MAY BE SHIFTED (OR SHIFT ITSELF) WHEN THE ENGINE AND IGNITION ARE OFF WITH THE KEYS STILL IN. MY INJURIES INCLUDE ANKLE (ON A LEG WITH A HIP REPLACEMENT), PTSD (DIAGNOSED AS CAUSED 100% BY THE CRASH) AND EXACERBATION OF MULTIPLE-SCLEROSIS CAUSING DEBILITATING BOUTS OF TRIGEMINAL NEURALGIA AND WITH ADDITIONAL BRAIN DAMAGE (PROVEN BY INCREASED LESION LOAD ON MRI SCAN). THEREFORE THE VEHICLE FAILURE ON THIS MODEL IS ALSO POTENTIALLY DANGEROUS TO OTHER OWNERS OF THE SAME MODEL CAR. WE ALSO RESEARCHED AND FOUND ONE ALMOST IDENTICAL CASE WITH THE SAME MODEL, THE SAME ROLL-IN-PARK BUT FAR MORE SERIOUS INJURY AS THE DRIVER WAS KNOCKED DOWN BY THE DOOR, APPARENTLY AS THE CAR ROLLED OVER.
TL* THE CONTACT OWNS A 2012 SUBARU OUTBACK. WHILE THE VEHICLE WAS PLACED IN PARK ON AN INCLINE, THE CONTACT SUDDENLY HEARD A SCRAPING NOISE AND NOTICED THAT THE VEHICLE INDEPENDENTLY ROLLED AWAY. THE CONTACT'S WIFE WAS LEANING INTO THE VEHICLE AND WAS PULLED IN REVERSE ALONG WITH THE VEHICLE. THE VEHICLE CRASHED INTO A CURB AND THE FRONT DRIVER'S DOOR STRUCK THE HEDGE, WHICH SLOWED THE VEHICLE DOWN. THE VEHICLE THEN CRASHED INTO A TREE HEDGE. THE CONTACT'S WIFE SUSTAINED AN INJURY TO THE RIGHT SIDE OF HER HIP THAT REQUIRED MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE DEALER WAS NOT CONTACTED. THE VEHICLE WAS TOWED TO AN AUTO SHOP WHERE ALL OF THE PANELS WERE REPAIRED; THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. AN INSPECTOR WAS SENT TO INSPECT THE VEHICLE, BUT NO DIAGNOSTIC FAILURE CODES WERE FOUND AND THE FAILURE COULD NOT BE DUPLICATED. THE APPROXIMATE FAILURE MILEAGE WAS 47,000. *TT *TR *BF *TR

TL* TAKATA RECALL. THE CONTACT OWNS A 2012 SUBARU OUTBACK. WHILE DRIVING APPROXIMATELY 35 MPH, ANOTHER VEHICLE CRASHED INTO THE FRONT DRIVER SIDE OF THE CONTACT'S VEHICLE AT A HIGH RATE OF SPEED. THE AIR BAGS FAILED TO DEPLOY. THE DRIVER SUSTAINED PAINS TO THE NECK, CHIN, AND COLLAR BONE. THE AIR BAGS FAILED TO DEPLOY. THE CONTACT RECEIVED MEDICAL TREATMENT. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO A YARD BY THE INSURANCE COMPANY. THE VIN WAS INCLUDED IN NHTSA CAMPAIGN NUMBER: 17V014000 (AIR BAGS), BUT THE CONTACT WAS NOT AWARE OF THE RECALL. THE MANUFACTURER WAS NOTIFIED. THE APPROXIMATE FAILURE MILEAGE WAS 72,000. PARTS DISTRIBUTION DISCONNECT.
TRAFFIC STOPPED ABRUPTLY AND MY CAR (SUBARU OUTBACK) CAME TO A STOP. I WAS REAR-ENDED BY A SEMI (SPEED UNKNOWN BUT DID SIGNIFICANT DAMAGE TO MY VEHICLE) THAT PUSHED ME INTO THE VEHICLE IN FRONT OF ME. THE VEHICLE IN FRONT OF ME WAS THEN PUSHED INTO ANOTHER VEHICLE. THE VEHICLE DIRECTLY IN FRONT OF ME (FORD FREE STAR)HAD ITS AIR BAG DEPLOY. MY AIR BAG DID NOT DEPLOY AND MY HEAD ENDED UP HITTING THE STEERING WHEEL. I WAS DIAGNOSED WITH A CONCUSSION. I AM WONDERING IF MY AIR BAG SHOULD HAVE DEPLOYED IN THIS TYPE OF SITUATION? *TR
MY SUBARU WAS HIT HEAD ON DRIVER'S SIDE BY ANOTHER CAR GOING 35-40 MPH. MY SUBARU'S AIRBAGS DID NOT DEPLOY. EXTENSIVE DAMAGE IN 13K RANGE TO FRAME AND LEFT FRONT OF CAR. WE OWNED THE CAR ONLY 2 DAYS AND WAS OUT FOR OUR FIRST DRIVE OWNING OUR FIRST BRAND NEW CAR. THINGS SEEMED ALL GOOD UNTIL WE STOPPED AT A BANK AND I PUT MY LEFT FOOT ON THE FOOT REST AND PRESSED MY BACK INTO THE SEAT TO RELEASE THE TENSION IN MY PANTS SO I COULD PULL OUT MY WALLET WHEN ALL THE SUDDEN CATCHING ME BY TOTAL SURPRISE THE SEAT GAVE WAY. A SHOCK WENT UP AND DOWN MY SPINE AS I GOT OUT OF THE CAR IN AWE OF WHAT JUST HAPPENED AN THE CONDITION I WAS IN I HAD TO SIT IN THE PASSENGER SEAT TO SEE IF IT WAS JUST A FLUKE OR IS THIS A REAL PROBLEM AND I JUST GOT THE WORST OF IT. WITH A LITTLE PRESSURE LEANING BACK IN THE SEAT FOR A FEW MOMENTS AND SNAP ALL THESE SEATS CAN BE BROKEN. I WAS DOING GREAT AND NOW EVERYDAY I HAVE NERVE PROBLEMS AND BACK ISSUES I HAVE NEVER EXPERIENCED IN MY LIFE. I AM HEIGHT WEIGHT PROPORTIONAL AND ACTUALLY A LITTLE ON THE THIN SIDE. THE DEALERSHIP AND SUBARU OF AMERICA SWORE THIS NEVER HAPPENED BEFORE. GOOGLE AND YOUTUBE "SUBARU OUTBACK LUMBAR SUPPORT BROKE" I FOUND PROBLEMS GOING BACK TO 2008!! I ASKED THEM TO BUY THE CAR BACK BUT THEY "BENT THE CLIP A LITTLE EXTRA" AS A FIX AND INSISTED I TAKE IT HOME. IT BROKE AGAIN WHILE JUST DRIVING 40MPH DOWN THE ROAD NORMALLY SO I CONVINCED THEM BY TALKING TO THE OWNER TO GIVE ME A TRADE UP TO THE ELECTRIC SEAT. LITTLE TO KNOW THE PASSENGER SEAT IS THE SAME SEAT AS IN THE BASIC MODEL. I OWN THE PREMIUM NOW AND I WILL DO A REPORT ON IT AS WELL AS I HAVE OWNED MANY FOREIGN CARS AND NOT ONE SEAT HAS EVER BROKE ON ME. TOYOTA SEAT HOLDS UP TO ANYTHING I PUT IT THROUGH AND I HAVE 300K MILES ON IT!!! SAME WITH MY HONDA CIVIC. BOTH BOUGHT USED... PLEASE MAKE THEM FIX THIS PROBLEM! I BEG YOU. I HOPE NO ONE ELSE EVER HAS TO DEAL WITH THE PAINS I HAVE HAD FROM THIS ALONG WITH STRESS EXPLAINING THE SITUATION TO THE DEALER AND DUE TO SUCH A SIMPLE LITTLE CLIP AND STUPIDITY. *TR
On October 27, 2021 while driving the car a number of vehicle light began flashing. We checked every thing we could and proceeded to drive it home, approximately 47 miles. First thing on 10/28/2021 it was examined by a Subaru dealer service department and they found no issues other than they replaced the serpentine belt and a few light bulbs. The car was driven again on 1o/29/21 and 11/1/21 with no incident. On the way home from work on 11/3/21 the same vehicle lights began flashing. We checked everything we could and found nothing wrong. We began travelling home and about 25 miles into the trip we heard what sounded like metal clanking. Shortly thereafter we began to lose power and the engine started smoking. The engine compartment was on fire by the time we were on the side of the road so we exited the car. In around 10-15 seconds the car was fully engulfed by flames and burned to the metal.
WAS DRIVING ON HIGHWAY WHEN LIGHTS ON DASH STARTED FLASHING ON & OFF, CRUISE, BRAKE, CHECK ENGINE, THE TEMPERATURE LIGHT WAS FLASHING BLUE & RED. I PULLED OVER TO MAKE SURE EMERGENCY BRAKE WAS OFF. TURNED CAR OFF AND ON AND CHECKED GAS CAP WAS TIGHT AS I HAD GOOGLED WHY IT MIGHT BE FLASHING. I CONTINUED FOR ANOTHER MILE AND ENGINE STARTED MAKING CLICKING NOISE WHEN I PRESSED GAS. I PULLED OVER AND STOPPED, CAR LIGHTS WERE STILL FLASHING AND IT BEGAN TO SMOKE UNDER HOOD. SMOKE STARTED COMING THROUGH AIR CONDITIONING VENTS AND WE GOT OUT IMMEDIATELY AND ENGINE CAUGHT FIRE.
DRIVING ON INTERSTATE 95 THE ENGINE CAUGHT ON FIRE AND VERY QUICKLY ENGOLFED THE ENTIRE CAR - BUMPER TO BUMPER.
CAR HAD FIRE AT STARTER WITH LOTS OF SMOKE COMING FROM TRANSMISSION AND ALTERNATOR .VEHILCE HAD BEEN HAVING SHIFTING ISSUES AND TRACTION FROM ALL WHEEL DRIVE NOT WORKING. I HAD VEHICLE TAKEN TO DIFFERENT DEALERS BUT NO ONE WAS ABLE TO FIX PROBLEM UNTIL THE DAY CAR CAUGHT FIRE WHICH TOLD THEM WHICH PARTS WHERE AT FAULT. MY VEHICLE IS TRUELY A LEMON, FOR IT HAS BEEN ONE PROBLEM AFTER ANOTHER FOR THE PAST 3 YEARS

Moisture can get inside the gearbox and disrupt the electrical connections on the mode selection switch circuit board. This can result in the vehicle failing to discern the engaged gear position, causing the reverse light not to illuminate and the rearview monitor to fail to display the image from the rearview camera. This increases the risk of accidents and injuries. The product does not comply with the Regulation on the approval and market surveillance of motor vehicles and their trailers, and of systems, components and separate technical units intended for such vehicles.
