I was in a motor vehicle accident when my passenger side vehicle got t-boned on 3/22/26. My 2018 chrysler pacifica limited airbags failed to deploy and the driver and passenger sustained whiplash injuries.
Vehicle was put into reverse and did not stop, it hit a pole. Then the driver tried to put in drive to get it off of the pole and it accelerated quickly and the driver could not stop the vehicle. There was little time to react as it hit the side of a building and airbags deployed. Driver stated that the car accelerated on it's own once it was put into gear.
We were traveling at approximately 45 miles per hour, slowed down around a bend in the road and hit an ice patch. The vehicle spun towards a truck stopped at a stop sign. We hit the driver's side of the truck with the driver's side of my vehicle. We continued to spin and hit the truck again with a side impact. and spun again after the second impact. The front airbags deployed but the over head side impact airbags did NOT deploy. My wife was driving and hit her head on the drivers side window resulting in a large contusion on her forehead. The only safety notice or recall information we had for this vehicle was concerning the electric start and ignition components with the batteries that left our vehicle stranded at one point, until we had that repaired. We never had any notice or alerts of any air bag deficiencies in the vehicle. The driver's side and driver's passenger side airbags should have deployed upon the impact. We had our two little girls 2yr and 9mo old, 12 yr old, myself, my wife and grandma in the vehicle. No one else was injured in the vehicle except my wife the driver, hitting her head on the side window.
The contact owns a 2018 Chrysler Pacifica. The contact stated that while she was driving at approximately 35 MPH, she was involved in an accident after another vehicle t-boned her vehicle. The air bags failed to deploy. The contact stated that she did not require medical attention; however, she sustained a mild concussion, a knee injury, and neck pain. A police report was filed. The vehicle was towed to a tow yard by her insurance company. The manufacturer was not made aware of the failure. The approximate failure mileage was 63,500.

TL* THE CONTACT OWNED A 2008 CHRYSLER 300. WHILE DRIVING APPROXIMATELY 38 MPH, THE CONTACT'S VEHICLE CRASHED INTO A SECOND VEHICLE. THE FRONT END OF THE CONTACT'S VEHICLE WAS SEVERELY DAMAGED, BUT THE AIR BAGS DID NOT DEPLOY. THE DRIVER SUSTAINED AN INJURED LEFT SHOULDER, WHICH DID NOT REQUIRE MEDICAL ATTENTION. THE VEHICLE WAS DESTROYED AND TOWED AWAY. A POLICE REPORT WAS FILED. THE DEALER AND MANUFACTURER WERE NOT NOTIFIED OF THE FAILURE. THE CONTACT INDICATED THAT THE VEHICLE WAS SERVICED PER NHTSA CAMPAIGN NUMBER: 16V352000 (AIR BAGS) IN 2016 AND HAD THE AIR BAG INFLATER REPLACED. THE FAILURE MILEAGE WAS 79,163. *DT*JB
TL* THE CONTACT OWNS A 2018 CHRYSLER PACIFICA. WHEN THE CONTACT PULLED INTO A PARKING SPACE, THE BRAKES DID NOT STOP THE VEHICLE AND THE GEAR DID NOT SHIFT INTO PARK. AS A RESULT, THE VEHICLE CRASHED INTO A WALL. THE CONTACT HAD TO PUMP THE BRAKE PEDAL A COUPLE OF TIMES UNTIL THE VEHICLE STOPPED. THE CONTACT SUSTAINED A MINOR INJURY THAT DID NOT REQUIRE MEDICAL ATTENTION. A POLICE REPORT WAS NOT FILED. THE VEHICLE WAS DAMAGED AND DRIVEN TO SUBURBAN CHRYSLER DODGE JEEP RAM OF GARDEN CITY (734-421-5700, LOCATED AT 32850 FORD RD, GARDEN CITY, MI 48135), BUT THE CAUSE OF THE FAILURE COULD NOT BE DETERMINED. THE DEALER SUGGESTED THAT THE CONTACT MIGHT HAVE DEPRESSED THE ACCELERATOR PEDAL INSTEAD OF THE BRAKE PEDAL. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND STATED THAT THERE WAS NO FAILURE. THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE WAS APPROXIMATELY 6,000.
6/20 THE ENGINE LIGHT CAME ON. THE DEALERSHIP CLAIMED THAT THEY PERFORMED THE DTC INSPECTION AND A SYSTEM UPDATE (TO PREVENT VEHICLE FROM STALLING). THE VEHICLE WAS RETURNED ON 6/23 AND IT BROKE DOWN ON 6/24. THE CAR WENT FROM 10MPH TO 43 MPH WHEN PRESSING ON THE GAS AND THEN INSTANTLY STOPPED AT AROUND 10MPH. THE VEHICLE TURNED OFF AND WAS UNABLE TO START AND HAD TO BE TOWED BACK TO THE DEALERSHIP. I HIT A CURB AND IF I WAS NOT AN EXPERIENCED DRIVER I COULD HAVE KILLED SOMEONE OR MYSELF OR MY 3 CHILDREN COULD HAVE BEEN HARMED IF THEY WERE IN THE VEHICLE. I HAVE PROVIDED A VIDEO OF THE INCIDENT ON MY YOUTUBE AND THE DOCUMENTATION FROM WHAT THE DEALERSHIP SAID THEY PERFORMED ON THE VEHICLE. THIS CAR NEEDS TO BE RECALLED AND OFF OF THE ROAD. PLEASE SAVE LIVES BECAUSE THIS WAS VERY SCARY AND I FEAR GETTING BACK INTO THE VEHICLE IF THEY FIX IT. I DO NOT WANT IT BACK. I WANT SAFETY FOR MY CHILDREN AND MYSELF. sliding door closed on my daughters hand even though the dealership claims the sensors work fine.
When I pushed the button on the handle to close the back sliding door, my 2 year old suddenly stuck his hand in front of where the door was shutting and the door did not stop. Thankfully I was able to push the button back open almost immediately but his hand bruised immediately. We applied ice and the swelling went down but it could have been much worse hadn't I opened the door back up right away. There should be a sensor that stops the door if something is in its way.

THE CONTACT STATED THAT WHILE RETRIEVING HER DOG FROM THE BACKSEAT, THE SLIDING DOOR BEGAN TO CLOSE. THE CONTACT WAS UNAWARE AS TO WHETHER SHE DEPRESSED THE SLIDING DOOR BUTTON OR NOT. THE CONTACT'S HUSBAND HAD TO MANUALLY PULL THE DOOR BACK WHILE THE DOOR WAS CRUSHING HER LEFT ARM. THE CONTACT SUSTAINED MINOR INJURIES TO HER LEFT ARM, SORENESS AND BRUISING; HOWEVER, NO MEDICAL ATTENTION WAS NEEDED. THERE WERE NO WARNING LIGHTS ILLUMINATED RELATED TO THE FAILURE. THE SAFETY PULL ON THE SEAT BELT DID NOT LET UP WHEN UNBUCKLING. IT FORCED MY DAUGHTER INTO ALMOST SUFFOCATION AND ULTIMATELY NEEDED TO BE CUT (BACK ROW, PASSENGER SIDE). I'VE NOW NOTICED THAT THE MIDDLE ROW, MIDDLE SEAT (8 SEATER) HAS A SIMILAR ISSUE. IT CONTINUES TO TIGHTEN AND DOES NOT LET UP ON, WHAT SEEMINGLY SHOULD BE A SAFETY FEATURE. SEE ATTACHED PICTURES
AN OBJECT FELL BETWEEN THE MIDDE SEAT AND THE ELECTRONIC SLIDING DOOR SO WHEN THE DOOR WAS ELECTRONICALLY OPENED THE OBJECT WAS CAUGHT CAUSING THE DOOR TO REVERSE AND TRY TO CLOSE HOWEVER I WAS LEANING THROUGHTHE DOOR AT THAT POINT AND THE DOOR CLOSED ON MY CHEST TRIGGERING THE DOOR TO TRY AND RE-OPEN. WHEN THE DOOR STARTED TO REPON I HIT THE OBJECT AGAIN IMMEDIATELY CLOSING WITHOUT ENOUGH TIME FOR ME TO GET MY BODY OUT. THE DOOR RE-CLOSED SQUISHING ME AND STOPPED. I WAS ABLE TO PUSH THE SIDE DOOR BUTTON TO OPENAND WRIGGLED OUT BUT THERE WAS A FAILURE IN THE LOGIC IF THE DOOR HITS SOMETHING BOTH OPENING THEN CLOSING. THIS IS A SAFETY ISSUE BECAUSE IF THIS HAPPENS THE DOOR CAN CRUSH YOU.
I WAS TRAVELING ON I-95 NORTH BOUND IN SOUTH CAROLINA AT 75 MPH IN THE FAR LEFT LANE. I WENT TO SHIFT BACK TO THE CENTER LANE WHEN THE POWER STEERING COMPLETELY FAILED AND SENT US ACROSS 3 LANES OF TRAFFIC. WE ALMOST SIDESWIPED A SEMI-TRUCK. I WAS ABLE TO BREAK ENOUGH TO END UP BEHIND THAT TRUCK IN THE FAR RIGHT LANE AND REGAIN CONTROL OF THE VEHICLE. THE RED STEERING WHEEL LIGHT WAS ILLUMINATED ON THE DASH BOARD WITH A MESSAGE POWER STEERING FAILURE SERVICE REQUIRED. I WAS ABLE TO MAKE IT TO THE NEXT OFF RAMP AND ONCE ABOUT HALF WAY UP THE RAMP THE POWER STEERING STARTED WORKING AGAIN. SEVERAL HOURS LATER THE POWER STEERING WENT OUT AGAIN THIS TIME I WAS GOING STRAIGHT AT 65 MPH IN NORTH CAROLINA. IT REMAINED OUT FOR A FEW MINUTES AND THEN STARTED WORKING AGAIN. IT DID THIS 4 MORE TIME ON OUR TRIP FROM FLORIDA TO MARYLAND. ON ONE OCCASION WE WERE IN TRAFFIC IN VIRGINIA AT 5 MPH AND IT WENT OUT FOR ABOUT 10 MINUTES BEFORE WORKING AGAIN.
We were involved in a small fender bender accident, and the car caught on fire. It started with the dashboard, and it spread to the rest of the vehicle. The car ended up burnt to a crisp. Nothing salvageable.

Chrysler (FCA US, LLC) is recalling certain 2017-2023 Pacifica Plug-In Hybrid Electric vehicles (PHEVs). An internal transmission wiring connector may short circuit, resulting in an unexpected engine shutdown.
Abhilfe: Dealers will update the Power Inverter Module (PIM) software and, if necessary, update the instrument panel cluster (IPC) and the electronic control module (ECM) software, free of charge. FCA US will notify dealers and begin notifying owners on or about: 03/21/2023 for 2021 model year vehicles, 05/02/2023 for 2019-2020 model year vehicles, and 06/15/2023 for 2017-2018 model year vehicles. Chrysler (FCA US LLC) is recalling certain 2018-2019 Pacifica vehicles equipped with an 8.4 inch radio display, 360 degree surround view camera system, and seat back video screens. A software error can cause the rearview camera image to remain displayed on the radio after the vehicle has been shifted out of reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rearview Mirrors."

Abhilfe: Chrysler will notify owners, and dealers will inspect the right front knuckle and lower control arm for damage, replacing the components as necessary. The right front lower control arm knuckle pinch bolt will be replaced with a larger-diameter pinch bolt. These repairs will be performed free of charge. The recall began April 26, 2019. Chrysler (FCA US LLC) is recalling certain 2014-2018 Dodge Journey, Charger and Durango, RAM 2500, 3500, 3500 Cab Chassis (more than 10,000lb), 4500 Cab Chassis and 5500 Cab Chassis, Jeep Cherokee and Grand Cherokee and Chrysler 300, 2014-2019 RAM 1500, 2015-2018 Dodge Challenger, 2015-2017 Chrysler 200, 2016-2018 RAM 3500 Cab Chassis (less than 10,000 lb), 2017-2018 Chrysler Pacifica and 2018 Jeep Wrangler vehicles. These vehicles are being recalled to address a defect that could prevent the cruise control system from disengaging. If, when using cruise control, there is a short circuit within the vehicle's wiring, the driver may not be able to shut off the cruise control either by depressing the brake pedal or manually turning the system off once it has been engaged, resulting in either the vehicle maintaining its current speed or possibly accelerating.
Abhilfe: Chrysler will notify owners, and dealers will inspect the software, and perform a software flash on the engine or powertrain control module, free of charge. The recall began June 4, 2018. In the event that cruise control cannot be disengaged while driving, owners should firmly and steadily apply the brakes and shift the transmission to neutral, placing the vehicle in park once it has stopped.
Chrysler (FCA US LLC) is recalling various Dodge, Chrysler, and RAM vehicles equipped with Kidde Plastic-Handle or Push Button 'Pindicator' Fire Extinguishers. These extinguishers may become clogged, preventing the extinguisher from discharging as expected or requiring excessive force to activate the extinguisher.
